Services offered are residential care, transitional support and supported living. The company is registered with the Care Quality Commission and complies with all care regulations. To provide a safe and quality service within the local area. This will involve delivering direct support to individuals who have learning disabilities, mental health issues and profound complex needs in a way that enables them to maximise their independence, maintain family relationships and access community opportunities. To represent the organisation in a positive way through when interacting with external partners including purchasers, service users, carers, families and communities and to contribute to Area and Organisational objectives. To be accountable for your practice and development of your skills and knowledge. The role will be based in the Essex area, the role will need to reflect the provision of 24/7 care as necessary and will include working over a 24/7 period. Main duties & responsibilities: Service User Support To ensure that the rights of the service user are upheld and promoted at all times in accordance with the principles of choice, respect, dignity and privacy. To ensure that all issues relating to the safeguarding of vulnerable adults are reported according to policies and procedures and in accordance with the Page 1 of 6
safeguarding policy. To ensure that if there are significant changes to individual service users needs these are raised with your service co-ordinator. To ensure that all service user records are maintained in accordance with policy and procedure. To ensure that all support is delivered in accordance with risk assessments, care plans and behaviour management plans that are in place and to contribute to the regular review of such documents as appropriate. To take the lead and support less experienced staff to work with service users. To bring to the attention of the Team Leader, Service Co-coordinator or Supported Living Manager any concerns or deficits in the provision of care. To strive to ensure service user satisfaction and to report complaints and concerns in accordance with policy and procedure. To promote service user involvement in all areas including service delivery, quality assurance and organisational development. To support the effective management of service user finances ensuring that all transactions involving service user finances are recorded, verified and audited in accordance with policies and procedures. To support service users to maintain their health and well being, this will include supporting them to attend appointments, manage their personal care and medication according to need. To support each individual to maintain a safe and secure environment in accordance with operational policies. Provide housing related support as required. To support service users with their choice of holidays. Administration Of Medicines Attend medication administration training and then be observed/supervised administering until deemed competent in accordance with TLC s Medicine Policy. Record keeping To contribute to the development of the support plans based on the assessed needs of the identified service users. To assist with written progress reports as directed by your line manager. Page 2 of 6
Read support plans, communication book and diary on a daily basis and contribute information as necessary. Organisational and Administrative To ensure that all actions and work undertaken is compliant with regulatory and registration requirements under the Care Quality Commission and Mental Capacity Act (2005). To attend training, supervision and other personal and professional development opportunities as directed. Responsible for complying with Health and Safety regulations and to have regard for the health, safety and welfare of self and other at all times. Responsible for reporting to the Team Leader, Service Co-ordinator, Supported Living Manager and HR manager any issues, incidents or concerns in respect of Health and Safety. To promote a positive and professional image of TLC Care Homes. when representing the organisation at a range of meetings and forums. To be pro active in implementing the companies policy on environmental issues. To provide information and reports as requested. Knowledge Can demonstrate a knowledge of; Previous experience of record writing Community Care Act National Care Standards Health and Safety Person centred Planning Adult Protection AF,I, QC and T Challenging Behaviour AF, QC and I Mental Capacity AF, QC and I Qualifications/ Education/ Training LDQ Award / NVQ level 2/Diploma Level 2 in Health and Social Care or a willingness to work towards. AF, I and QC Other requirements You will also be required to follow a shift pattern, but still work flexibly in order to meet the needs of the services within TLC Care Homes Ltd.
