Current Position Role initially 2 days per week Increased to 4 days per week Clinical Weekly clinics at community location and GP practices Writing guidelines and getting drugs to formulary Educational Medicines management leadership for community neurology service and GPs Education sessions for GPs and other healthcare professionals, patients and carers Commissioner Voting member of MTRAC & AMMC
The Multidisciplinary Neurology Team Other support services: Dudley Rehabilitation Specialist Pharmacist Virtual ward Community Rapid Response Community Nurses Social workers Expert patient panel Wheelchair Service Speech and Language Therapists Public Health
Community Neurology Service Patients 18 years and above Have a Dudley GP Have a diagnosis of a long term Neurological Condition except for conditions seen by other teams e.g stroke / ABI
How the community Neurology Service operates Weekly MDT meetings for complex reviews and patient allocation Patients can self-refer or be referred by a Consultant, GP or other healthcare professional
Successes to date Quantified: 34,810 saved over last 12 months by improving formulary adherence and generic prescribing Excellent patient feedback questionnaires Close to complying with NICE guidance on 6 monthly specialist review for patients Still to be costed: Saving of consultant time as longer time between consultant reviews Reducing GP contacts Reduction in hospital admissions by improved prescribing of complex medication

These clinics are like a breath of fresh air and I have learnt more about my condition today than during the last ten years since diagnosis..
