2017 2018 School Wellness Team Mini Grant Funding Application Student Wellness Vision: A district that fosters a culture of healthy, active learners. Student Wellness Mission: To improve the health of students and families. This mission will be accomplished through education, environmental changes, and opportunities to establish life long healthy nutrition and physical activity habits. To support wellness teams throughout the district, the D6 Wellness program will provide a limited amount of grant funding provided by The Colorado Health Foundation to support School Wellness Teams across WCSD6. These funds are to be used to support school wellness related activities and materials related directly to the increase in physical activity, nutrition, or mental health for students. Interested applicants must complete the attached application form explaining the request and outlining how the funds will be used to increase or improve physical activity, nutrition, or mental health. Each school wellness team may apply for up to $4000.00 for one year of funding. Schools Wellness Mini Grant Information All School Wellness teams are eligible to apply. Funds will be allocated based on merit of applications (see attached rubric for details). Funding awards are one time awards and must be spent by March 30 th, 2018 (exceptions may be made on a case by case basis) Funds received are up to $4,000 All expenses paid for from the grant must be approved by the Wellness Department and Grants Accountant, and must be detailed in the School Health Improvement Plan. Funds may not be used for stipends. Requirements for funding Must be used for increasing physical activity, nutrition, or mental health for students. Must be reflected in detail in the 2017 2018 School Health Improvement Plan. Must participate in monitoring and evaluation as requested by The Colorado Health Foundation and the WCSD6 Wellness Department 1
Application Process Applications will be accepted until Monday, September 18 th at 11:59 pm (NO LATE APPLICATIONS WILL BE ACCEPTED). This is a COMPETITIVE GRANT. All applications will be evaluated and rated by WCSD6 Wellness staff. Timeline Applications available Tuesday, August 15 th, 2017 Applications due Monday, September 18 th, 2017 at 11:59 pm Awards announced end of September, 2017 Funding will need to be spent by March 30 th, 2018 (exceptions may be made on case by case basis) Allocation of funds Successful applicants will receive a mini grant account designated to their own school. Funds will be managed by student wellness team leader(s) and office manager. PLEASE NOTE: This is not a reimbursement grant. Please do not make any purchases prior to receiving grant approval. Application is not a guarantee of funding. Evaluation Each recipient of this grant will be asked to complete a final report and/or submit data before the end of the 2017 2018 school year. This evaluation may include a summary of details of grant activities and level of success achieved. Information on the number of participants, outcomes, and overall evaluation may be requested. Completion of this evaluation is required. 2
What We Fund For the 2017 2018 school year, the Wellness Department will provide grants to District 6 schools, staff, and teachers to enhance school wellness culture and climate in three categories. Category 1: Nutrition and Health We will consider grant applications that aim to create cultures and climates in schools that are supportive of health education and promotion, nutrition education, and improved cafeteria environment. Some examples include: Purchase cooking materials to support healthy cooking in the classroom (similar to Chef in the Classroom) Materials to support a wellness fair (must collaborate with community partners) Painting a mural in the cafeteria to support a healthy and happy eating environment Purchase materials for creation of a new garden, gardening maintenance, and/or garden education. Working with administration to schedule recess before lunch for all grade levels. Funding can help with transition, such as purchasing coat racks for students to use when they come off the playground, purchasing hand sanitation areas for students (ex: hand sanitizer on wall) Reduce food waste by purchasing share table and/or composting bins Category 2: Mental Health We will consider grant applications that aim to improve mental health for students within the school, such as training for staff, materials for mindfulness, and programming in schools. Some examples include: Training all staff in A Mindful Life Support for training all staff in Youth Mental Health First Aid (These trainings are often free. Support might include: books, mental health materials to promote program, etc.) Purchasing materials to support mindfulness based activities with students in the classroom Category 3: Physical Activity We will consider grant applications that aim to increase physical activity in the school day and before or after school programming. Some examples include: Purchasing fitsticks to provide to all staff Purchasing materials for physical activity kits that teachers can check out Scanners to help support running club Materials to host a fun run, such as obstacles, prizes for participation, etc. What We Will NOT Fund In order to promote sustainability of programming after the grant ends, some items we will not fund include: Stipends Items that can be provided from community partners (example: Healthy Kids Club may provide t shirts for fun runs and jog a thons) Activities or materials not related to wellness If you are not sure your project is eligible, please check with Wellness Specialist or Wellness Coordinator BEFORE YOU APPLY 3

Healthy Kids Club (HKC): Provides technical assistance for school wellness teams and sponsorship/technical assistant for walkathon fundraisers. Programs offered free of charge by HKC include the Schools on the Move 5210 Challenge, BstrongBfit, Healthy Kids/Healthy Start kindergarten education, 5210 Health Heroes, and the Healthy Kids Run Series. HKC also offers classroom resources including Minds in Motion Fit Sticks. a.
