Request for Applications Community-Based Programs and Community-Academic Partnership Programs Overview Nancy G. Brinker promised her dying sister, Susan G. Komen, she would do everything in her power to end breast cancer forever. In 1982, that promise became Susan G. Komen for the Cure and launched the global breast cancer movement. Today, Komen for the Cure is the world s largest grassroots network of breast cancer survivors and activists fighting to save lives, empower people, ensure quality care for all and energize science to find the cures. Thanks to events like the Komen Race for the Cure Series, we have invested nearly $1 billion to fulfill our promise, becoming the largest source of nonprofit funds dedicated to the fight against breast cancer in the world. With funds raised at the Komen National Race for the Cure, grants will be made to support evidence-based strategies and promising practices to reduce breast cancer disparities in Washington, D.C. and seven surrounding counties. Funding Priorities Komen has conducted a needs assessment of the National Capital Area (available at www.komen.org). Drawing upon that assessment, other local and national studies, and over twenty-five years of experience, Komen has identified three funding priority areas: 1. To increase access to breast health information and services to women living in neighborhoods with high poverty rates. 2. To remove physical, cultural and linguistic barriers that women face throughout the entire continuum of breast health care. 3. To engage the faith community in ensuring the delivery of culturally competent breast health information and services.

June 10, 2008 Grant writing Workshop Virginia July 9, 2008 Technical Assistance Sessions Virginia July 10, 2008 Application Deadline August 11, 2008, 5:00 p.m. EDT Award Notification November 2008 Anticipated Funding Start January 1, 2009 Eligibility Applicants and institutions must conform to the following eligibility criteria to be considered for funding: Applicants must ensure that all past and current Komen-funded grants or awards are up-to-date and in compliance with Komen requirements. Institutions must be non-profit organizations located in or providing services to one or more of the following locations: o Washington, DC o Montgomery County, MD o Prince George s County, MD o Alexandria City, VA o Fairfax County, VA o Loudoun County, VA o Arlington County, VA o Prince William County, VA Funding Information Komen for the Cure will award two types of grants through this Request for Applications: Community-Based Programs A single institution may request up to $150,000 over two or three years. The program must include a significant process evaluation 1 component. Community-Academic Partnership Programs A lead agency may request up to $700,000 over two or three years on behalf of a coalition or partnership. The coalition or partnership must include at least one community-based organization and one academic institution. The program must include a significant impact evaluation 2 component.

Submission Process Applications must be submitted using proposalcentral, an online grant application system. All applications must be submitted electronically through the online system, which can be accessed at the following website: https://proposalcentral.altum.com INFORMATION REQUIRED Title: Provide a short, descriptive title for the program. Grant Summary Information Abstract (1,500 characters limit): Provide a brief description of the proposal, including the following: 1) the purpose of the program; 2) a description of key activities; 3) a summary of evaluation methods; and 4) concluding remarks regarding the likely impact of the program. Service Area: Indicate which locations will be served by the program. Target Populations: Identify the primary target populations for the program. Priority Area: Select the primary priority area that your program addresses: physical, cultural and linguistic barriers; neighborhoods with high poverty rates; and/or engage the faith community. Program Description The program description presents the program plan and how this plan addresses the intent of the grant program. The document should be uploaded as a single PDF file, with a page limit of 8 pages, including figures, tables and diagrams. The document should contain one-inch margins and use 12 pt. font. Background: Describe the organization s history, mission, and goals. Describe current programs and recent accomplishments. Goals and Objectives: State the program goals 3 and measurable objectives 4. Explain how the goals and objectives address the selected priority area. Evidence-based Strategies/Promising Practices: Describe the activities that will be conducted to accomplish the goals and objectives. (Narrative, table, or logic model is acceptable.) Describe how your approach uses or adapts evidence-based strategies 5 or promising practices 6. Explain why you chose this approach.

