Grant applications now being accepted for Breast Health and/or Breast Cancer Education, Treatment Support or Screening Projects REQUEST FOR APPLICATIONS The Coastal Georgia Affiliate of - along with those who generously support us with their talent, time and resources - is working to better the lives of those facing breast cancer in our community. We join more than 100,000 breast cancer survivors and activists around the globe as part of the world s largest and most progressive grassroots network fighting breast cancer. Through events like the Komen Savannah Race for the Cure, we have invested over $1 million in local breast health and breast cancer awareness projects in 9 counties throughout Coastal Georgia. Up to 75 percent of the net proceeds generated by the Komen Coastal Georgia Affiliate stay in the Coastal Georgia area. The remaining income goes to the national Grants Program for energizing science to find the cures. ABOUT SUSAN G. KOMEN: Nancy G. Coastal Georgia Affiliate RFA page 2 of 6 STATEMENT OF NEED: Applications will be accepted for any breast health or breast cancer screening, treatment, education, or support project within the Coastal Georgia Region: Bryan, Bulloch Camden, Chatham, Effingham, Glynn, Liberty, Long and McIntosh Counties. However, projects that specifically address the objectives outlined below will be given priority. The findings from the 2011 Komen Coastal Georgia Affiliate Community Profile revealed an increased need for access to care for the underserved throughout Coastal Georgia. The 2011 Community Profile can be found on our website at www.komencoastalgeorgia.org. Drawing from the profile, Komen Coastal Georgia Affiliate has identified the following priority areas: 1. Projects focusing on increasing the number of women who receive breast health services outside of metropolitan areas. 2. Projects that offer low-cost or free breast screening and diagnostics for lowincome and under-insured women who are not eligible for BCCP or cannot receive BCCP services. 3. Projects that increase the number of educational and outreach programs that can demonstrate successful outcomes of increasing the number of women seeking and receiving mammography services, particularly in African American, Latina and Military Populations. 4. Programs that increase navigation services promoting seamless care along the breast health continuum from education through diagnosis, treatment and follow up care. GUIDELINES AND INSTRUCTIONS FOR APPLICANTS The purpose of this program is to address the breast health and breast cancer screening, treatment, and education needs of Coastal Georgia. ELIGIBILITY: The maximum grant request is $60,000.00. Any proposed Project which seeks grant funding in excess of this amount will not be reviewed or considered. Project must be specific to breast health and/or breast cancer; e.g. if a project is a combined breast and cervical cancer project, funding may only be requested for the breast cancer portion. Applicants must be a federally tax-exempt organization under the Internal Revenue Service code, i.e., a nonprofit organization, educational institution or governmental or public agency. Proof of federal tax exemption must be provided in the Budget Summary section of the application. Applicants must provide proof of liability insurance in the Budget Summary section of the application. Services must be provided within the Komen Coastal Georgia Affiliate service area: Bryan, Bulloch, Camden, Chatham, Effingham, Glynn, Liberty, Long and McIntosh Counties. All past and current Komen-funded grants or awards to applicant are up-to-date and in compliance with Komen requirements. Proposed projects must not duplicate services that are already provided by or an existing Komen Coastal Georgia Affiliate grantee. Institutional grantees submitting more than one (1) grant proposal in any grant cycle must demonstrate appropriate internal support, cooperation and coordination among programs and/or services to meet Komen guidelines
Coastal Georgia Affiliate RFA page 4 of 6 If applicant, or any of its key employees, directors, officers or agents is convicted of fraud or a crime involving any other financial or administrative impropriety, then applicant is not eligible to apply for a grant during this current cycle and will not be eligible to apply for a new grant until the later of 12 months after the conviction or until applicant can demonstrate that appropriate remedial measures have been taken to ensure that any criminal misconduct does not recur. ALLOWABLE EXPENSES: Funds may be used for the following types of program expenses: Salaries for program staff: Salaries will be permitted only if necessary to the existence of the program. Clinical services or patient care costs: the amounts requested for services should be based on the