Completing the FIT form The FIT form won t take long to complete, but will ask for specific details throughout. It will be helpful to have the following information to hand: Your ENGIE Account number Supply MPAN number MCS certificate or ROO-FIT number (as applicable) EPC number (Where you have a solar installation) Your Bank Details Copies of documents that can help us verify your identity and address (e.g. bank statement and passport) The details of your Generation meter (make, model, meter readings and a photo of the meter from the day you submit the application) If you have a separate Export meter, the details of this. The details in this FIT Application form are needed for ENGIE to set up your Feed in Tariff Plan and validate your application. The form is based on the information we are required to collect and provide to the regulator, Ofgem, who manages and oversees the FIT Scheme. Tips for completing the form This symbol indicates a question to help you decide if you need to fill in a particular section or not. Notes Most domestic installations are around 4kW in capacity. All solar and wind installations below 50kW in capacity are MCS Accredited and you should have an MCS Certificate provided by your installer which should have on it most of the details you will need. MCS Certification also covers all eligible micro-chp Installations. We need a meter reading from you on the day we receive your application form. This is to allow us to pay you correctly and for your application to be processed. We may ask you to take a photograph of your meter to assist in verifying meter readings. 3. SITE DETAILS Please provide the information below about the location of the installation. (e.g. home solar panels). The Supply MPAN means the Meter Point Administration Number. This can be found on your electricity bill. It may also be called your electricity supply number or supply number and it is a 23-digit reference. Here is an example of what an MPAN could look like. We just need the MPAN Core. This is the last 13 digits of your MPAN. INSTALLATION DETAILS Please fill in the details about the micro-generation installer and accreditation(s). A Microgeneration Certification Scheme (MSC) certificate is an internationally recognised quality assurance scheme that certifies both products and installation companies ensuring Microgeneration products are installed to a high standard. You need to provide an MCS certificate number as part of your application for FIT, to confirm that your system has been installed in line with the MCS installation standards. The MCS Certificate also tells you the type of generation installed, if it is stand alone, the Declared Net Capacity and the Total Installed Capacity - all required in table 4. Please note that if you are applying for FIT for the first time, and your MCS Certificate is dated before the 21st of January, 2016 you will not be eligible for FIT. ROO FIT accreditation is the accreditation scheme for all AD and Hydro installations below 5MW, and all solar and wind installations above 50kW, but below 5MW. You will need to provide your ROO-FIT Accreditation number here, and attach a copy of the confirmation letter or email from OFGEM. 5. GENERATION METER DETAILS Please complete the details for the meter associated with the micro-generation product. The information on the Generation Meter for can be found on your MCS certificate. If you are sending your application via post, or any non-instant method of application, we will ask for these when we receive the application.. However, if you have ROO-FIT Accreditation then the read used should match that already submitted to OFGEM. Where you are switching FiT supplier, we will ask for a meter read on the date the switch is confirmed. 6. EXPORTED ELECTRICITY Information around exporting electricity back to the national grid. If your installation is Off Grid then this section does not apply, please indicate this below then go to section 7. If you get electricity from the National Grid then you re on-grid. If you live in a property with no electricity (other than what you generate) you are likely off-grid and won t have a Supply Number. Is your application Off Grid? Yes No Installations below 30kW are not required to have an export meter. If your installation is below 30kW and you do not have an export meter, exported electricity will be deemed using the Ofgem guidelines. Please indicate this below then go to section 7. Installation is less than 30kW with no export meter? Yes No If you have an export meter there are two options: 1. A Power Purchase agreement (PPA) This is a contract to buy your metered export at a specific price. If you install a larger system and want a PPA Opt-Out and ask us about a suitable contract. 2. Export Tariff a government controlled price which you are paid for your export. If you want this Opt-in. If you have an export meter (mandatory for installations above 30kW in order to receive export payments) please select one of the following options: I wish to receive the Export Tariff (Opt-In) I do not wish to receive the Export Tariff (Opt-Out) No Export (select this option if you are Off Grid, do not have an export meter or you use everything you generate on site) Please note that you may only choose to opt in or out on an annual basis. Feed in Tariff (FiT) Declarations New installations and extensions The notes below are to help you complete this document. ENGIE must follow the FiT scheme rules determined by the market regulator, Ofgem, who require us to undertake certain checks and gather information and declarations from you. The rules state that all applications for new solar PV installations with an Eligibility Date on or after 1 April 2012, need to be accompanied by a copy of this document with the relevant section signed and dated. The Eligibility Date is normally the date on which you first applied to a FiT provider to join their FiT scheme and receive FiT payments. You will need to complete this document unless you have an existing FiT installation for which you successfully applied to get FiT Payments from a FiT provider before 1 April 2012. 1. If your application is for a new solar PV installation, you must sign two declarations; a. one declaration from the Energy Efficiency section and; b. one declaration from the Multi-Installation section. To do this, tick one box that applies to you in relation to the Energy Efficiency Declaration section on page 2 and one box in relation to the Multi-Installation Declarations Section. Please then sign the two relevant declarations (one of 1 to 4 and either 5 or 6). 