INFORMATION REQUIRED Evaluation Plan: Describe how you will measure that you are achieving the objectives and how you will assess the impact of the program on the priority area selected. Important: Community-Academic Partnership Programs must include a significant impact evaluation component. Organizational Capacity: Describe the organization s experience serving the target population. Describe the other organizations, if any, participating in the program. How does this collaboration strengthen your program. Explain why your organization is best-suited to carry out the program. Describe the steps you will take to ensure that your program and organization are culturally competent. Sustainability: Explain how this program and its impact will be sustained long-term. What resources (financial, personnel, partnerships, etc.) will be needed to sustain this effort over time? How will those resources be secured? Budget Enter budget numbers into the online tool. Funds may be used for the following types of program expenses: Salaries and fringe benefits for program staff Consultant fees Meeting Costs Supplies Travel Other direct program expenses Indirect costs limit to 20% of direct costs Funds may not be used for the following purposes: Clinical services or patient care costs Equipment Medical or scientific research (costs for program evaluation are allowed) Paid media (film, radio, web sites, etc.) Scholarships or fellowships Construction or renovation of facilities Political campaigns or lobbying

For Community Academic Partnership Programs only This document should be uploaded as a single PDF file. INFORMATION REQUIRED Komen for the Cure supports partnership in which responsibilities, privileges and power are shared. The partnership must be documented through a single Memorandum of Agreement (MOA) among all partners. The MOA must specify the roles and resources that each organization will bring to the program. The MOA must cover the entire program period and it must be signed by individuals with the authority to represent the organization. If a signed MOA is not available at time of submission, Letters of Support will be accepted. However, funding will be contingent upon receipt of the MOA. References (No page limit): List references (journal articles, statistics, studies, etc) cited in the grant application. Upload as a separate PDF file. Budget Justification (Two page limit): For each line item in the budget, including personnel, provide a detailed description of how the funds will be used and why they are programmatically necessary. List all other committed and pending sources of support for the program. Curriculum vitae or job descriptions (for new or vacant positions) for key personnel involved in the program (Two page limit per individual). Curriculum vitae should provide education/training information, previous employment, experience, honors and publications relevant to this program. Selection Criteria - Applications will be judged on the following criteria: Impact: Will the program have a substantial positive impact on disparities in breast cancer mortality. How closely does the program align with the funding priorities? Feasibility: How likely is it that the objectives and activities will be achieved within the scope of the funded program? Is the budget appropriate and realistic?

Capacity: Does the organization, Program Director and his/her team have the expertise to effectively implement all aspects of the program? Is the organization respected and valued by the target population? Is it culturally competent? Collaboration: Does this program enhance collaboration among organizations with similar or complementary goals? Sustainability: Is the program likely to be sustained? Is the impact likely to be long-term? Multiple Applications: An organization may apply for both the Community-Based Grant Program and the Community-Academic Partnership Program. However, an organization may not apply for multiple Community-Based Programs or serve as the lead agency for more than one Community-Academic Partnership Program. If you are a current grantee and your grant does not end this year, you are not eligible to apply. An organization may only have one current Community Health and one Community-Academic Partnership Program grant at a time. Common process evaluation methods include patient satisfaction surveys, interviews with program participants, and activity logs. 2 Impact evaluation measures changes in behavior, knowledge, attitudes, or beliefs. Change may be assessed among individuals, providers, organizations, or communities. Common impact evaluation methods include pre/post tests, surveys, and key-informant interviews. 3 A goal is a broad-based statement of the ultimate result of the program being undertaken. 4 An objective is a measurable, time-specific result that the organization expects to accomplish as part of the grant. Objectives are specific approaches to achieve the goal. 5 Evidence-based strategies are programs that have been proven to result in a specific outcome, reviewed by peers, and usually published in a public health or medical journal. 6 Promising practices are programs that have proven successful, but for which there may not yet be enough evidence to prove that it has resulted in a positive outcome. They may also be called emerging best practices.
