Meet Your Mojave Microgrants Application for Field Trip Transportation Funding Thank you for your interest in partnering with Get Outdoors Nevada (GON). GON is committed to the sustainability and vitality of our community through the promotion and protection of Nevada s great outdoors. Please complete the following application to be considered for funding. Microgrants are available to schools that demonstrate a need for assistance and complete the following application. Transportation funding is limited to $250 per bus per request. This is a reimbursable grant. Your school must order the bus and funding will be reimbursed upon submission of a final report summarizing your experience, how it benefits your community and enhances appreciation and understanding of Nevada s special outdoor places. Instructions The Application and Selection Process Applications are accepted on a rolling basis. Funding is limited to $250 per bus per trip. There are no limits on the number or frequency of submissions from a single applicant. Applications must be submitted no less than two weeks prior to the requested field trip date. Funding may not be applied to past field trips. Funding is not available for other field trip expenses such as entrance fees. Successful requests will demonstrate a strong connection between classroom curriculum and field trip context. Successful requests will demonstrate volunteer components. This can include anything from a clean-up, educational art projects, or other service minded projects. GON can provide guides, lesson plans, or experiential activities for your trip. 3. What are your desired educational goals? How will you prepare the students for this trip, or follow up on these outcomes after the field trip? 4. How will your trip/classroom include a volunteering or servicelearning component? This could include a physical volunteer project, but could also include a follow up art project, creating an educational bulletin board, etc. These projects should have a purpose and presentation site (school hallway, community center, etc.) 5
5. Provide any other relevant details. Consider possible time constraints, and past experiences on field trips. Please inform us whether you will require special accommodations such as wheelchair seating on a bus. List other sources of funding for your trip and any restrictions or provisions on those funds. 6
Part III Agreement Approval of this application by the Get Outdoors Nevada creates specific obligations on the part of the recipient, including but not limited to an understanding that: the awarding and amount of any funds shall be subject to the sole discretion of GON; that recipients must submit a final evaluation report with all requested documentation, receipts, and an invoice form before receiving funding from GON; that all applications and evaluation reports become the property of GON, which retains the right to supply others with a description and materials related to your trip. GON, its employees, and affiliates shall assume no responsibility or liability for claims of loss or damage of any kind of property, or for claims of injury to any person in connection with this funding. Recipients agree to comply with all requirements of this application as described by the instructions included in this document. As the approval process for field trips scheduled through the Clark County School District CCSD can take a minimum of 4 weeks to complete, the Get Outdoors Nevada GON suggests that teachers submit the required forms to their administrator as early as possible. All documents listed below must travel on the field trip in the possession of the lead teacher. Do not forward copies of the following documents to Get Outdoors Nevada. Required Forms for Teachers: In addition to completing the following forms, teachers must prepare an itinerary and a list of student names and numbers for the trip. CCF-798 Field Trip Plans and Request for Approval of Principal CCF-145 Out-of-District Travel Request CCF-101 Employee Absence Request/Authorization Chaperone Release Form Other Documents May Be Required. Teachers should include the following documents in a packet to be sent home with participating students, along with a letter to parents. CCF-455 Student Medical Permission Form CCF-796 Field Trip Permit Scheduling CCSD Transportation CCSD transportation may be scheduled through interact - your office manager can assist with your request. Field trips using CCSD transportation may depart no earlier than 9:00 AM, and must return no later than 1:00 PM. If your field trip cannot be accommodated in this time frame, GON recommends contacting a private bussing organization. GON works from a list of agencies with insurance approved by CCSD. Charter Bus Services Attached is the current list of approved CCSD travel charter buses. Get a quote from 2 before selecting one service. 9
Part VI Submitting a Final Report for Reimbursement Reimbursement requires the completion of the attached final report. See link: https://goo.gl/forms/qtqq1ufray1qgbou2 Complete and answer all questions to the best of your ability. This final report is an essential step in finalizing your agreement with Get Outdoors Nevada. Final Report must be received by GON no later than two weeks following the date of the trip. Incomplete or late final reports may result in loss of funding. Submit a minimum of five age-appropriate student experiences in writing (guiding questions are provided if needed). Pictures, drawings, paintings, or other creative activities can accompany these writings from attendees. Recipients must submit an invoice (a template is attached) to GON for reimbursement. If an invoice is not received by GON within one month of the date of the trip, GON may, at its discretion, reallocate grant funds to other applicants. Reimbursement will be mailed within 10 business days after receiving final report and materials per application instructions.
