Junior 4-H AWARD PORTFOLIO (KAP 7-12 Year Olds) Name Project Years in Project Date of Birth County/District Miami Club How to put together the Junior 4-H Award Portfolio (KAP): Complete this form and submit it for each project. It can be hand written. Contents: Put each form inside a folder and secure pages so they don t fall out. Use Dividers if you are applying for more than 1 Miami County Project Award. Label the front of each cover with your name, county/district, and club. You may expand this form unlike the Kansas Award Portfolio, but it cannot be more than 6 pages. It is acceptable to leave sections blank for young 4-H ers. Put the information in this order: 1. Personal Page from 4-H Record Book (a copy is OK) 2. 4-H Story (Double Spaced on one side of the paper, not to exceed 6 pages. Use the story to expand on your project learning, skills, accomplishments, and personal growth. Recommendations: 7-8 years olds should probably have about 1 page while 11-12 year olds should be closer to the 4-5 pages. In your 4-H Story, highlight all the projects you were enrolled in that year, but concentrate most of your story on the project you are basing your Award Portfolio on. 3. SECTION 6: PROJECT PHOTOGRAPHS Photographs are limited to one page of up to six photos. Photos should be captioned. Some ideas for photos are ones that you are doing things, things you made, or exhibited in the project. You can use your own page for photos if you are unable to get the formatting right on this one. Although not required, you are encouraged to include picture of leadership or citizenship you have done within this project. Photographs: Maximum of six photos. Maximum of 1 page front side only. 5
