Rules and Requirements 1. ASQ is the program owner and copyright holder of the ITEA Criteria, ITEA judges training materials, and all documents and templates related to basic ITEA fundamentals. ASQ does not authorize anyone to use the ITEA Criteria or any intellectual property held by ASQ for any purposes beyond those required to meet and support the ASQ ITEA preliminary- and final-round judging process. This includes providing ANY paid or unpaid coaching or consulting services (now or in the future) around ASQ s International Team Excellence Award Criteria, Award or Training Programs, or any other related ITEA material unless such services are provided through a formal written contract with ASQ s Learning Institute. Anyone found in violation of this policy will be disqualified. 9
Rules and Requirements 2. Projects must either be active or have been completed within the last 2 years. Please note that active projects must address all parts of the ITEA Criteria. Preliminary Round Applicants are required to include: -Measures/Metrics/Stakeholders/Tools -Primary goals at beginning of project -Results for those goals at the end of the project -Project time frame (start and official end month and year) 3. Projects submitted previously are not eligible for resubmission. 10
Rules and Requirements 4. Voice Over PowerPoint file(s) have to be aligned to the ASQ Award Criteria. File size should not exceed 50 MB, and should include slide numbers and criteria item numbers. Presentation should not exceed 45 minutes. 5. Preliminary Round and Final Round Presentations MUST have the same metrics, measures, goals, results, etc. Modifications are limited to presentation s alignment to the award criteria. Presentations that don t confirm to requirements will be disqualified. 6. Entries will not processed without payment. Entry fees for Preliminary Round must be received by September 8, 2017. 11
Summary of Preliminary Round Entry Process 1 Identify project(s) for submission Identify projects completed in the last two years that achieved measurable results. Projects submitted previously are not eligible for resubmission. Complete the Intent to Submit Form. 2 Select project(s) for submission. You may submit up to five entries from any participating site or location. This award process is open to teams from all over the world. 3 Create a Voiceover PowerPoint presentation that describes your completed project and ensure that it is aligned with ASQ's Team Excellence Award Criteria. PowerPoint file size should not exceed 50 MB, and should include slide numbers and criteria item numbers. Presentation should not exceed 45 minutes. 4 Complete the online entry form and submit payment (Regular Rate: $699 US Dollars per team; Public Education Rate: $199 US Dollars per team). A separate entry form should be submitted for each team. DEADLINE: September 8, 2017. 12

Before pressing submit button, print the completed form for reference. Payment. Regular Team Entry Fee is $699 US Dollars per team; Public Education Team Entry Fee is $199 US Dollars. Entries will not be processed until payment is received. The process for payment will vary depending on the method you select in the online form. - Paying by Credit Card. You have the option to pay for multiple team award submissions with one transaction. Update the quantity box on the shopping cart page to reflect the number of team applications submitting prior to clicking the checkout button. - Paying by check. Mail check along with a printout of this completed Team Entry form to ASQ HQ. - Payment by International Wire Transfer to ASQ Transfer Voiceover PowerPoint files, Team photo, and your organization's logo to ASQ. You do not need to have a Egnyte account to transfer files to ASQ. You will receive an email confirmation that your files have been received. 13
Final Round Entry Process December 2017 Qualified teams (from the preliminary round entry process ) are notified of selection for the final round. Teams confirm their participation to present live at ASQ s World Conference. January 2018 Teams receive scoresheet summary from ASQ. March May 2018 Teams upload final round presentation materials. Teams complete online registration form. **Rates per team presenter: $699 Early Bird Rate by March 2018; $799 Regular Rate after March 2018. **Rates subject to change.
