Computer Provider Order Entry (CPOE) Use computerized provider order entry (CPOE) for medication orders directly entered by any licensed healthcare professional who can enter orders into the medical record per state, local and professional guidelines. (p.44567) The number of unique patients with at least one medication in their medication list seen by the EP during the EHR reporting period (p.44334) The number of patients in the denominator that have at least one medication order entered using CPOE (p. 44334) Threshold: Over 30% CPOE (by licensed professional) for at least 1 medication, and 1 lab or radiology order for 60% of unique patients who have at least 1 such order (order does not have to be transmitted electronically)

Drug-Drug and Drug Allergy Checks Implement drug-drug and drug allergy interaction checks (p.44567) Key Information: Capability of conducting automated drug-drug, drug-allergy checks is included in the certification criteria for certified EHR technology. This automated check provides information to advise the EP s decisions in prescribing drugs to a patient. (p.44335) Measure: The EP has enabled this functionality for the entire EHR reporting period. (p.44567) Employ drug-drug interaction checking and drug allergy checking on appropriate evidence-based interactions

Patient Demographic Data Record all of the following demographics: Preferred Language,Gender, Race, Ethnicity, Date of birth. Report on Clinical Quality Measures Report ambulatory clinical quality measures to CMS or, in the case of Medicaid EPs, the States. (p.44567) Measure: Successfully report to CMS ambulatory clinical quality measures selected by CMS in the manner specified by CMS (p.44567) For 2011, provide aggregate numerator and denominator through attestation as discussed in section II (A)(3) of the final rule. For 2012, electronically submit the measures as discussed in section II (A)(3) of the final rule (p. 44567) Continue as per Quality Measures Workgroup and CMS

Clinical Decision Support Tools Implement one clinical decision support rule relevant to specialty or high clinical priority along with the ability to track compliance with that rule. (p.44567) What does Clinical Decision Support Tool entail? HIT functionality that builds upon the foundation of an EHR to provide person involved in care processes with general and person specific information, intelligently filtered and organized, at appropriate times, to enhance health and health care. These tools could include, but are not limited to, evidence based medicine templates, decision trees, reminders, linked online resources, scientific evidence and consensus (p.44350) Use CDS to improve performance on highpriority health conditions. Establish CDS attributes for purposes of certification: 1. Authenticated (source cited); 2. Credible, evidence-based; 3. Patient-context sensitive; 4. Invokes relevant knowledge; 5. Timely; 6. Efficient workflow; 7. Integrated with EHR; 8. Presented to the appropriate party who can take action

Patient Copies of Health Information, upon Request Provide patients with an electronic copy of their health information (including diagnostics test results, problem list, medication list, medication allergies, upon request. (p.44567) The number of patients of the EP who request an electronic copy of their electronic health information three business days prior to the end of the EHR reporting period (p.44355) The number of patients in the denominator who receive an electronic copy of their electronic health information within three business days (p.44355) Threshold: Over 50% Continue Stage 1

Provide Clinical Summaries Provide clinical summaries for patients for each office (p.44567) The number of unique patients seen by the EP for an office visit during the EHR reporting period (p.44359) The number of patients in the denominator who are provided a clinical summary of their visit within three business days (p.44359) Threshold: Over 50% Patients have the ability to view and download relevant information about a clinical encounter within 24 hours of the encounter. Follow-up tests that are linked to encounter orders but not ready during the encounter should be included in future summaries of that encounter, within 4 days of becoming available. Data are available in humanreadable and structured forms (HITSC to define)

Information Exchange Capability to exchange key clinical information (for example, problem list, medication list, medication allergies, and diagnostic test results), among providers of care and patient authorized entities electronically (p.44568) Threshold: Performed at least one test of certified EHR technology s capacity to electronically exchange key clinical information (p.44568) Connect to at least three external providers in primary referral network (but outside delivery system that uses the same EHR) or establish an ongoing bidirectional connection to at least one health information exchange

Drug Formulary Checks MENU Implement drug formulary checks (p.44568) Measure: The EP has enabled this functionality and has access to at least one internal or external drug formulary for the entire EHR reporting period (p 44568) Make core requirement.

Clinical Lab Test Results Incorporate clinical lab-test results into EHR as structured data (p.44568) Number of lab tests ordered during the EHR reporting period by the by the EP whose results are expressed in a positive or negative affirmation or as a number. (p.44346) Number of lab test results whose results are expressed in a positive or negative affirmation or as a number which are incorporated as structured data. Medication Reconciliation The EP who receives a patient from another setting of care or provider of care or believes an encounter is relevant should perform medication reconciliation (p. 44568) The number of transitions of care during the EHR reporting period for which the EP was the receiving party of the transition. Provide Timely Access Provide patients with timely electronics access to their health information (including laboratory results, problem list, medication lists, and medication allergies) within four business days of the information being available to the EP (p.44568) The number of unique patients seen by the EP during the EHR reporting period (44358) MENU The number of patients in the denominator who have timely (available to the patient within four business days of being updated in the certified EHR technology) electronic access to their health information online (p.44358) Threshold: Over 10% Patients have the ability to view and download (on demand) relevant information contained in the longitudinal record, which has been updated within 4 days of the information being available to the practice. Patient should be able to filter or organize information by date, encounter, etc. Data are available in humanreadable and structured forms (HITSC to define).

MENU Summary Care Record The EP who transitions their patient to another setting of care or provider of care or refers their patient to another provider of care should provide summary care record for each transition of care or referral (p.44568) Number of transitions of care and referrals during the EHR reporting period for which the EP was the transferring or referring provider (p.44364) The number of transitions of care and referrals in the denominator where a summary of care record was provided (p.44364) Threshold: Over 50% Move to core

Immunization Registries Capability to submit electronic data to immunization registries or immunization information systems and actual submission according to applicable law and practice (p.44568) MENU Measure: Performed at least one test of certified EHR technology s capacity to submit electronic data to immunization registries and follow up submission if the test is successful (unless none of the immunizations registries to which the EP submits such information has the capacity to receive the information electronically).
