Catholic Health Services of Long Island ( CHS or the System ) operates six hospitals (and one affiliated) and offers a wide range of services throughout Long Island, including three nursing homes, hospice, and the largest regional home care group in Long Island and a community-based agency for persons with special needs CHS has maintained a steady market share of approximately 22% in Long Island and is an essential provider in Nassau and Suffolk and Eastern Queens CHS is in the midst of a transition from a volume based (heads in beds) fee for service provider to an integrated delivery system designed to manage the health needs of a population CHS has made investments in Electronic Medical Records ( EMR ) EPIC and all six hospitals and 100 s of physicians both employed and voluntary are up and running In 2012 CHS embarked on a journey to create a physician hospital organization, the purpose of which was to contract with all payors commercial, Medicare and Medicaid in an Accountable Care model Today, over 700 physicians are in the CHS PHO
In Q4 2013, CHSLI and SJEH entered into a clinical affiliation agreement. SJEH in 2012, 9,127 discharges of which 8,292 or 90.85% (safety net) were Medicaid or dual eligible. CHS commits to SJEH that it will apply as a lead hospital in a DSRIP network to include SJEH and 3 CHS hospitals in Nassau County. In early 2014, DSRIP is officially announced.
1. On October 1, 2014, CHSLI completed the acquisition of 50% of BHP and its related companies BIPO and BIPA. St. John s Episcopal Hospital (SJEH) is a partner in our 50% and other hospitals are considering buying in. CHSLI has a PHO with 700 doctors, Beacon has over 300 combined plus 1,000 physicians 2. BHP is an ACO that participates in the MSSP and has 20,000 lives 3. BIPA has contracts with a number of commercial MCO s and has approximately 40,000 lives 4. SJEH has a risk contract with Health First covering 12,000 Medicaid lives 5. We anticipate that in the 2 DSRIP networks, we will be in managing an additional 180,000 lives BEACON HEALTH PARTNERS