cost of the services, and not on a usual or customary charge Meeting costs: Conference, meeting, travel and supply costs that are included in proposed budgets must be reasonable and directly related to the Project s objectives and activities Supplies Travel Other direct program expenses Indirect costs: if applicable, should be no more than 12% of direct costs. Equipment costs: if applicable, may not exceed 30% of direct costs and should be used exclusively on this project. Funds may not be used for the following: Medical or scientific research Scholarships or fellowships Construction or renovation of facility Political campaigns or lobbying Endowments Debt Reduction EDUCATION MATERIALS: is a source of information about breast cancer for people all over the world. To reduce confusion and reinforce learning, we require that grantees provide educational messages and materials that are consistent with those promoted by Komen, including promoting the message of breast self-awareness and knowing your risks for breast cancer. Coastal Georgia Affiliate RFA page 5 of 6 SUBMISSION REQUIREMENTS: All applications must be submitted online at http://affiliategrants.komen.org. Applications mailed in will no longer be accepted. Applications must be submitted before December 13, 2013 at 5:00 pm. The online application system will not take applications after this time. No late submissions will be accepted. REVIEW PROCESS: Each grant application will be reviewed by at least three independent reviewers. They will consider each of the following weighted selection criteria: Impact (25%): Will the project have a substantial positive impact on increasing the percentage of people who enter, stay in, or progress through the continuum of care? Will the project have a substantial impact on the priority selected? How closely does the project align with the funding priorities stated in the RFA? Does the project have a sufficient and documented plan to evaluate its impact? Feasibility (20%): How likely is it that the objectives and activities will be achieved within the scope of the funded project? Is the project well planned? Is the budget appropriate and realistic? Does the budget justification explain in detail the reasoning and need for the costs associated with the project? WEIGHT Capacity (25%): Does the organization, Project Director and his/her team have the expertise to effectively implement all aspects of the project? Is the organization respected and valued by the target population? Is it culturally competent? Collaboration (15%): Does this project enhance collaboration among organizations with similar or complementary goals? Are the roles of the partners appropriate and relevant? Sustainability (15%): Is the project likely to be sustained? Are partnerships likely to be sustained past project period? Is the impact likely to be long-term? The grant application process is competitive, whether or not an organization has received a grant in the past. Funding in subsequent years is never guaranteed. CONTRACTS: A grant contract will be the legal mechanism for funding. All approved grantees will be required to execute a contract with the Affiliate prior to receipt of funds or initiation of Project activities. PAYMENT AND REPORTING: A payment of 50% of the grant award amount will be made to a grantee within thirty (30) days after receipt of the fully executed contract. The first progress report is due at the end of the first six (6) months of the contract. Provided that the mid-year site visit and progress report show that the grantee has demonstrated satisfactory performance and the need for
Coastal Georgia Affiliate RFA page 6 of 6 additional grant dollars, the remaining 50% of the grant funds will be released within fifteen days of receipt of such satisfactory progress report. A final report is due within forty-five (45) days of completion of the grant period. LETTERS OF SUPPORT AND ADDITIONAL MATERIALS: Submission of additional materials (e.g., reprints, complete curriculum vitae or letters of support) not specifically requested by this RFP are unwelcome and will not be reviewed or considered. CONFIRMATION OF RECEIPT OF APPLICATION: Confirmation of receipt of application will be sent via email communication to the project director following review for compliance to guidelines. If immediate confirmation of receipt is requested, please include a self-addressed, stamped postcard that will be returned to you immediately upon receipt of the application. Please do not contact the Komen Coastal Georgia Affiliate regarding the status of the application during the review period. ANNOUNCEMENT: Announcement of grants awarded will be made in February 2014. Project directors will be notified of the outcome of the review via email communication from the Grant Chair or Mission Coordinator.
