8:50 9:10 am and Exhibits 9:10 10:00 am Prevention of ASCVD: New Recommendations for Hypertension Management and the Use of Aspirin Craig D. Williams, PharmD, BCPS, FNLA Prevention of heart attack and stroke remain key goals of primary care. Optimizing antiplatelet and hypertension therapies are two key pharmacologic strategies to reduce the risk of heart attack and stroke. Recent changes to hypertension guidelines in 2013 have created confusion in primary care and have already been questioned by a large, new study published in 2015. Recommendations on the use of aspirin for primary prevention were substantially changed by the United States Preventative Services Task Force (USPSTF) in 2016. This program will review the key new recommendations for both blood pressure control and optimal use of aspirin and some of the data underlying those new recommendations. ACPE Details Learning Objectives: Apply the new USPSTF commendations for aspirin use to their clinical practice. Discuss the implications of a major new clinical trial which has questioned our current blood pressure goals. Drug companies, pharmacy benefit managers and even pharmacies are being asked to justify the prices being charged to insurance companies and patients. What is behind the rising costs of drugs including dramatic increases in the price of some generic medications? What can be done to stem these rising costs? What policies will be put forth by leading Presidential candidates as part of their health platforms? These and other related topics will be discussed in this session on rising drugs costs. Implementation of 2015 HB2879 - Pharmacist Prescribing of Contraceptives Fiona Karbowicz, RPh This presentation will provide an in depth view of the new law that people all over the country are talking about! We will discuss the law s origination and the prescriptive authority for contraceptive drug therapy it grants to trained pharmacists. A focus will be on the policy and rules related to the law s implementation. ACPE Details Learning Objectives: Outline the historical background and discuss the specific elements of 2015 HB2879. Provide an overview of the policy and rulemaking process related to this law. Describe the Standard Procedures and rules developed by the board for the prescribing of contraceptives by pharmacists. Prescribing Hormonal Birth Control in Oregon Lorinda Anderson, PharmD, BCPS At the beginning of the year a new law allowing pharmacists to prescribe some forms of self-administered hormonal contraception went into effect making Oregon the first state in the country to expand women s access to these forms of effective birth control. This session will cover therapeutic basics of how pharmacists can apply the Board of Pharmacies algorithm and rules into practice, and how these correlate to the practice guidelines for Medical Eligibility Criteria (US MEC). ACPE Details Learning Objectives: Review new evidence and recommendations for women s contraception, why it is important, and how it applies to pharmacists prescribing. Discuss the risks associated with hormonal contraceptives, and how these apply to the categories of use set forth in the US Medical Eligibility Criteria (US MEC) guidelines. Apply the self-screening risk assessment questionnaire to any given patient in order to judge her eligibility for self-administered hormonal contraceptives. Cannabis: Smoldering Considerations in Chronic Pain Management Traci Hamer, PharmD, MAT This session will offer an overview of clinical considerations of marijuana use in patients living with chronic pain. Topics will include a discussion of evidence for efficacy for the treatment of chronic pain, mechanism of action, pharmacokinetics, and various safety considerations namely short- and long-term adverse effects and associated drug interactions. The presentation will also offer suggestions for how to talk to patients about this growing field of information what we can accurately say about safety and efficacy (and how we can say it). ACPE Details Learning Objectives: Review evidence for use of marijuana in the treatment of chronic pain. Explain how marijuana s mechanism of action leads to beneficial and/or adverse effects. Discuss the modifying factors that affect its pharmacokinetics and ultimate dose/effect. Fawzy Elbarbry, PhD, RPh An overview of the basic and clinical pharmacokinetics of vancomycin, followed by discussion of the several methods and guidelines utilized to dose and monitor vancomycin in normal and special populations. Real cases will be presented and attendees will be given time to apply the principles. ACPE Details Learning Objectives: Utilize the basic scientific knowledge about vancomycin to predict its PK behavior. Apply the PK principles to appropriately dose vancomycin for patients with a variety of disease states. Identify appropriate times during therapy to obtain vancomycin serum concentration. Lewis II, PharmD, FIDSA This presentation will discuss recent evidence surrounding early goal-directed therapy in patients with sepsis as well as the recent CMS core measures for sepsis. A case-based discussion of septic hospitalized patients involving different bacterial/fungal causes & disease states will be provided. In addition emphasis will be placed on recent guidelines and optimizing therapies for antibacterials and antifungals as well as their implementation in the hospital setting. ACPE Details Learning Objectives: Describe recent literature involving early goal directed therapy for sepsis. List key issues in the new CMS measures for sepsis. HIV in 2016: An Update for Pharmacists Geoff L Heureux, PharmD, AAHIVP This session introduces the most recent trends in HIV medicine that directly affect pharmacist practice. There will be a brief look at the lifecycle of the HIV virus so that we may understand drug targets and basic principles of antiretroviral therapy. We will examine and identify the DHHS Guideline Preferred Regimens for the treatment of HIV-1. Special attention will be given to recognition of the main class drug interactions and side effect of these preferred regimens. We will then turn our attention to Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP). Last, we will discuss the role of pharmacists in the retail, ambulatory, and in-patient settings in managing the patient living with HIV. ACPE Details Learning Objectives: Briefly describe the life cycle of the HIV virus and identify drug targets. Identify the basic structure of a DHHS Guidelines preferred regimen. Establish a basic familiarity with the drug interactions and side effects of classes of drugs in a DHHS preferred regimen. Identify the medications used for PrEP and PEP and establish a basic familiarity with who is a candidate for those protocols. Identify ways in which pharmacists may manage HIV patients in the retail, ambulatory care, and in-patient setting. 1 credit hour; HIV/AIDS therapy. General Seminar Information continued Gary Clausen Memorial 2-mile Run/Walk We challenge you to dig out your running shoes and head bands and come ready to run! This free event is for all OSHP Annual Seminar attendees and honors Gary Lee Clausen, a former OSHP President and avid runner who repeatedly won this annual event. Prizes will be awarded to the top male and female finishers (but don t let that keep you from running)! Come on and air out! Pre-registration is required. Cancellation Policy Cancellations on or before March 28, 2016 will receive a full refund less a 20% administrative fee. In the event of inadequate registration, weather problems, or other events beyond OSHP s control, the Annual Seminar may be moved or cancelled. Continuing Education Credit The OSHP Annual Seminar is intended for pharmacists, pharmacy students, pharmacy technicians, and industry representatives with an interest in health-system practice. Application has been made to the Oregon Board of Pharmacy for 16 total hours of continuing education, including 1 hour of law/ethics CE, and 2 hour of patient safety/medication error CE. Note: 13 hours is the maximum CE that any one attendee can receive based on the current programming. This program has been planned and implemented in accordance with the policies of the Accreditation Council on Pharmacy Education through the joint sponsorship of Oregon State University College of Pharmacy and Oregon Society of Health-System Pharmacists. Oregon State University College of Pharmacy is accredited by the Accreditation Council on Pharmacy Education as a provider of continuing pharmacy education. Program No.: UANs listed with respective sessions in schedule of events; Credits: 15 hours (1.5 CEU); Activity Type: Knowledge The Statement of Credit shall be issued only after successful completion of the program. The lobbyist supports legislation proposed by the Statewide Pharmacy Coalition, which protects the interest of pharmacy practice in Oregon. Examples of recent successful legislative activities include technician licensure and increased opportunity for pharmacists to provide immunizations.
