Patient lists for series completion Helping ensure patients receive all doses in a vaccine series When providers have patients who have been prescribed a vaccine with multiple doses in a series, they may encounter situations where patients may be delayed in receiving or have not received the subsequent doses in the series, due to varying reasons and, therefore, not complete their recommended dosing regimen. To help ensure patients receive all the required doses, it is paramount for providers to have the patient return for the remaining dose(s). Benefits of Patient List Reports for vaccine series completion Patient lists, called Patient List Reports in e-mds, are a useful tool to identify patients who have a gap-in-care according to clinical guidelines. They may help improve quality health care, such as by increasing vaccination rates. Patient Lists can also be helpful in identifying patients who have not completed all required doses in a vaccination series. Considerations: The number of patients appearing on a Patient List Report may be impacted by the clinical data available in the EHR; for example, if a vaccination was administered by another provider or in the pharmacy, it may not have been recorded in the EHR and patients will be included in the Patient List Report showing they need additional doses This Guide provides a highlevel overview of how to create Patient List Reports for vaccine series completion within e-mds. This overview is designed to provide guidance for you, your practice electronic health record (EHR) champion, or IT staff. The patient list criteria should consider active patients only (not deceased or inactive as determined by the practice) User privileges to configure and run Patient List Reports may be limited to select users with specific security privileges or subject to practice business policies If you have further technical questions, consult your internal or external EHR support resources. Please note that this Guide was created based upon e-mds version 8.0. Features and their locations may change as new software versions are released. Create a Patient List Report for series completion of an immunization The ability to generate a list of patients who are due, or overdue, for a test or procedure is a powerful preventive health care tool. Proactively identifying patients can have an impact to the quality of patient care, patient compliance, and improvement of patient outcomes. E-MDs provides multiple tools to track patient compliance, and one frequently used tracking method is to generate patient lists. Immunization series completion is a common use of these patient lists. In e-mds, it is possible to generate a Patient List Report to identify a list of patients who need additional immunizations to complete a series. Create a Patient List Report for an immunization using Clinical : Note that this method requires that Clinical have been set up for Product 1st and 2nd dose schedules. Detailed information about creating these clinical rules can be found in the document titled Creating an alert for GSK. Create a Patient List Report for an Immunization using An alternative method to run the report is to use in the Registry Processor. look for CPT codes that have been added to the patient chart as an indicator that the patient has had the vaccine. Patients with multiple doses will be listed twice. It may be necessary to print the list or export it to a spreadsheet for further analysis to determine those patients who require a second dose Select the File menu to Save As or Print the report.
