NAT MOORE FOUNDATION FAQS Who is Nat Moore? Nathaniel "Nat" Moore (born September 19, 1951) is a former American college football player at the University of Florida and a former professional football player for the Miami Dolphins. He played the position of a wide receiver during his thirteen-season tenure (1974-1986) in the National Football League (NFL). Where does funding for the Nat Moore Foundation come from? The Nat Moore Foundation is primarily funded through the proceeds of its annual Florida Golf Classic gala and golf event. Corporations and community leaders from Florida and across the country are sponsors of the event. The Foundation also receives grants from corporate funders, such as Wells Fargo and through scholarship partners, such as ClearChannel Outdoor, who provided funding for scholarship at Miami Northwestern, Edison, and Jackson Senior High Schools in 2008 and in 2011. What is the Nat Moore Foundation s Florida Golf Classic? The Nat Moore Foundation s Florida Golf Classic is a celebrity-filled, fund raising gala and golf tournament. The proceeds from the Florida Golf Classic support the Nat Moore Foundation s scholarships, grants and programs. What is the Nat Moore Foundation s mission? The Nat Moore Foundation s mission is Helping Kids Help Themselves. The Foundation is dedicated to providing educational, cultural and recreational activities to South Florida s disadvantaged youth, ages 5 to 18. Is the Nat Moore Foundation a nonprofit organization? Yes. The Nat Moore Foundation is a 501 (c)(3) public foundation. What does the Nat Moore Foundation do? The Nat Moore Foundation provides: Academic scholarships to deserving high school seniors for their higher education, Annual grants to community-based organizations, Matching grants, up to $5,000, to assist with transportation costs for youth sports team competing in regional or national championship games, and Page 1

Direct service programs, in collaboration with community partners, supporting the physical and academic success of children and youth. What are the Nat Moore Foundation s accomplishments? The Nat Moore Foundation has dispersed nearly $2 million to various youth and social service organizations, Helping 60 students attain their higher education through scholarships, and Providing support services to hundreds of children and youth through academic, fitness and life-skills programs. URBAN SCHOLARSHIP PROGRAM FAQS When will the application for an award from the Nat Moore Foundation Urban Scholarship Program become available? The Nat Moore Foundation Urban Scholarship Program applications are available August 1, 2012 through January 31, 2013. What is the deadline for submitting applications for an award from the Nat Moore Foundation Urban Scholarship Program? The Nat Moore Foundation Urban Scholarship Program application and all supplemental materials are due to the Foundation no later than JANUARY 31, 2013. Can I submit my application by fax? Can I still apply? Due to the large number of applications submitted each year, no application will be accepted after the JANUARY 31, 2013 deadline. Is there a fee to apply? No. The Nat Moore Foundation has no scholarship application fees. What are the Nat Moore Foundation Urban Scholarship Program eligibility requirements? To apply for an Urban Scholarship Program award, you must: Be a graduating high school senior from Miami-Dade or Broward Counties High Schools, Demonstrate financial need, Page 1

Plan to attend a 2-year or 4-year college, or technical school, Achieve a cumulative, non-weighted GPA of 2.0 or above on a 4.0 scale, and Demonstrate commitment to community service. I am a home-schooled student. Can I apply? Graduating, high school seniors from Miami-Dade and Broward Counties schools, who can demonstrate financial need, may apply for the Nat Moore Foundation Urban Scholarship Program awards. I attend a private high school. Can I apply? Yes. Graduating, high school seniors from Miami-Dade and Broward Counties schools, who can demonstrate financial need, may apply for the Nat Moore Foundation Urban Scholarship Program awards. Is there an age limit? Applicants to the Nat Moore Foundation Urban Scholarship Program must be a graduating, high school senior. I received a General Education Diploma (GED). Can I apply? Applicants to the Nat Moore Foundation Urban Scholarship Program must be a graduating, high school senior. I am currently in college. Can I apply? Applicants to the Nat Moore Foundation Urban Scholarship Program must be a graduating, high school senior. Is there a minimum SAT or ACT score requirement? No. What if my SAT or ACT score changes after I submit my application? Should I update you on my scores? No. Only scores submitted with completed applications will be considered. Page 2

Is there a minimum grade point average (GPA) requirement? Yes. All graduating high school seniors must hold and maintain at least a cumulative, non-weighted 2.0 GPA or above on a 4.0 scale. Awarded students must maintain a 2.8 GPA while pursuing their higher education to ensure continued scholarship support. Do I have to be a United States citizen to apply for a scholarship? Applicants are required to be a U.S. citizen or a permanent resident. Is there an income limit? The Nat Moore Foundation considers financial need as an important factor when selecting recipients of the Urban Scholarship Program awards. Applicants must demonstrate financial need. Can I use the Nat Moore Foundation Urban Scholarship Program award at any college and for any major? All scholarship awards may be used at a higher educational institution that: Maintains a regular faculty and curriculum, Has a regularly organized body of students in attendance, and Is listed by the U.S. Department of Education as being accredited by recognized accrediting agencies. All scholarship recipients may pursue any major of their choice leading to a bachelor s degree or the equivalent certification from a technical educational institution. Can I use the Nat Moore Foundation Urban Scholarship Program award for on-line or nontraditional higher educational institutions? All scholarship awards may be used at a higher educational institution that: Maintains a regular faculty and curriculum, Has a regularly organized body of students in attendance, and Is listed by the U.S. Department of Education as being accredited by recognized accrediting agencies. Is the Nat Moore Foundation Urban Scholarships renewable awards and what is the amount of the award? The Nat Moore Foundation Urban Scholarship Program usually provides $10,000 scholarship awards, payable at $2,500 each year over four years. Each year, the award recipient must show their good standing by providing proof of continued enrollment in their school of choice and a cumulative 2.8 grade point average (GPA). However, the scholarship amount may vary depending on the need of the student and special scholarship programs initiated by sponsor partners. Unused funds from scholarship awards are re-apportioned. If I am awarded a Nat Moore Foundation Urban Scholarship, how do I get my money? The Nat Moore Foundation contacts each award recipient and provides them with an Urban Scholarship Program Award Packet. Once the award recipients return signed acceptance forms and enrollment confirmations, the Foundation contacts each award recipient s chosen higher educational institution to arrange payments. All payments are made directly to the schools. Do I have restrictions on how to spend my scholarship money? Yes. The Nat Moore Foundation Urban Scholarship Program funds may be used for any purpose that supports the recipients higher education, including but not limited to: Tuition & fees Books, supplies and equipment Room & board Travel or research that are incidental to attaining a degree (requires approval by the Nat Moore Foundation) Internships (requires approval by the Nat Moore Foundation) Study abroad programs (requires approval by the Nat Moore Foundation) If I am awarded other scholarships or financial aid, does it affect my eligibility for the Nat Moore Foundation Urban Scholarships? Recipients of other scholarships and financial aid are still eligible to receive a Nat Moore Foundation Urban Scholarship Program award. However, the Foundation asks that award recipients who receive full academic scholarships (other scholarships or financial aid that covers all their academic expenses) from another source immediately notify the Foundation and withdraw their application so that the opportunity for the award can be extended to fellow graduating students in need. How are Nat Moore Foundation Urban Scholarship Program recipients chosen? A Scholarship Committee, consisting of Nat Moore Foundation Board Members, meets each year following the scholarship deadline to identify candidates who will then be submitted to the Foundation s Board of Directors for their final approval. When are the winners notified? Are all applicants notified? The award recipients are notified personally by June 30th of each year. All other applicants are notified by e-mail or mail. Page 5
