1. Welcome Hillsborough County Health and Social Services would like to welcome you as a new member of Hillsborough HealthCare. The information contained in this Member Guide provides information on the health care services, networks and clinics available to you through Hillsborough HealthCare. The guide also describes how you should choose your clinic and the rules you should follow when using the clinic. It also provides information on where you can go to find out more information about Hillsborough HealthCare and describes other services that may be available to you depending upon your situation. 2. Who Qualifies? All Hillsborough County residents may apply for Hillsborough HealthCare. Assistance is available for clients who qualify. Social Services personnel determine who qualifies for Hillsborough HealthCare. To qualify, you must: Be a Legal United States resident. Be a Hillsborough County resident. Have Income at or below 100% of poverty level. Have Assets within established departmental guidelines. Not be eligible for any other coverage through another state, federal or private insurance program, except as specified in Plan Descriptions. Not be convicted of three (3) separate felony offenses on three (3) different days after April 6, 2005 in Hillsborough County. This is called the 3 strike policy. 3. How Do I Enroll? In order to qualify as a member of Hillsborough HealthCare, you must first enroll in the program by completing an on-line enrollment application at: http://www.hillsboroughcounty.org/hss/hhcprogram/program.cfm. You can also enroll at any Hillsborough HealthCare Network location or you can call Health & Social Services at (813) 272-5040 for more information. 4. Membership Cards Once you are enrolled you will receive a Card that shows you qualify for Hillsborough HealthCare and that you are a Member. The Card will look like the one below: 3

Reasons for Disenrollment You may be disenrolled from Hillsborough HealthCare for any of the following reasons: You are no longer a resident of Hillsborough County. You become eligible for coverage by other local, state, or federal program. You become 65 years of age and eligible for Medicare. If you qualify for Services provided by Medicare, you are no longer eligible for payment by Hillsborough HealthCare. You become employed and you are enrolled for health care coverage through your employer. Your income exceeds the maximum poverty limit guidelines. You fail to comply with or no longer meet eligibility requirements. You violate the county s three (3) strike policy. You have not followed the programs you were asked to participate in. Your behavior is inappropriate 6. How do I Re-enroll? In order to maintain your Membership in the Hillsborough HealthCare, you must periodically show that you still qualify for the program and you must re-enroll in the program by providing additional information requested by your case manager. At the time of re-enrollment, you will generally receive a letter from Health & Social Services requesting additional information. You can provide the information by mail or you can re-enroll at any Hillsborough County Health and Social Services Neighborhood Service Center location. You can also call Health and Social Services at (813) 272-5555 for general help. 4

If you allow your eligibility to end and wish to re-enroll in Hillsborough HealthCare, you should contact your case manager to begin the re-enrollment process. 7. How Do I Pick a Clinic and a Primary Care Physician (PCP)? Once you qualify for Hillsborough HealthCare, you will select a clinic within the Hillsborough HealthCare network. You can choose whichever clinic is most convenient for you. It will be at this clinic, which serves as you medical home, that you will receive your health care services. A Primary Care Physician (PCP) will be assigned to you. Your Primary Care Physician will coordinate and assist you with all your medical care. 8. How Do I Change My Clinic or Primary Care Physician (PCP)? You can change your PCP, Network and Clinic if you have a good reason, such as relocating to another part of Hillsborough County. It must be approved by Hillsborough HealthCare Staff. It is their decision whether to approve your request or not. If approved and changed by Hillsborough HealthCare Staff, the change becomes effective the first day of the following month. You will need to request and sign a release of medical information to have your medical records forwarded to your new Clinic and Primary Care Physician. 9. What Kinds of Health Care is Covered? A. Hospital Care received at a Hospital This is when you are admitted to a hospital. The hospital care you receive while you are in the hospital is covered by Hillsborough HealthCare. The only exception is psychiatric hospital care, which is not covered. For scheduled admissions arranged by the specialist, you must have a referral from the Primary Care Physician (PCP). 5

B. Specialty Physician Care Specialty Physician Care, for example, when your doctor wants you to see a Cardiologist, it is covered by Hillsborough HealthCare, but you must have a referral from your assigned Primary Care Physician (PCP). C. Outpatient Clinic Visits These are Outpatient Clinic Visits and are generally covered by Hillsborough HealthCare. These may include your doctor visits for: Preventive care and routine checkups to help keep you well. Treatment when you are sick. Doctor ordered laboratory, x-ray and other routine diagnostic tests. Doctor ordered medical equipment like wheelchairs, supplies, home health care and oxygen services. To arrange for you to see a specialist when necessary. Arrange for you to receive outpatient therapies such as physical/occupational/speech therapies, wound care, cardiac therapy, and pulmonary therapy. D. Laboratory and X-ray Services Laboratory and x-ray services ordered by your Doctor as part of your outpatient care are covered by the Hillsborough HealthCare. You must use the Laboratory and x-ray facilities assigned to your network. E. Dental Services Only emergency care necessary to pull a tooth is covered. No referral is required. You can Walk-in since no appointment is required. A referral by a Dentist at one the Hillsborough HealthCare designated dental clinics is required to be seen by the oral surgeon. 6

F. Medications Most Medications are covered. You can obtain your medications from all contracted Pharmacies in Hillsborough County. Check your Hillsborough Member HealthCare card for your co-pay. G. Physical Therapy, Speech Therapy, and Occupational Therapy Therapy Services ordered by your Doctor as part of your outpatient clinic visits are covered by Hillsborough HealthCare. Your Doctor must refer you to the appropriate facility. H. Medical Equipment and Medical Supplies Medical equipment such as a wheelchair or crutches, and oxygen or other medical supplies ordered by your doctor as part of your outpatient clinic visit are covered by Hillsborough HealthCare. I. Orthotics and Prosthetics Specific medical support such as special shoes, artificial legs and arms ordered by your doctor as part of your outpatient visit are covered by Hillsborough HealthCare. Once you are approved for this special support, you will be contacted for a fitting. J. Home Health Care Your Primary Care Physician or Specialist may feel you need some assistance while you are in the home after you have been in a hospital. If 7

you need this additional help, a nurse will call you prior to the first home visit to let you know when someone will be coming out to see you. K. Skilled Nursing/Rehabilitation Services Your Primary Care Physician or Specialist may feel you need some assistance after you have been in a hospital before you can return home and may place you in a skilled nursing facility or rehabilitation facility. Hillsborough HealthCare will only cover up to 45 days from the effective date you are admitted to the facility. L. Emergency Medical Care Emergency Medical Care is covered by the Hillsborough HealthCare when utilized for real emergency situations. You should always let your Primary Care Physician (PCP) know when you have gone to an Emergency Room. 10. When Do I need a Referral? You must have a referral from your assigned Primary Care Physician (PCP) before you can be seen by a specialist such as a Cardiologist. If you do not have a referral, Hillsborough HealthCare will not pay for your visit. So it is important that you always go to your assigned Primary Care Physician (PCP) for any of your health care needs. 11. Where are the HealthCare Networks & Clinics?
