For the purpose of medical care or dental care obtained outside of California, this authorization is given with the intent that any consent given pursuant to this authorization shall be the consent of each of the undersigned. The undersigned understand and agree that YMCA shall not be legally or financially liable for any bill or medical expense incurred, or for any cause of action or claim arising from any medical care or dental care provided, or the lack of medical care or dental care. YMCA WINTER BASKETBALL PARENT CONTRACT I, to ensure the safety and integrity of the participants in the YMCA Youth Sports programs, hereby agree to abide by all the rules and guidelines established by the YMCA Youth Sports League. I will ensure that the members of my family and friends who may be participating in the YMCA programs will also abide by all rules and guidelines. I agree to the following: 1. I will praise the success of each player, regardless of which team the player is on. 2. I will offer genuine thanks to the referees and league directors volunteering their time to benefit our children. 3. I will support the decisions of my child s coach and ensure my child is on time for practice and games. I will also pick my child up promptly following the completion of practice. 4. I will support the YMCA and its programs, helping as much as possible when asked. 5. If I have a concern about someone else s behavior, I will contact the YMCA Gym Supervisor located on site or contact the YMCA Administrative offices at 626.432.5880. 6. I realize that we are guests in the gyms that we use. I will ensure all facilities care for according onsite gym use rules. This includes only bringing water into the gyms (i.e. not chips or Gatorade), removing any trash from the gyms, and keeping my children off of other school equipment. 7. I realize that we are guests in the neighborhoods that surround the gyms. I will do my best to respect the privacy of the people in the immediate area. This includes being quiet outside of the gyms and parking in school parking lots (as opposed to in front of people s houses) whenever possible. PASADENA YMCA Winter 2014 Jr. Lakers Basketball League Parent Information Sheet Parent Night There will be an informational meeting for all parents at the Coral Center on December 10 th from 6-7 PM. We will cover league rules, policies, and traditions. Where and when are the practices? Most practices will be held at the Coral Center on weeknights. The first practice will be held the week of January 6 th, 2014. Where and when are the games? Games will be held on Saturday s. Games are scheduled to be held at the Coral Center, South Pasadena Middle School, and Pasadena City College. Depending on age/division. What is required? The NEW Jr. Lakers League uniform. Attend Parents night. Attend the Clinic/Skill Assessment. Where and when is the Clinic/Skills Assessment? Skills assessment will be held on December 7 th, 3PM at the Coral Center. Due to the size of the league, we cannot honor special requests (i.e. practice days, coaches, others on the same team, etc ).
