AgrAbility National Training Workshop Farmer/Rancher Scholarship Application The National AgrAbility Project is pleased to offer a limited number of scholarships to farmers, ranchers, and other agricultural workers with disabilities, 1 and their spouses/caregivers (when applicable) to attend the 2011 National Training Workshop (NTW), scheduled for November 7-10 in Indianapolis, Indiana. Scholarship applications are due by September 2, 2011. All scholarship funds must be used exclusively for costs of attending the NTW, including: o Cost of NTW registration for AgrAbility customers o NTW sponsored hotel accommodations up to 4 nights November 7, 8, 9, & 10 at the Hilton Hotel and Suites in downtown Indianapolis, IN ($134.55/night, including taxes & fees) NOTE: While we have 12 accessible rooms reserved at the Hilton, it is possible that these may be booked by the time scholarships are announced. Therefore, scholarship recipients needing accessible rooms may be asked to stay at the Hyatt Regency, which is approximately one block from the Hilton. o Approved travel (airfare and transportation to/from airports, OR if driving, mileage reimbursement at the rate of $0.51 per mile roundtrip to and from the NTW up to 800 miles roundtrip based on Google Maps mileage.) o Approved meals not covered by the registration cost. 2 Registration and hotel accommodations (excluding incidentals) will be paid by the National AgrAbility Project. Airfare (if required), approved meals, and mileage will be reimbursed to scholarship recipients AFTER the NTW. Original receipts MUST be presented for reimbursement of each expense claimed with the exception of meals, which will be reimbursed using a per diem rate. Actual scholarship amounts will be determined by the number of applications received and the amount of sponsorship funds available to the National AgrAbility Project. Full or partial 1 Both AgrAbility customers and farmers/ranchers from states not currently served by AgrAbility projects are invited to apply for scholarship funds. 2 U.S. General Service Administration guideline for Indianapolis will be followed: $10 for breakfast, $15 for lunch, and $30.75 for dinner applied only for those meals NOT provided during the NTW conference. Meals not included in registration are lunch and dinner on November 7, dinner on November 8 & 10, and breakfast and lunch on November 11 if staying overnight on the 10th. All meal reimbursements are dependent upon arrival/departure times. This application package is based on work done by the 2000-2008 National AgrAbility Project. Page 1
