Request for Qualifications to Obtain Qualifications and Capabilities for Environmental Consulting Services The purpose of this Request for Qualifications is to obtain qualifications and capabilities from qualified individuals and firms interested in providing services for surveying/design/bidding for asbestos abatement, hazardous materials abatement, demolition bid packages, and other various environmental consulting services for Salida High School with Salida School District R-32-J. All questions should be made in writing and addressed to the Facilities Manager for the District: Mr. Walt Sneddon 905 D Street Salida, CO 81201 wsneddon@salida.k12.co.us 719-530-5449 Three (3) copies of a response to the Request for Qualifications shall be received no later than 1:00 p.m. March 21, 2011 at the above address. It is the responsibility of the submitting firm to ensure RFQ response packages are received prior to the deadline. RFQ response packages received after the above date and time will not be considered. Location of Construction Work: Salida High School 905 D Street Salida, CO 81201 Overall Project Description: BEST Project Salida School District R-32-J is in need of an Environmental Consultant in support of demolition of the old Salida High School, construction of a New High School, renovation of Kesner Building and associated utility tunnels with the Salida High School project site. Below is a description, schedule, and budget of the BEST Grant funded program. The project shall be designed and constructed through an integrated design process consistent with the Owner s requirements, all applicable Colorado Department of Education guidelines and standards, and high performance building standards with a desired goal to achieve Leadership in Energy and Environmental Design (LEED) Gold rating designation within the project budget and schedule parameters. The Governor s Energy Office has also been engaged and will be involved with the energy options and decisions. The Blythe Group has been retained as the Architect. The Team of Haselden / Diesslen has been retained as the CMGC. The Project went through Master Planning for the purpose of generating a scope of work and budget as a requirement in the B.E.S.T. grant application process. The project is

currently in Schematic Design with the SD report due April 2011. The scope items listed below are subject to change as the project design and budget are further developed. Overview of the Proposed 2010 BEST Grant Project The new high school will include twenty-two classrooms, auditorium, two gymnasiums and locker rooms. New data/phone technology systems, video communication, door access control, and security camera system will be included in this project. The current high school was built in 1922 with renovation/additions in 1958, 1962, and 1969. Davis Bacon wages and LEED Gold certification will be required for the Salida construction projects. The 1922 Kesner Building will remain intact and renovated to include the district administrative offices and additional classroom space for Horizon s Exploratory Academy. Colorado Historic Society will be included in the design and renovation of this facility. The new high school will be built on/near the current track site. Existing high school building will remain in place until new building is completed. District will move out of the old building and directly into the new facility summer of 2012. Abatement work that benefits the construction schedule will begin May June of 2011 primarily in utility tunnels and areas in Kesner and old high school buildings that would not require put back of new materials. The remainder abatement and hazardous material removal will occur prior to demolition in the summer of 2012. A geo-exchange system will be considered for this project. New track and soccer field will be constructed after old high school building is torn down. The Salida School District is looking to engage an Environmental Consulting firm to assist the project team in completing survey work as necessary to identify asbestos and hazardous materials, developing the abatement design, solutions and schedules that add benefit and value for the new high school construction and for the renovation of the Historic Kesner building with subsequent demolition of the old high school buildings. The entire project will be complete by October of 2012. Abatement Project Scope Description In order to ensure compliance with AHERA regulations for abatement and demolition, management plan review updates will be required with field verification for the content of building materials and to ensure the proper quantity of samples have been taken per homogeneous area and year of construction. Furthermore, the district will need updates completed to the existing management plans to reflect field conditions after constructions/demolition is complete. Preparation of Request for Qualifications documents for bidding to abatement and demolition contractors. Abatement documents will include specifications for removal, drawings detailing the limits of the work areas, and specific contract requirements for the performance of the abatement work.

