Awards Nomination Form The Pike County Branch of the National Association for the Advancement of Colored People (NAACP) solicits public input of award nominees for the 2017 Freedom Fund Banquet Awards. The Branch requests Nomination Forms be completed and mailed to the branch by April 06, 2017. Award Categories Lifetime Achievement - Pike County Resident, demonstrated leadership skills, has gone beyond the call of duty/responsibility. Community Service- Pike County Resident, served without compensation, participated in a number of community service projects beyond that of a board member of a given organization. Outstanding Business Person- owner, partner, or major shareholder of a "for profit" business headquartered in Pike County that has been operational for at least three years. The nominee has exhibited exemplarily customer service practices and/or community involvement/contribution particularly in the African American Communities of Pike County. Unsung Hero/Heroine- person who has enriched the Pike County community by serving as a catalyst for a cause, issue or organization without past recognition or compensation. 3
Lifetime Achievement Award Objective: To recognize and honor an Individual who has exemplified leadership in the community through organizational efforts to improve the quality of life or lead the way in creating a safe and drug free community for all Pike County Residents. Example: Organized community events to address improve the quality of life and create a safe environment. Criteria: Any resident living within Pike County Demonstrated leadership skills One who has gone beyond the call of duty and responsibility? Categories: There will be one award presented in this category Judging Elements Did the community benefit? The number of activities organized and implemented Did activity have an impact on the community? Ability to include other organizations and entities Created ongoing collaborations Statistics to improvement in life and environment 4
Community Service Award Objective: To recognize and honor an individual who has served unselfishly in Pike County; one who has participated in events that impact and improve the quality of life for Pike County Residents. An individual who has demonstrated personal and financial sacrifice by serving the community. Example: Devoted sweat equity and financial contributions Criteria Any resident living within Pike County Served without compensation Participated in a number of community service projects Categories: There will be one award presented in this category Judging Elements Positive and visual impact within the community Created an atmosphere for ongoing service Highlights of service rendered 5
Outstanding Business Award (1 of 2) Objective: To recognize and honor businesses that have demonstrated "Best Practices" of entrepreneurship such as fiscal responsibility and work; possess Innovation in operating their business within Pike County and has demonstrated efforts to engage the entire community. Example: sponsorship and support of community events, community involvement beyond normal business practices particularly in the African American communities In Pike County. Criteria Any for-profit business headquartered in Pike County The nominee must be the owner, partner or major shareholder of the business, and active in its day-to-day operations The business must have been in operation for at least three years Categories: There will be one award presented in this category. Judging Elements Business Growth and Performance Sound Business Strategies and Practices Sound customer Service Strategies and Practices Unique and Innovative Approaches Community Involvement and Contribution Measurable efforts of increased workplace/clientele diversity 6
Outstanding Business Award (2 of 2) All of the NAACP Freedom Fund Awards shall be determined based on the particular category requirements. Nominations may include nominations from the Nominating Committee or community. Applications must be received prior to consideration. Award period covers fiscal year ONLY. If there are more than one nominee per category, Committee shall rate each applications based on level of approved criteria. Nominees must be a resident of Pike County. Nominations must be made by Freedom Fund Awards Committee or individuals at least 30 days before Freedom Fund Banquet. The Chair of the committee must sign the nomination, submit to Branch for approval, and signed by President of the Pike County NAACP Branch. 7
Unsung Hero/Heroine Award Objective: To recognize a resident of Pike County who has enriched the area by serving as a catalyst of a cause, issue or organization without past recognition or compensation. Example: Organized community events to address improve the quality of life and create a safe environment. Criteria Pike County Resident Initiated a cause, issue, or formation of an organization that impacted the African American Community in general or a specific person within the community. Served without any consideration of personal compensation or recognition Category: No more than one award in this category annually. Judging Elements No past recognition or compensation. 8
