As Pauline Mountain had given her apologies for this meeting Mark Lupton welcomed all and a round table of introductions were carried out. 2 Minutes and Matters Arising. The minutes were read and agreed as an accurate record. Updates to the Action Log were given (as attached) with no further comments made. 3 HWL Update. A handout of the summaries which highlighted key themes and issues as reported to HWL for the months of February, March & April 2016 were made available to each attendee. Project work plans for 2016 ML explained to the group that HWL would be delivering fewer projects this coming year however advised that these were chosen to have the most robust impact upon outcome. These projects included: Delayed discharge Prescriptions and medications Care services Access to services for people living in rural communities Specialist dental services CAMHS (Children and Adolescent Mental Health Services) Preventative services Cancer services Diabetes Dementia Further details to these projects will become available as and when. Healthwatch Hub Network. ML reminded the group that the HWL Hubs network is still growing and for interested organisations/support/social groups to make contact with him for more information or to join this network: mark.lupton@healthwatchlincolnshire.co.uk Patient Stories. HWL are looking for patients or users of health and social care services to come forward and share their experience which in turn can be fed up to Trusts and Commissioners at board level to give a more powerful viewpoint of a patient pathway. RH presented the group with the latest update on the Care Portal (presentation attached). Questions raised by this group have been collated with those from around the other 3 provider network meetings and are to follow these minutes. 5 Lincolnshire Health & Care Self Care RH gave a presentation on the Community Directory of Services (presentation attached). Questions from the floor included: Q. How often is site updated? A. Updated information directly from groups listed. Contact Lincs2Advice to input anything incorrect/missing.
Q. Working group asked a number of questions of LHAC concerning Directory. Not yet received a response. Alzheimer s Society not listed on site where does the information come from? A. JT Forms may have been completed by a colleague as far back as a couple of years ago but now may need updating. Contact them directly. Take details and feed through to Lincs2Advice. Who???? Q. Any promotional materials for Lincs2Advice? A. Yes business cards etc. Will be at Lincs show promoting care portal and Lincs2advice. Q. Suggest screensaver to highlight service? A. Will take back to Lincs2Advice. 6 Provider Updates and Issues. VC Terrence Higgins Trust screening coordinator/health promotion in south Lincs. Services include HIV point of care testing. Currently trying to get the message out to hard to reach groups in the community. They have issues with traveller postcodes. Stigma is also a big issue can cause problems trying to get the message into certain public venues (bars etc). RB LPFT working with wellbeing to work for MH patients improve effectiveness Centre of excellence status received. Not providing in some areas Boston not covered and not a lot of cover in east and Spalding/Stamford. Take referrals from integrated community teams. Like all services they need more staff. AS JT CL GW SM KT JS Headway Lincs support patients/families. Support groups in county. Receiving funding for full time post for 3 years from Bromhead Charity. Wellbeing service/age UK they support ages 18 and upwards with things like telecare, adaptions etc. Self-Care Project Lincs West CCG piloting care navigator service. Discharge service & resettling service. Support individuals for 72 hrs following discharge. Also offer advocacy for older person. HWL volunteer Out in the community on behalf of HWL listening to views from members of the public about things such as hospitals/gps. Common issues include: discharge problems, no beds available A&E, no results when attending appointments etc. Lincolnshire CVS (Community Transport) Transport is still an issue. Patients are finding themselves not eligible for transport home because they can drive. They do link in with NSL and when dealing previously with them directly find the patient is entitled to transport. Free transport for health related/social transport. Advocate for POWHER They are the designated service within the county to support people who need to raise complaints regarding care and treatment funded by the NHS. Making sure that issues are fed back to services so that they are aware when there is a problem and can work to put things right. PALS (Patient Liaison and Advice Service) within the NHS Trusts do not deal with complaints - they deal with concerns which are issues that can be resolved within a few working days and do not require an investigation. Steps2Change talking therapy service. Various sites. MH work counselling/cbt. Wait for service strive to get wait timing down. Lot of work is done by telephone looking at developing online services. Alzheimer s Society They offer information and signposting. Support in various ways, ie power of attorney, until end of life, choosing care home etc. Provide learning programme for carers. Can profile match patient with a volunteer. Free service. Issues are GPs referring into the service, poor referral rate only couple from Lincoln, none east. Anyone can self-refer if they are a resident of Lincs. Why is there poor referral rate? GPs previously wouldn t diagnose because there were no services available, but now lot of services. Re-educate GPs? Advice to promote through practice managers. Already been to 14 surgeries but no referrals yet. Offered dementia friendly training.
LS Butterfly Hospice Provide respite/end of life support. 300 patients through unit. Only one person using bed last week but know there are people who need so trying to promote and raise awareness with Drs/GPs. Not just for cancer end of life. Provide respite for those in last year of life for maybe a week. Frustrating when people who need it are not being referred. TM LCHS - Moving towards consistent urgent care service. Patient access is walk in. Out of hours service by 111 appointment/home visit. Transport is a massive issue. People who call but have no transport, in east especially. Objective is to keep people at home. Lot of change going on. LACE transport did help previously. KW LPFT adult inpatient division - Recently employed 2 bed managers to look at adult inpatients and those placed out of county e.g. are they appropriately placed? News on PICU (Psychiatric Intensive Care Unit) unit in Lincoln, they will hear something later this year maybe, as long as planning permission granted, so another 10 beds in county for patients. KT LPFT Steps to Change Looking to publicise the service in local communities. Voice to change forums. Anyone can self-refer. AN LPFT CAMHS (Children and Adolescent Mental Health Service) Professional advice line/new crisis home treatment service 24 hrs. Out of hours provide eating disorder service running now 2 years plus. Pathway through service = problem is that people stay so long. Issues with south GPs giving incorrect advice and not referring. Service gap for young people with autism. Monitoring all GPs. Can keep in touch with HWL. RH LHAC On track for consultation in Nov. STP (Sustainability and Transformation Plans) on track. Mid July for options for public consultation. Case for Change document ready to go back end of next week. 7 Evaluation and Actions to be taken away.
