Placement Information Clinical Area: Older Persons Mental health. Northumberland, Tyne & Wear NHS Foundation Trust is one of the largest mental health and disability Trusts in England employing more than 6,000 staff, serving a population of approximately 1.4 million, providing services across an area totalling 2,200 square miles. Location: Placements may be based at either: Barton centre, Cherry Knowle Hospital, Ryhope, SR2 ONB Tel: 0191 566 7063 or Monkwearmouth Hospital, Newcastle Road, Sunderland, SR5 1NB, Tel: 0191 569 9149 / 0191 569 9050 Core hours: Core Hours 09.00-17.00 Working hours may be negotiated with practice placement educator depending on service needs. Half an hour is available for lunch. Dress Code; Uniform is not required. Students are asked to dress informally but smartly. Blue denim is not acceptable. Dress should reflect a professional image. Students should also ensure they display a current identity badge at all times. Supervision: Each student will have a minimum of one hour of supervision weekly with his/ her supervisor. Additional supervision, of a more informal nature, will be provided in relation to student needs. Food: Meals are usually eaten within the department. Kitchen facilities are available to store and prepare food. Vending machines and canteen facilities are presently available on both sites. Transport: A number of buses run to both the hospitals. The Occupational Therapy Team The Occupational Therapy team consists of a Head Occupational Therapist, Band 5 and 6 Occupational Therapists and a Technical Instructor. Occupational Therapy Service Overview The Older person s mental health Occupational Therapy department provides Assessments and Interventions to people aged 65 years and over. This includes those with organic illness such as dementia, (includes younger people with dementia) and those with functional illness such as psychosis, bi-polar illness and depression A service is provided across South Tyneside and Sunderland into in-patient wards, day units. Memory protection service and the community mental health team (this includes memory clinics, planned home treatment and challenging behaviour team). Cherry Knowle Hospital Hawthorn ward provides assessment, treatment and rehabilitation by a multi disciplinary organic disorders such as dementia. Rosewood ward provides assessment, treatment and rehabilitation by a multi disciplinary functional disorders such as depression Sycamore Ward provides assessment, treatment and rehabilitation by a multi disciplinary functional or organic disorders displaying challenging behaviours that cause significant problems for either themselves or the setting they are living in. Monkwearmouth Hospital Community Mental Health Team Provide specialist assessment, treatment and care for older people with mental health problems arising from functional or organic disorders within South Tyneside and Sunderland locality. Also based within this team are the Challenging Behaviour and the Home Treatment Teams. Grange Day Unit Assessment, treatment and rehabilitation by a multi disciplinary health and social care team for older people with mental health problems arising from functional or organic disorders. Wearmouth View provides specialised long term care for people with complex mental illness to ensure that seamless integrated care pathways are provided for older people with mental health NHS long term care needs Memory Protection service A newly commissioned service which focuses on early diagnosis and intervention for those presenting with memory problems. South Tyneside District Hospital Ward 18 provides assessment, treatment and rehabilitation by a multi disciplinary health and social care team for older people with mental health problems arising from functional disorders such as depression Palmers community Hospital Ward 1 provides assessment, treatment and rehabilitation by a multi disciplinary health and social care team for older people with mental health problems arising from organic disorders such as dementia. 3

Occupational Therapy Process Referrals Occupational therapy referrals are received from all members of the multidisciplinary team including consultants, nursing staff from the wards, day unit and memory clinics, CPN s, Social Workers and Physiotherapists. Assessment Following referral an initial assessment of need will be undertaken. This will seek to identify any evident problem areas which inhibit daily living. It will also take into account the strengths of the individual. Individual functional assessments examine the impact of mental state on occupational performance and the use of occupations as tools in the diagnostic procedure. Occupational therapy aims to enable individuals with mental illness to return to maximum possible independence using a holistic approach to identify their needs in personal care, domestic activities, leisure and social activities, relationships and emotional distress. Intervention A range of Occupational Therapy interventions are carried out according to individual need. These may include; Recommendations regarding domiciliary and social support Maintenance of existing skills in occupational performance areas Recommending and providing equipment Environmental adaptations Advice to carers Relaxation/anxiety management Individual rehabilitative work Group work 1:1 work Documentation Within the department the Model of Human Occupation is used as a framework. Documentation has been developed to support this. Other approaches and assessment tools are used as indicated. NTW NHS Trust utilises RIO an electronic patient record system. IT account, RIO access and ½ day training will be arranged at the commencement of placement. 5
