MESSAGE FROM CHAIRMAN, EDITORIAL COMMITTEE Welcome to 6 th issue (2018) of the ASQua Newsletter. We have been trying to bring out this issue for long but could not; however, I am happy to note that Editorial Committee could make it with the contribution of some members and the secretariat. ASQua since its inception in 2007 has witnessed a significant change including a formal registration in Malaysia. We would like to thank our President Prof. Yuichi Imanaka for his leadership and guidance. The Society is experiencing a positive change as membership is growing and members are being exposed to global expertise through participation in various conferences and activities. We woudl like to congratulate Dr. Wui-Chiang Lee and Prof. Dr. Seung-Han Suk on their appointment as the Chairman of Technical Committee and Reserach Committee respectively. I seek involvement of all members to contribute for our Newsletter. On behalf of the Executive Board, I would like to express my heartfelt appreciation to all members for their continuous support. MALAYSIA ASQua s Successful Nomination and Election to the ISQua Board for term 2017-2019 ASQua has been successfully re-elected as an Institutional Member position on the ISQua Board for the October 2017 September 2019 term of office. There were more than four organisations nominated to stand in the institutional membership category and there were high turnout of voters this year with 47% of Individual Members and 51% of Institutional Members voting. The election results was announced officially at the Annual General Meeting (AGM) at QE II, London, on Monday 2nd October 2017. ASQua took up office immediately as ISQua Board Member following the close of the AGM. ISQua current Board Members are: 1) Ms. Wendy Nicklin (President) 2) Professor Jeffrey Braithwaite (President Elect) 3) Professor Cliff Hughes (Immediate Past President) 4) Professor Shin Ushiro (Board Member) ASQua Institutional 5) Professor Yuichi Imanaka (Board Member) ASQua Institutional 6) Mr. Peter Carter (Board Member) 7) Dr Bhupendra Kumar Rana (Board Member) ASQua Individual 8) Dr Stephen Clerk (Board Member) 9) Mr Bruno Lucet (Board Member) 10) Ms Jacqui Stewart (Board Member) 11) Ms Leslei Thompson (Board Member) BMJ INTERNATIONAL FORUM FOR QUALITY AND SAFETY IN HEALTH CARE (IFQSH) The BMJ-IHI IFQSH forum was successfully held from 24 th to 26 th August 2017 at Kuala Lumpur Convention Centre with strategic partners, MSQH and the Academy of Medicine Malaysia. The forum was declared opened by His Royal Highness Sultan Nazrin Muizzuddin Shah Ibni Almarhum Sultan Azlan Muhibbuddin Shah. HRH Sultan Nazrin also presented the opening speech to an audience of more than 1040 delegates registered for the forum where 414 were from Malaysia. It was noted that 70% of the 414 Malaysian delegates were from Ministry of Health Malaysia, KPJ and Pantai. There were more than 300 posters accepted for presentation and display of which 87 or nearly 30%, were from Malaysia.

INTERNATIONAL SOCIETY FOR QUALITY IN HEALTH CARE (ISQUA) ISQUA 34 TH INTERNATIONAL CONFERENCE 2 ND TO 04 TH OCT 2017 MSQH Committee members and staffs registered for the ISQua's 34th International conference from 02 nd to 04 th October 2017 at QE II Centre, London, United Kingdom. This year s theme was Learning at the System Level to Improve Healthcare Quality and Safety. A total number of 1529 delegates from 80 countries registered for the conference and 20 for the exhibition booths. There were 18 delegates registered from Malaysia. The conference was declared open by Professor Cliff Hughes, ISQua President and Professor Jennifer Dixon, CEO of The Health Foundation. The conference was awarded the Patients-Included status which reflected the conference s focus on incorporating the experience of patients whilst ensuring that they are not excluded or exploited. MSQH participated in an exhibition promoting ISQua KL 2018 with support from MyCEB and Tourism Malaysia. MSQH was privileged to have provided a 10-minutes Welcome to Kuala Lumpur 2018 address on 04 th October 2017 by MSQH President, Y.Bhg Tan Sri Datin Paduka Siti Sa diah Sheikh Bakir. The welcoming address was supported by KPJ Healthcare Berhad s rendition of Malaysia Truly Asia opening song by Mr. Mohd Taufik Ismail and Madam Mah Lai Heng. A total number of 1702 posters were submitted where only 704 were accepted (e-posters 76) from 73 countries.

