The owner, John Holbrook, is the 5 th child born to L. Dale and Beverly Holbrook. John graduated from Olympus High School in 1977. He graduated from Cyprus College of Mortuary Science with an Associate of Science Degree and a Mortuary Science Degree. John was awarded the Scholastic Achievement Award for scholastic excellence and the Roslin R. Ragsdale Award for outstanding student leadership. John married Sandy Cochran Holbrook in 1981. Sandy graduated from the University of Utah with a Bachelor of Accounting Degree as well as a Master in Professional Accountancy. Eldon Barton graduated from the Mesa Community College Mortuary Science Program in 2007, receiving the Mu Sigma Alpha award for graduating at the top of his class. He has worked with the Holbrook family as a licensed mortician since graduating from college. He assists families with funeral and cremation arrangements, funerals, visitations, office management, and other areas in funeral management. John Coomans started working with the Holbrook family mortuary after retiring as a Division Director with the Utah Insurance Department, regulating the insurance industry. He assists the Holbrook staff with the preparation and coordination details of funeral visitations, viewings and services. He has vast experience in officiating and directing funerals. He and his wife Stance have resided in the Canyon Rim and East Mill Creek communities of Salt Lake County for over forty-five years. Mortuaries are businesses that will always have a place in society. If you look closely, you ll see there s one in just about every corner of town. The important thing is knowing which one is going to provide you the best service during that difficult time. Holbrook Mortuary here in Salt Lake City is one you can count on. It s been around since 1941. John Holbrook owns the mortuary. He took over the business that his father L. Dale Holbrook started. John grew up watching his father work as a mortician and decided that was a career he wanted to pursue. He graduated from the Cyprus College of Mortuary Science with an associate of science degree and mortuary science degree. John s wife Sandy also helps him run the mortuary. She earned a master s degree in professional accountancy and manages the financial side of the business. The Holbrooks goal is to provide professional service with a home-like atmosphere. The Holbrooks hold true to that statement. I ve seen the mortuary and it resembles a home. They take pride in offering quality merchandise for the best possible prices. They have a wide selection of caskets from four different manufacturers, burial vaults from two different manufacturers, headstones, four manufacturers of cremation urns, flower ordering and arrangements, a full line of temple clothes and traditional clothing, American flag display cases, a wide variety of printed programs, registration books and thank-you cards and a typewritten book of services from permanent records. Their services include transfer of the deceased when death occurs; embalming, dressing and preparation; legal paperwork and death certificate; arrangements for viewing and visitations; accommodations for church or mortuary service and all cemetery arrangements. They ll also prepare obituaries for any published paper and even offer 24-hour professional help. Programs: Holbrook Mortuary offers quality color programs. We will assist you in funeral program outline, organist, vocal solos, and church or clergy notification. 9. Veteran Benefits: Holbrook Mortuary will assist in applying for Veteran Burial Benefits, burial at any veteran cemetery, veteran headstone, and all claims. A copy of the veterans DD214 (honorable discharge) will be needed to complete the application process. 10. Cremation Urns: Holbrook Mortuary has large selection of Wood, Marble, and Metal urns available. 11. Social Security: Holbrook Mortuary will assist you in notifying Social Security of death and applying for any Social Security benefits. 5

GENERAL PRICE LIST * Effective Feb 1, 2016, but subject to change without notice * The goods and services shown below are those we can provide to our customers. You may chose only the items you desire. However, any funeral arrangements you select will include a charge for basic services and overhead. If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. All prices are referred to on a cash basis. PROFESSIONAL SERVICES... $1,945.00 Our professional charge includes but is not limited to consultations with the family and clergy (Mon to Fri from 9am to 2pm), arrangement and direction desired, preparation and filing of necessary notices, mortuary will schedule funeral and visitation dates and times on a Monday to Friday 11am, authorizations and consents, and other services and attendance prior to, during and following the funeral, e.g. cemetery, crematory, etc. Also general costs of doing business, e.g. insurance, licensing, taxes, etc. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains). Cash payment is required and is considered payment 24 hours before funeral services are rendered. Holbrook Mortuary offers a casket purchase discount (875) for those families who select one of our caskets. Holbrook Mortuary will also offer a funeral day schedule discount (300). PREPARATIONS (Transportation/Embalming/Care/Shelter) Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have a right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.
