Assess on the following areas on a scale of 1-5. (1= this area needs to be rethought 5=this area is strong) Environments 1. Exterior of building (Was the landscape well kept? Is the building up to date?) 2. Outside (When you drove up to the parking lot, was there a warm, updated and inviting environment?) 3. Lobby (How did the lobby feel when you walked in? Was it warm and inviting? ) 4. Bathrooms (Were they clean? Enough of them?) 5. Signage (Bathroom, kids, auditorium, guest service) 1

6. Music (Was there music in the lobby?) 7. Colors of walls (Were they warm and inviting?) 8. Community (Was there a sense of community and relationships in the air?) 9. Dress of participants (Did the dress of the participants help you feel welcome and comfortable?) 6. Language of the leader (Did the leader use language that you didn t understand?) 7. Length of time (Was the length of the music time appropriate?) 8. Volume (Did the volume compliment of compete with the gathering?) 9. Other guests (Did you feel other guests would relate to the music?) 10. Help (Did the music leader help people to understand what was happening and what to do?) 5