ASQua BOARD MEETING AND AGM ON 24 TH AUGUST 2017 ASQua organised its 2 nd Executive Board (Post Registration) and Annual General Meeting at Kuala Lumpur Convention Centre, on 24 th August 2017. Eight of thirteen executive board member countries from Japan, Taiwan, Malaysia, India, Japan, Thailand, Indonesia and Korea, attended the meetings. Professor Yuichi Imanaka, President ASQua chaired the meetings. Dr Kadar, CEO MSQH attended the meetings representing MSQH. The members RESOLVED the following matters at both meetings: Approval of previous minutes of Board Meeting held on 18 th October 2016; Adoption of Board Report for period ending 31 st July 2017; Membership status report and approval of new members; Appointment of Chairman of Standing Committees; Treasurer s report for 31 st December 2016 and 31 st July 2017 (budgeted and actual); Open ASQua s current account at CIMB Bank Berhad Malaysia; Support of ASQua s nomination in the ISQua s election 2017-2019; Proposal of ASQua values; Support for ISQua KL 2018 participation from ASQua members for delegates registration and paper submission.

JAPAN COUNCIL FOR QUALITY HEALTH CARE The Japan Council for Quality Health Care (JQ) was established in 1995 as an independent, non-profit organization which aims to improve health care and welfare in the nation. MISSION We perform activities from a neutral, scientific standpoint, holding ethics and autonomy in high regard, to improve quality and safety in health care, and to ensure reliable health care system, thus contributing to the improvement of the nation's health and welfare. VALUE - To build a relationship of mutual trust and cooperation with patients, families, healthcare providers, and all other related parties. - To deal with all matters in an unbiased, fair manner. - To ensure transparency and accountability to society. - To bring together our collective strength from a scientific and professional perspective, to support the improvement of quality and safety in health care. - To continually strive for higher standards. WHAT WE DO We have the following functions which lead to the improvement of health care quality and safety. (1) Hospital Accreditation Our third-party evaluation (accreditation) of hospitals for health care quality and safety supports them to identify their key challenges and strengthen their system for quality assurance and continuous improvement. We also help hospital staff learn more about management and improvement of health care quality and safety. New hospital accreditation standards 3rdG(3rd generation):ver.2.0 is being launched in April 2018. https://www.jq-hyouka.jcqhc.or.jp/ (2) Patient Safety Promotion Our accredited hospitals voluntarily join our network Patient Safety Promotion. We exchange opinions and challenges faced by healthcare professionals, having meetings and training sessions, discuss and investigate these matters, and feedback to health care institutions nationwide.

(3) The Japan Obstetrics Compensation System for Cerebral Palsy Through the insurance mechanism, we financially assist families caring for children who suffered from cerebral palsy at childbirth. We also analyze the root causes of cerebral palsy for recurrence prevention. Thus medical disputes can be avoided or swiftly settled, and can contribute to the improvement of obstetric care as a whole. http://www.sanka-hp.jcqhc.or.jp/documents/english/index.html (4) EBM and Guidelines (MINDS) We support the developers of evidence-based clinical practice guidelines (CPGs), assess the quality of CPGs, and diffuse them to the real practice fields also via the Internet. By collecting and analyzing information from various medical institutions nationwide, we release our findings via our website and other media to prevent accidents and improve patient safety throughout the nation and the world. http://www.med-safe.jp/contents/english/index.html (6) National Database of Near-miss Events in Pharmacies We take the initiatives to reduce medical adverse events through this national database of near-miss events in pharmacies, as the above medical adverse events database. http://www.yakkyoku-hiyari.jcqhc.or.jp/contents/english/index.html (7) International Affairs We positively and open-mindedly cooperate with international experts and organizations, contribute to the state of the art, and make efforts to improve healthcare quality and safety globally and locally. We have contributed and will contribute to the improvement of quality and safety in health care through various functions which benefit patients, health care professionals, organizations and societies. We will continue to take on new challenges for quality and safety in health care. THE JOINT COMMISSION OF TAIWAN (JCT) The Joint Commission of Taiwan (JCT) has its 7 th Board, with new appointment of Dr. Chii-Jeng Lin as the President, and Dr. Pa-Chun Wang as CEO in 2017. The Board has shown its diversity by consists of representatives from Ministry of Health and Welfare, Taiwan Hospital Association, Taiwan Nongovernmental Hospitals & Clinics Association, Taiwan Medical Association, healthcare professionals associations, and patient representatives, which has brought a wide range of experiences and expertise to our organization. JCT continues to encourage Taiwan delegates to participate ISQua s International Conferences. In 2017 London conference, 230 Taiwan delegates with more than 325 abstracts were presented from a total of 1,600 attendees. JCT presented 1 e-poster presentation, 24 poster presentations and hosted the session The Improvement and Development of Mental Health in Taiwan, with the goal of sharing JCT s experience in healthcare accreditation and quality improvement. After the conference, Dr. Peter Lachman, CEO of ISQua, and Cliff Hughes, Immediate Past President of ISQua, visited Taiwan in November. They both felt amazed at the achievement of healthcare quality in Taiwan, and look forward to having strategic collaboration with JCT in the future. Furthermore, the 41 st World Hospital Congress of the International Hospital Federation (IHF) was hosted by Taiwan Hospital Association in Taiwan in November, 2017. JCT was honoured to be part of this grand conference as a co-organizer. There were over 800 healthcare professionals coming from all over the world. JCT also communicated the ideas which capture the current thinking and challenges in the areas of quality improvement and patient safety by holding a session. In recent years, JCT was dedicated to promote patient-centered care, population care, and smart healthcare. Through the supporting from the government and hospitals, many Patient Decision Aid (PDA) were developed for Shared Decision Making (SDM). In addition, JCT continues to provide the accreditation and certification services in various fields. The Disease-Specific Care (DSC) development was one of the major progresses by expanding the scope of certification to chronic population including heart failure, stroke, diabetes, kidney disease, and chronic obstructive pulmonary disease.

