Fidelis SecureCare is a Medicare Advantage Special Needs Plan contracted with Medicare to provide eligible individuals with a clinically superior level of care. Fidelis SecureCare expands coverage beyond original Medicare, adding benefits vital to Member health and well-being, including: Healthcare services delivered on-site, in the Member s residence An individually tailored care plan Complete Part D Prescription Drug coverage all in one plan Paid transportation for off-site medical appointments Preventive care and screenings, plus vision, dental, podiatry services and more Medication monitoring and management Coordination of all providers involved in our Member s care Virtually no claim forms to file or paperwork to manage And, many of these benefits can be yours with low or even no copays. Experienced medical professionals. Consistently attentive care. Our network is comprised of credentialed physicians, hospitals, home care agencies and other clinical professionals selected for their experience and proficiency in geriatric medicine. At enrollment, Members choose a Primary Care Physician who works with nurse practitioners, physician assistants, pharmacists and others in an integrated Care Team. Our Fidelis Care Manager oversees the care delivery process, making sure each Member s individual treatment plan is implemented accurately and efficiently. Our experience in elder care continues to show that frequent and personal attention is indeed the best medicine. Each Fidelis Care Team works one-on-one with Members to help them avoid emergencies by: Increasing the number of on-site visits per month Watching carefully for changes in condition and evaluating symptoms as they arise Providing prompt treatment of disease occurrences or any complications Fidelis makes sure medications are managed properly. Yes. Fidelis SecureCare does cover both Medicare Part B and Medicare Part D prescription medicines all in one convenient plan. How can I compare my options? When you speak with a Fidelis Sales Representative, you will be advised of your Fidelis plan options and you will be given a full Summary of Benefits, which compares Fidelis SecureCare to Original Medicare. The charts in that booklet list most of the important health benefits included in the plan. For each benefit, you can see what our plan covers and what the Original Medicare Plan covers. Our Members receive all of the benefits that the Original Medicare Plan offers, plus additional benefits, some of which may change from year to year.
