Ministry of Advanced Education and Skills Development Student Financial Assistance Branch 2018-19 Application for Ontario Out-of-Country Bursary for Deaf Students Purpose The Ontario Out-of-Country Bursary for Deaf Students provides non-repayable assistance to deaf postsecondary students studying outside of Canada. This bursary assists students with eligible education costs that are in excess of the funding received through OSAP for Full-Time Students. For the purposes of this bursary, deaf students refers to postsecondary students who are deaf, deafened, or hard of hearing. Eligibility requirements To be eligible for this bursary, you must meet all of the following requirements: You have a permanent hearing disability; You are enrolled in a postsecondary school outside of Canada that is approved for OSAP purposes; You are enrolled in an eligible program of study where the only or primary language of instruction is American Sign Language (ASL) or Quebec Sign Language (see the Eligible programs of study section, below); You are taking 40% or more of a full course load; Your eligible education costs exceed the Canada-Ontario Integrated Student Loan maximums for your study period; You have submitted a 2018-19 OSAP Application for Full-Time Students, including all required documents and have met all eligibility requirements for that program. Eligible programs of study Programs provided in ASL at the following schools outside Canada are considered eligible programs for the purposes of this bursary: Gallaudet University in Washington, D.C. National Technical Institute for the Deaf in Rochester, New York Rochester Institute of Technology in Rochester, New York Other schools outside Canada where students are enrolled in a program where the only or primary language of instruction is ASL or Quebec Sign Language may also be eligible. If you are not sure if you or your program of study are eligible for the purposes of this bursary, contact the Student Financial Assistance Branch. Required documents You must provide the following documentation along with this bursary application: Proof of your permanent hearing disability (a medical certificate or an audiological report); Proof of your room and board costs during your study period from your school if you are living on campus; Proof of your out-of-country health insurance costs during your study period (e.g. a written estimate from your insurance company or your school s health plan). 1

Note regarding currency conversion The ministry requires that all documented costs be provided in U.S. dollars. The conversion rate used by the ministry is based on a two-week average. Fall disbursements will use an August two-week average, winter disbursements will use a December two-week average and spring disbursements will use an April two-week average. Any differences between the exchange rate used to calculate your bursary funding and the exchange rate at the time you receive your bursary funding will not be adjusted. Note: You may be charged a fee by your school for converting your bursary cheque into U.S. funds. You will not be reimbursed by the ministry for this fee. Reminder To be considered for this bursary, you must also submit a 2018-19 OSAP Application for Full-Time Students including all required supporting documents. Your application and required documents must also be received by the corresponding deadlines. Application and required document deadlines To be considered for this bursary, your completed bursary application must be received by the ministry at the address listed on this page no later than 60 days before the end of your study period. All required supporting documentation must be received by the ministry no later than 40 days before the end of your study period. Where to send your application and required documentation Your completed 2018-19 Application for Ontario Out-of-Country Bursary for Deaf Students and all required documentation must be sent to the ministry at the following address: Student Financial Assistance Branch Attention: Administrator, Ontario Out-of-Country Bursary for Deaf Students Ministry of Advanced Education and Skills Development PO Box 4500, 189 Red River Road, 4th Floor Thunder Bay, Ontario P7B 6G9 Getting your funding Your total bursary entitlement will be split into two or three equal payments that will be available at your school at the beginning of each term/semester. Each instalment will be in the form of a cheque issued in Canadian dollars. Your school must confirm your enrolment before they can release any bursary cheque to you. You are responsible for contacting your school s financial services office or international students office to pick up your bursary cheque. Bursary funds are to be used to pay the fees owing to your institution first. 2

Contact the school you are planning to attend if you have any questions regarding the process to obtain a study visa. 3

Section E: Consents, instruction, declarations and signature of applicant Notice of collection and use of personal information Your personal information, including your Social Insurance Number (SIN), provided on this application form and in all other communications related to this application and any award of an Ontario Out-of- Country Bursary for Deaf Students (Bursary), including previous applications and awards of OSAP assistance will be used by the Ministry of Advanced Education and Skills Development (the ministry) to administer and finance the Bursary. In addition, your SIN will be used as a general identifier in administering the Bursary. The ministry may use contractors, auditors or other authorized third party administrators for any of these activities. 5

2018-19 Application for Ontario Out-of-Country Bursary for Deaf Students Social Insurance Number: Consents, instruction, declarations and signatures of applicant - continued Administration includes: determining your eligibility for a Bursary; verifying your application; verifying any award of a Bursary; maintaining and auditing your file; assessing and collecting overawards; enforcing the legislation set out below and your agreements with the ministry, and monitoring and auditing your postsecondary school to ensure that they are administering the Bursary appropriately. In addition, administration by the ministry includes: public reporting on the administration and financing of student assistance programs; planning, delivering, evaluating and monitoring the Bursary for quality and improvements in both content and delivery; conducting risk management, error management, audit and quality assessment activities; conducting inspections or investigations; and conducting policy analysis, evaluation, and research related to all aspects of OSAP including contacting you to participate in surveys. Financing includes: planning, arranging or providing funding of the Bursary. The ministry administers the Bursary under the authority of s. 5 of the Ministry of Training, Colleges and Universities Act, R.S.O. 1990, c. M.19, as amended and s. 10.1 of the Financial Administration Act, R.S.O. 1990, c. F. 12, as amended. 2018-19 Application for Ontario Out-of-Country Bursary for Deaf Students Social Insurance Number: Applicant s consent to the indirect collection, use and disclosure of personal information (REQUIRED) I agree that until any Bursary overpayments are assessed and repaid, the ministry can, without limitation, collect and exchange personal information about me that is relevant to the administration and financing of the Bursary with any person or organization referred to on this application who may have information relevant to my continuing eligibility for the Bursary, including my postsecondary school; the ministry s contractors, auditors or other authorized third party administrators; collection agencies operated or retained by the federal or provincial government, and consumer reporting agencies. I understand that if I withdraw my consent it will affect my eligibility for and the amount of Bursary assistance. Applicant s declaration I have given complete and true information on and in connection with this application. I understand that information I provide in connection with this application will be verified and audited and the ministry may also conduct inspections and investigations. I understand that the funding I receive under the Bursary must be used to pay my academic fees first. I understand that I may be required to repay all or part of the bursary award if the information I provide in connection with this application is found to be inaccurate for any reason or if any information I provide changes, including my study period and/or my course load. I understand that any change to the information I provide and any change resulting from verification and audit may affect my eligibility for and the amount of my Bursary and that I may be required to repay all or a part of the Bursary award. I understand that if I fail to provide complete and true information; fail to promptly notify my school s financial aid office or the ministry in writing of changes to any information I have provided, including my disability, my address and/or financial, academic, family, and/or study period status; or fail to fulfil any obligations respecting the repayment of any overawards the ministry may restrict me from receiving this Bursary or other OSAP assistance in the future, and may take legal action and may require me to repay any assistance that I received.
