Bariatric Care Solutions

The Obesity Challenge Obesity is now recognised as a global epidemic. Obesity rates in the UK and Eire are among the highest in Europe and have increased dramatically over the last few years to such an extent that around 25% of adults are now classed as obese. Key Fact In the UK, 3.6% of women and 1.8% of men are classed as severely obese 1. Obesity increases the risk of a number of health issues including heart disease, diabetes, musculoskeletal disorders, cancers, depression and anxiety. Severely obese individuals are three times more likely to require social care than those with normal weight, resulting in an increased risk of hospitalisation and associated health and social care costs and nursing risks. Bariatric patients have a number of unique challenges including: Difficulty mobilising Increased body weight and body mass inhibits the ability to move and flex. May drag their heels and sacrum when getting in and out of bed increasing the risk of shear and friction. Entering or exiting the bed safely may be very challenging and increases the risk of falls. Impaired respiration Respiration may be compromised due to impaired diaphragmatic movement and subsequent impaired tissue perfusion. Positioning at an elevated head of bed angle is vital due to respiratory challenges. Increased risk of pressure damage A lack of mobility, difficulty in repositioning, increased skin to skin contact and increased sweating all increase the risk of pressure damage. Increased body weight may make it more difficult for the caregiver to view bony prominences. This may allow pressure ulcers to develop in areas that are not considered high risk such as hips, trunk, torso, occiput. Areas under skin flaps may be missed when washing. Individuals may be at risk of developing skin damage from equipment e.g. from bed rails if they are placed on a bed that is too narrow. Compromised healing Wound healing is slow due to a decrease in wound collagen deposition. Blood supply to fatty tissues may not be adequate to provide appropriate oxygen and nutrition. Bariatric patients generally have poor diets with fatty foods which exclude essential vitamins and nutrition to aid wound healing. Sweating/moisture build-up People who carry more weight are prone to profuse sweating, often suffering skin breakdown, rashes and intertrigo (inflammation that occurs on skin folds).
