CD Catholic Health serves as a member of the Diabetes Coalition, responsible for diabetes risk assessment and coordinating walking program for Sisters Associates and local community. CD Associate/Employee Education Programs: uffalo public schools & colleges, Senior Centers, Appletree usiness Park, Food Pantries, Hospitals, Churches, Assisted Living/Retirement Centers, Employee Wellness lectures CD Associate/Employee Health Screenings: Employee Health Fairs, Health Fairs, lood Pressure CD Managed Care Diabetes Management Initiatives: 3 rd consecutive year of this initiative with Health Now & IHA. Patients identified with Obesity or at high risk for obesity are identified by pediatricians and referred into a 10 week group counseling program which is provided by CIPA at no expense to the. Tertiary Mercy Hospital and Sisters of Charity Hospital both provide a number of educational sessions focused on the needs of mothers and babies. Educational offerings include: PCAP, breast feeding initiative, prenatal teaching, natural contraception methods. Catholic Health participates in CHAMPS which addresses Nutrition, Exercise, Selfesteem, and disease prevention in 13 middle schools in Erie County. The focus is on smoking, cancer, stroke, cardiac, and diabetes. Program coordinator for WNY Osteoporosis Resource Center/NYSOPEP (NY State Osteoporosis Prevention and Education Program). NYSOPEP provides multi county bone health education emphasizing regular physical activity, nutrient-rich diets and bone-healthy lifestyle choices to promote lifelong skeletal health. Middle school bone health curriculum developed. Two support groups are maintained in Erie County and one in Chautauqua County. WNY NYSOPEP is a member of P2 Collaborative. Catholic Health is a program sponsor for Healthy ones Chautauqua, an evidence-based exercise program based on Tuft's University Strong Women Program. Catholic Health is a member of Living Healthy Task Force of Erie County, dedicated to bettering the health and wellness of all who live and work in Erie County. Catholic Health is a member of Worksite Wellness Committee for uffalo Public Schools. Catholic Health collaborates with numerous community and faith based groups, including The Wellness Institute and e Active NY, Partners For Prevention, Erie County Dept. of Senior Services, D'Youville College and many others.
