The Josie King Foundation s mission is to prevent others from dying or being harmed by medical errors. Goals for the Care Journal Help patients and their families organize the detailed information that they receive when in the hospital. Bridge the communication gap that may sometimes exist between caregivers and the patient. Care Journal Partner Program We wish to encourage medical organizations to partner with us to distribute the Care Journal to their patients. Hospitals typically identify units where their patients could benefit most from the Care Journal. Commonly selected units include: Oncology Pre- and post-transplant Intensive care While distribution techniques vary, most often a nurse or patient coordinator gives the Care Journal as a gift to the patient and family after admission. 3
Optional Bookmark Some hospitals choose to customize a bookmark that doubles as a welcome message to their patients. The hospital simply uses this template to develop their personalized welcome message, which is placed into the front pocket of each Care Journal. Recommended Donation To cover the cost of development, design, printing and shipping, we recommend the following donations when requesting a Care Journal: $15 per Care Journal for individuals $7.50 per Care Journal for groups requesting a bulk order (Plus $1 if requesting optional bookmark) Our primary goal is to get the Care Journal into the hands of patients.
