The Student Health Fee: As part of the tuition and fees package, allows eligible students the convenience of accessing healthcare on campus. The fee covers the cost of most routine visits. Nominal fees apply for services such as labs, X-rays, immunizations, physicals, and procedures. Services rendered by specialty clinics are billed by the specialists. 2. Student Health Insurance Plan Charge: All NC State students are required to have health insurance while enrolled in classes. A charge for the Student Health Insurance Plan (SHIP) will be billed to each student s account.

Students are required by North Carolina Public Health Law to provide proof of immunizations prior to the start of classes Students are required to: 1. Submit their Immunization Record 2. Complete the TB Questionnaire 3. Complete the Health History Form The Immunization Coordinator will communicate with you through the HealthyPack Portal about the status of your Immunization Compliance. healthypack.dasa.ncsu.edu Federal Health Insurance Portability and Accountability Act (HIPAA) regulations require that students age 18 or older sign a Authorization release form in order for any health care information as well as billing information from Student Health to be shared with parents or guardians. This can be completed by students during the appointment checkin process. For students under the age of 18, we need a consent of care form completed before seeing the student.
