Backround The Day Hospital Association (DHA) is an umbrella organisation representing registered private day hospitals (77 licenses) throughout South Africa. Individually, these hospitals offer distinct comfort and cost advantages to patients in their areas of operation. Collectively, the DHA strives to promote the advantages of day hospitals in terms of reducing the cost of private healthcare, to bring the advantages of day hospital treatment to the attention of doctors, medical schemes and patients and to create awareness of the national footprint of day hospitals. Discovery DAY HOSPITAL ASSOCIATION Recent publications by Discovery Specialist are not moving, patients are followers. Consumers have no incentive to be aware of hospital costs as schemes normally cover these costs. Needs to be benefits for specialists. Alternative reimbursement mechanisms are needed in future. Creating networks with service providers. Fee for service. Discovery s role to manage costs negotiate lower healthcare costs hospital tariffs.

Recent publications by Medscheme Medscheme Similar findings have been reported by Medscheme. It is crucial that by partnering with Medical Schemes viable day facilities will be created thereby increasing the national footprint. Proposed regulation change to be put in place to improve competition.

How can we make healthcare more accessible to the general public? Day Hospitals can focus on specialization: International statistics confirm Day Hospitals are 20-25% more cost effective. Excellent quality outcomes Accommodate high volumes Assist acute hospitals to create more theatre time Quick turn around in theatre Focussed on shorter procedures Make use of the same staff/shifts better team approach Perform procedures only on healthy patients less risk Less capital intensive facilities do not perform all the different acute procedures ARM and fixed fees require better volumes Partnering with schemes on lower options Work with specialists performing the procedures Alternative option create healthy competition

What quality measures are being focused on? The DHA has set up a task team to begin measuring quality standards in all our members facilities, focusing on the below: Client Satisfaction Prevention and control of infection Clinical risk assessments Clinical audits of heath records Training competent staff Health & Safety measures Financial management

Fixed Fees The current fixed fees below with some funders represent approx. 60% of the current procedures done in a day facility: Abortion spontaneous Eyelid lacrimal surgery minor Arthroscopy shoulder therapeutic Haemorrhoidectomy major Breast Minor Procedures Male genital minor procedures Cataracts Peripheral nerve neuroplasty Circumcision all except new born Tonsillectomy and Adenoidectomy D&C GIT scopes 1 Day

Patient Safety The same specialists that work in an acute hospital work in the day hospitals Training of the theatre and ward staff equates to the same level of experience of staff in acute hospitals New developments in anesthesia and surgery have led to reduced procedure time with less invasive procedures, improving safety Stringent requirements by licensing authorities Agreements in place with nearby acute hospitals to admit patient should something go wrong.