Conduct mandatory abatement pre-bid conferences, generation of pre-bid conference meeting minutes; publish all necessary project addenda, and review evaluation of abatement contractor bids, including a recommendation for award. Attendance at project meetings as needed for the coordination and planning of work activities prior to the start of abatement, including assistance in processing of any regulatory variances required. This includes attendance at weekly progress meetings. On-sight staffing during abatement work for technical assistance to the abatement contractor, observation of compliance with project documents and regulations, post-abatement visual inspections of the work areas, project air and soil monitoring/clearances, daily air and soil monitoring and final air and soil clearance sample collection. Develop and track project schedules, negotiate changes in the scope of abatement work, and coordinate Owner interface and communication on the jobsite. Laboratory analysis of air clearance samples prior to the start of demolition. A written closeout report for the work conducted including all project documentation for oversight and air monitoring. These include daily logs, inspection forms, air monitoring field data, visual inspection forms, manifest tracking forms, and laboratory final reports. All project documentation from the abatement contractor will be included in the closeout report. The contractor information will include but is not limited to permits, daily logs, containment sign in sheets, OSHA monitoring, MSDS information, and waste manifests. The closeout report will be bound and clearly identified for the project. Project Schedule: Current Project & Design Schedule 1. Schematic Design, December, February 2011 2. General Conditions: Salida School District R-32-J reserves the right to reject any and all proposals or any part thereof, to waive or decline any formalities, informalities and any irregularities in any proposal received and to accept any portion of the proposal or all items if deemed in the best interest of the District. The selection or appointment of any firm under this Request for Qualifications is at the discretion of the Board of Education and is final without recourse. All firms submitting a proposal for consideration acknowledges the conditions stated herein. Salida School District R-32-J will not be liable for any costs incurred by bidders in the preparation of a proposal or for any work performed prior to the award of a contract. Proposals must include all information as asked for in this request. Failure to do so may result in rejection of the bid at the option of the Facilities Manager/Superintendent. The School District is exempt from City, County, State, and Federal Sales/Excise Taxes. Certification of tax exemption will be issued upon request. Proposers (and its employees, representatives and subcontractors) agree to abide by and comply with all applicable Federal, State and Local codes, laws, rules, and regulations in the performance of this contract. The firm, in its performance of this contract, shall comply with all of Board regulations, rules, and policies. The awarded firm shall also secure all necessary licenses, permits, and privileges required for the proper performance of the work, and shall perform all work in accordance with laws, ordinances, codes, etc. Successful proposer shall comply with the Patriot Act (Office of Foreign Assets Control), and be able to provide documentation that all employees, suppliers, and subcontractors are not on the SDN list. The awarded firm shall be held entirely responsible for any and all damage to adjacent property, accidents or injuries to employees and the public by reason of work included in these specifications or plans. The firm shall not allow any laborer or employee to wear objectionable clothing or caps or use profanity in any manner while on school property. Use of illegal substances or tobacco on school property is strictly prohibited. Contractor s Responsibilities: Inspection of the work or materials shall not relieve the contractor of any obligation to fulfill the contract as prescribed. Work and materials not meeting specifications shall be corrected at Contractor s expense and unsuitable work or materials may be rejected. Contractor will be responsible for providing a clean and safe environment surrounding the work area at all times including, if necessary, fencing of equipment, storage, and work area. Contractor shall furnish all required personal safety equipment.

Contractor shall perform work in accordance with good trade practice and in a neat manner to the satisfaction of the District and its Owner s Representative. Qualifications Section: In order to select the firm best qualified for the Salida School District project, we are asking that the prospective Firm respond to the following questions: Work Experience List all relevant work experience (abatement and demo) and qualifications related to the proposed project. Provide references of similar abatement and demolition projects completed. Provide a minimum of six references for recently completed work to include company name, individual to contact and telephone number. Provide a copy of an example survey and design similar in scope to this abatement/demolition. Personnel Provide a list of key personnel, their skills and qualifications, certifications, technical competence, experience on similar projects, capacity to undertake the project, and their availability throughout the duration of the project. Insurance Certificate The selected firm will be required to indemnify and hold Salida School District and its agents and employees harmless from and against all suits or actions of any kind, including, but not limited to, workers compensation claims brought against them for or on accounts of any damages or injuries received or sustained by any parties, by or from the acts of the successful firm or its agents or employees. The selected firm shall indemnify and hold the Salida School District and its agents and employees harmless from any and all claims, causes of action, suits and liabilities, including attorneys fees and costs arising out of or relating to disclosure of any information included in the successful firm s proposal. Prior to the commencement of any work, Firm shall forward Certificates of Insurance to Salida School District. The insurance required shall be procured and maintained by the Firm for the duration of the contract. The insurance shall be with a carrier licensed in the state of Colorado and shall have A or better Best rating. Please attach a proposal copy insurance certificate that lists limits. Firm shall name Salida School District as additional insured on all insurance except Workers' Compensation. The Firm shall furnish Salida School District. Billing Rates Provide company billing rate sheets for 2011 and 2012.

Cost Proposal In addition to providing billing rates, provide an estimate of man-hours and reimbursable expenses to update management plans for Kesner building and Salida High School. Also include an estimate for the number of samples that will need to be taken at each facility. Evaluation Criteria: Award of Contract After evaluation of the Request for Qualifications, the District may request oral presentations. The Firm will be selected based on work experience, experience of proposed personnel, how the firm ensures competitive bidding, and billing rates. The District will choose the most qualified and most capable firm to perform the work as outlined above. The undersigned hereby affirms that: He/she is a duly authorized agent of the propose; He/she has read the RFQ terms and conditions and any technical specifications, which were made available, and fully understands and accepts those terms unless specific variations have been expressly listed and identified with the firm s response to this RFQ.
