Provide basic first aid care and emergency care for students and staff. 2. Assess the health status of students and ensure they have appropriate IHP s with collaboration from parents, students, teachers and physicians. 3. Maintain current health records of all students on the computer and complete appropriate submission of state reports to the district. 4. Comply with the procedures for submitting staff injury and student accident reports. 5. Comply with the policies of the district and state laws relative to the administration of medicine to students. 6. Comply with the district s policy regarding the reporting of communicable diseases to the local health departments. 7. Notify parents regarding their student s illness or injury, informing parents of action taken and seeking further instructions from parents. General Bidding Instructions and Conditions 1. Bidders must furnish an IRS W-9 form indicating the bidder s taxpayer identification number, if not already on file with the District. The W-9 must be dated within the current calendar year. 2. As a condition for award of any contract for services, the bidder must provide a sworn affidavit and documentation affirming enrollment in E-Verify and stating the bidder does not knowingly employ any person who is not authorized to work in the United States (per RSMo 285.530). 3. Bidders must include a certification of qualification for an LPN or RN. 4. Bidders are solely responsible for all costs incurred in the preparation of responses to this RFP. Required materials in the response are the bid form (below), W-9 form (if required), and a quote for services. 5. To be considered for this project, vendors must deliver bids to the electronic web address above prior to 3:00 p.m. CT on May 1, 2017 which will then be publicly open and read. The District will reject any bids received after the deadline. 6. Certification of Non-Collusion: The bidder, by offering his/her signature to this proposal, agrees to the following: Bidder certifies that this bid is made without any previous understanding, agreement, or connection with any person or firm, or corporation making a bid for the same items, and is in all respects fair without outside control, collusion, fraud or otherwise illegal action. 7. Reservation for Rejection and Awards: The District reserves the right to accept or reject any or all bids or parts of bids, to waive irregularities and technicalities, and to request rebids on the requested materials. The District also reserves the right to award the contract on such material the District deems will best serve its interests. It further reserves the right to award the contract on a split order basis, lump sum, or individual item basis, or such combination as shall best serve the interests of the District unless otherwise stated. The District also reserves the right to waive minor variations to specifications. 8. All bidders must furnish unit prices with extended totals and total sum of bid. In the event of conflict between unit price and total, the unit prices will be used to determine the successful bidder. 9. Payment: All invoices shall be submitted to the Board of Education Central Office, 1002 South Jefferson St., Kearney, MO 64060 in attention to Jill Lytten. Said invoice shall be due and payable within thirty (30) days following written approval by the proper Department director. 10. All bidders must agree to respect the following timeline: a. April 20, 2017: RFP Released b. May 1, 2017: Bids Due c. May 5, 2017: Bid Winner Announced 11. Failure to comply in any manner with applicable statues, ordinances, or codes shall result in said contractor replacing the goods, services and/or work performed in order to effect compliance or in liquidated damages in the amount required to effect compliance with said statutes, ordinances and codes together with any costs associated with collection of said damages.