2. If your application is for an extension to an existing PV installation, you must sign one declaration from the Energy Efficiency Declaration section only. Tick one of the boxes on page 2 in relation to the Energy Efficiency Declaration then go on to sign the relevant declaration 1 to 4. 3. The energy efficiency requirements applies to all installations except: a. Community or school installations, which may have relaxed requirements (Declaration 4); b. Stand-alone installation which may be exempt (Declaration 2). The requirement to sign the declarations will therefore apply to most normal installations on domestic premises. This means, you will normally sign: Declaration 1 or Declaration 3 depending on whether the EPC rating of D or above has been achieved, and; Declaration 6, unless you or whomever will receive the FiT Payments owns or receives payments for 25 or more FiT eligible Solar PV installations. The declarations may also ask you to enclose further information (for example a copy of your EPC Certificate) - please ensure you include this when you return the signed forms. Energy Efficiency Declaration Tick one of the following boxes in relation to the energy efficiency requirement and sign the relevant declaration overleaf: The energy efficiency requirement does apply and an Energy Performance Certificate (EPC) rating of level D or above has been achieved (complete Declaration 1) The energy efficiency requirement does not apply because my installation stand alone which means it not wired to provide electricity to a building (complete Declaration 2) The energy efficiency requirement does apply and an EPC rating of level D or above has not been achieved (complete Declaration 3) Community energy and school installations ONLY. The energy efficiency requirement does apply and an Energy Performance Certificate (EPC) rating of level G or above has been achieved (complete declaration 4) Multi-installation Declaration Tick one of the following boxes (unless your application is for an extension to an existing FiT installation) in relation to the multi-installation requirement and sign the relevant declaration overleaf: The FIT generator 31 or nominated recipient 32 owns or receives FIT payments from 25 or more other eligible solar PV installations (complete Declaration 5) The The FIT generator 31 or nominated recipient 32 does not own or receive FIT payments from 25 or more other eligible solar PV installations (complete Declaration 6) 31 FIT generator means in relation to a FIT installation, the person identified as the owner of that FiT Installation, whether or not that person is also operating or intending to operate the Eligible Installation. 32 nominated recipient means a person appointed by a FIT Generator to receive FIT Payments in respect of an accredited FIT Installation owned by that FIT Generator and recorded as such on the Central FIT Register. Energy Efficiency Declarations (sign one declaration only from Declarations 1-4) Declaration 1 I certify in respect of this application for accreditation that all of the following are applicable: a. the eligible PV installation is attached to or wired to provide electricity to one or more relevant buildings; b. a valid energy performance certificate is enclosed in respect of the building (or one of the buildings) to which the PV installation is attached or wired to provide electricity; c. the enclosed energy performance certificate is the most recently issued energy performance certificate in respect of that building; d. the enclosed energy performance certificate certifies that the relevant building to which it relates has been assessed as being within bands A-D; Signed Counter signed Dated Declaration 2 I certify in respect of this application for accreditation that the eligible PV installation is not attached or wired to provide electricity to any relevant building. I have enclosed evidence supporting this declaration that the eligible PV installation is not attached or wired to provide electricity to any relevant building. Declaration 3 I certify that declarations 1 and 2 above do not relate to my eligible solar PV installation. An EPC level D or above is required AND has not been achieved. I understand that this means I will receive the lower FIT generation tariff. Signed Dated Declaration 4 I certify in respect of this application for accreditation of a community energy/school installation on behalf of (name of community organisation or education provider) that all of the following are applicable: a. the eligible PV community energy/school installation is wired to provide electricity to one or more relevant buildings at the address below; Address of the building to which the installation is wired: b. a valid energy performance certificate is enclosed in respect of the building (or one of the buildings, which is not a dwelling) to which the PV installation is wired to provide electricity; c. the enclosed energy performance certificate is the most recently issued energy performance certificate in respect of that building; d. the enclosed energy performance certificate certifies that the relevant building to which it relates has been assessed as being level G or above; Signed Dated Version: 24.07.2017 ENGIE P a g e 9

Multi-installation Declarations Sign one declaration only from declarations 5-6, unless your application is for the accreditation of an extension to an existing PV installation in which case you do not need to sign either of these declarations) Where Nominated Recipient is mentioned, this means someone other than the FiT Generator (i.e. the owner of the installation) who will receive the FiT Payments. This name only needs to be completed where there is a nominated recipient, otherwise it can be left blank. Declaration 5 I ( the FIT Generator ) (and 33 I ( the Nominated Recipient *)) certify in respect of this application for accreditation that either the FIT Generator or the Nominated Recipient (if there is one) are, or have applied to be, the FIT Generator or Nominated Recipient for 25 or more other eligible PV installations located on different Sites. In this certification, references to the FIT Generator and Nominated Recipient include all persons who are connected persons 34 in relation to them.