JCT is deeply saddened to announce that Dr. Bor-Shen Hsieh, former JCT President who passed away peacefully on February 5, 2018. Dr. Hsieh has initiated and accomplished major polices during his term as JCT President from 2002 to 2010, such as post-graduated year training, caring clinical and healthcare providers, and community healthcare group programs, all were milestones for JCT. Moreover, with Dr. Hsieh s fully support, JCT achieved its first ISQua s IAP award in 2006, afterwards, JCT received ISQua s IAP awards for External Evaluation Organizations, Healthcare Standards, and Surveyor Training Programs.

KARS, INDONESIA KARS ACTIVITY IN 2017-2018 National Standard of Hospital Accreditation (SNARS) 1 st edition is ready and launched on August 8, 2017 by the Minister of Health of the Republic of Indonesia at the opening Annual Scientific Meeting and National Accreditation (PITSELNAS III). SNARS 1 st edition which is a hospital accreditation standard compiled by KARS by consistently referring to IHSG 4 & 5 edition and reference in making Accreditation Standard set by ISQua. There is a new chapter included in SNARS 1 ST edition, Chapter National Program which is a substitute for the MDGs chapter on Accreditation Version 2012. This chapter aims to support the success of Ministry of Health programs that reduce maternal and infant mortality and improve maternal and infant health, HIV / AIDS, decreased morbidity rate of Tuberculosis, control of antimicrobial retention and geriatric services. After the socialization of SNARS 1 st edition, it is also necessary to compile the SNARS 1 st edition Instrument Book, which is a detailed description of SNARS 1 st edition. This instrument is expected to be of great benefit to the Investigator in providing an assessment of accreditation (scoring) at the hospital, which is used as a guide in conducting regulatory review and review of evidence documents on the application of accreditation standards during hospital surveys. This instrument is also very useful for hospitals in drafting regulations required by accreditation standards, providing training to staff who handle accreditation as well as reference in conducting preparatory and standard implementation activities. With this Instrument book, it is hoped that there will be no difference in perception between KARS surveyors and hospitals with one standard or assessment element. CONGRESS OF KARS The KARS Congress was held on February 6, 2018 in Jakarta for the election of KARS members 2018-2023 and elected: Executive Chairman: Dr. Sutoto, M.Kes Executive Secretary: Dr. Djoti Atmodjo, Sp.A, MARS Executive Treasurer: Dr. Masaah Amatyah S, M.Kes USE OF WEBINAR METHOD FOR SURVEY WORKSHOP Due to the considerable number of KARS surveyors (about 700 people) spread across Indonesia from Aceh to Maluku, it is necessary to set up a Webinar method to carry out long-distance workshops for surveyors who are unlikely to come to Jakarta. Since June 2017 has started the use of Webinar for workshop for surveyors. This activity is directly managed by KARS IT staff with camera and broadcasting equipment owned by KARS. The software application, originally only capacity for 100 participants of the Webinar, has now been upgraded to a capacity of 500 Webinar participants.

With this Webinar method the training and improvement of the competence of surveyors can be done efficiently, and without the surveyor leave the place of duty. For the future of the Webinar facility will also be extended to accreditation guidance for hospitals located in remote / remote locations. Quality and Accreditation Institute (QAI) Quality and Accreditation Institute (QAI) was set up to create an ecosystem of education, training, quality improvement and accreditation. It is believed that this organisation would provide a platform to stakeholders including professionals and organisations, associated with quality in any way, to share their wisdom and knowledge in order to make its Vision realised. This will further provide tremendous opportunities to all concerned to learn and contribute in improving organisations engaged with QAI. Different activities would be initiated under different verticals in a manner that they remain independent of each other. QAI aims to operate globally. VISION: Nurturing the largest global pool of organisations and people through quality and accreditation framework. MISSION: To conceive and deliver education, training, accreditation and related programs in partnership with stakeholders using an approach of co-design and co-creation.
