1115 PSH Eligibility Meet one of the following criteria: 1. Chronically Homeless persons with disabili@es (as defined by HUD), or 2. Frequent or lengthy ins@tu@onal contacts, or 3. Frequent or lengthy adult residen@al care stays, or 4. Frequent turnover of in-home caregivers or providers, or 5. PRISM Risk score of 1.5 or above

ALTSA PSH eligibility requires... 1. 1115 PSH eligibility PLUS 2. Meets ALTSA target popula<on eligibility (aged, blind or physically disabled) which includes: A Medicaid financial determina@on and An assessment of an individual s func@onal unmet needs

Comprehensive Assessment Repor<ng Evalua<on (CARE) Single assessment tool used across aged and disabled popula@ons. The assessment performs the following func@ons: Func@onal eligibility determina@on for state plan and waiver services Medical Psych/social ADLs/IADLs Iden@fica@on of service plan needs Service Episode Record (SER): case notes Client Details (demographics and case management tools) Included are other assessments such as the Mini-mental Status Exam, Iowa Depression Scale, Cogni@ve Performance Scale, Zarit Burden Scale, an alcohol/ substance abuse screening tool and the Supports Intensity Scale.

What You ll Need to be an ALTSA Provider Client Service Contract Meet general contractor requirements* Experience to provide services per contract Business License Administra@ve and accoun@ng procedures Criminal history background check Insurance requirements Sufficient staffing, no history of significant deficiencies, etc. Have a specific commitment to: Permanent Suppor@ve Housing and Housing First principles, HUD s health and safety standards for affordable housing, State and federal privacy and security regula@ons, Harm reduc@on principles, and The provision of services to people with complex long term care needs. * hlps://www.dshs.wa.gov/altsa/home-and-community-services/informa@on-poten@almedicaid-contractors

Provider Qualifica<ons 1. The Suppor<ve Housing Agency must have at least one year of demonstrated experience and ability to provide services per the specifica<ons in this contract and maintain all necessary licenses, registra<on, and cer<fica<on as required by law. Employees of the Suppor<ve Housing Agency providing Community Choice Guide: SH Specialist services must meet the following criteria: Bachelor s degree in in a related field with one years experience in the coordina@on of suppor@ve housing or in the coordina@on of independent living services in a social service sejng, or Two years experience in the coordina@on of suppor@ve housing or in the coordina@on of independent living services in a social service sejng under qualified supervision. If the services to be provided require licensure or cer@fica@on, the employee shall have the applicable license or cer@fica@on, which shall be current and in good standing. 2. Cer<fica<on of commitment to Suppor<ve Housing quality standards include the following to ensure delivery of quality services and common program prac<ces across provider agencies

ALTSA Suppor<ve Housing Providers Will have a contract with ALTSA that will cover: Ability to bill hourly for pre-tenancy or post-tenancy supports Ability to bill daily for permanent suppor@ve housing services Will par<cipate in: Con@nuous improvement processes, including joint learning circles/fidelity reviews with the Behavioral Health Administra@on (BHA) to assure provider consistency Data collec@on to determine outcome measurements

Authoriza<on: HCS or AAA authorizes PSH services ALTSA HQ prescreen: email or phone call Referral to ALTSA case manager (CM) and connec<on to PSH provider (hourly billing) PSH provider locates housing, works with CM to arrange move Client move-in (daily service billing)
