During the year 2012 2013 ( for 01/07/2012 to 30/06/2013) Sealed Offers under two envelope system are invited from eligible contractor for providing Annual Repair and Maintenance Services listed under scope of work in this bid document. Details of the project for which tenders are invited are as follows:- Nature of Facility Hospital / Dispensary / Offices / Residential / Others. Scope of Services to be provided :- The agency will be responsible for Repair and Maintenance of following : Internal Electrical works. Transformer sub station D.G. set Pump set Lift 2 x 20 TR Central AC Plant Repair and day to day maintenance of Generator, pumps, lift, boiler, ROP. Water filters, ceiling fan, wall fan, Desert Cooler, Water cooler Refrigerator, L.T. Switch gear central Panels, Internal wiring, geyzers, Fly killite, glow sign board, defridger, exhaust fan, etc. Air conditioning units consisting of split ACs & Window A.C. Street light L.T. line overhead and under ground cables etc.

INSTRUCTIONS TO THE TENDERERS 1. Sealed offers under two bids system in the prescribed forms are invited from eligible Tenderers for annual repairs and maintenance of civil work / Annual repair maintenance and running operation of electrical services listed under scope of services in this bid document. 2. The amount of earnest money deposit (EMD) shall be Rs. 109000/- for civil work and 127400 for electrical work in the form of DD/Pay Order, drawn in favour of ESI FUND A/c No. 1 Model Hospital Indore (M.P.) 3. Tender form shall be complete in all respect, Incomplete tender or tenders without E.M.D shall be treated as invalid. 4. Last date of submission of tender is 26/6/2012 Upto 14.00 Hrs. 5. Date and time of opening of tender is 26/6/2012 at 14.30 Hrs. 6. Each and every page of the tender documents should bear the stamp and signature of the authorized representative. Format I & II enclosed shall be filled without exception. The tenderer shall also enclose the latest Income Tax Clearance Certificate and proof of VAT/works Contract Tax Registration where applicable. 7. The rates for each and every item shall be quoted in Figure and words. In case of any discrepancy in rates, the rates written in words shall prevail. 8. The envelop containing tender document shall be sealed and bear the name of work and the name and address of the tenderer. 9. The site for the work is available and can be seen on any working day during office hours by contacting The JE (E), ESIC M.H, Indore. 10. The Competent Authority of ESIC, reserves the right to accept or reject any tender or all tenders without assigning any reason. 11. Conditional tender are liable to be rejected. 12. The tender for the work shall remain open for acceptance for a period of sixty days (60) from the date of opening of tender. 13. These instructions shall form a part of the contract document. 14. The EMD of unsuccessful Tenderers shall be refunded within one month after the award of work to the successful tenderer.

15. Rates quoted shall be firm and fixed and are inclusive of cost of manpower, material, machinery, tools and plant etc. all taxes (including service tax.) duties and levies, insurance etc. no escalation of whatsoever nature shall be payable. 16. (ESIC TO CHOOSE ALTERNATIVE APPLICABLE) A. Alternative I : (for works value over Rs. 30 Lacs) I. The tenderer should have minimum three years experience as on 26/06/2012 in works of similar nature II. The tenderers should have successfully completed the following work in the last 7 years : (i) One similar work of value not less than rupees 4350400/-.for civil work and Rs. 5093754/- for electrical work (Equals to 80% the value of work to be executed) (ii) Two similar work of value not less than rupees 2719000/-for civil work and Rs. 3183597/- for electrical work. (Equals to 50% of the value of work to be executed) (iii) Three similar works of value not less than 2175200/- for civil work and Rs. 2546877/- for electrical work. (Equals to 40% of the value of work to be executed) III. The tenderer should have minimum inhouse manpower to cover requirements of Formats I & II. IV. Average Annual financial turnover during the last 3 financial years should be at least 30% of the value of work to be executed (copies of balance sheets signed by CA to be submitted). B. Alternative II (for works value below Rs. 30 lacs). III The tenderer should have minimum inhouse manpower to cover requirements of Formats I & II. 17. Submission of bids: proposals should be submitted in two separate envelopes namely, Technical Bid (Part A) and Financial Bid (Part B). The technical and financial bids duly sealed in separate envelopes must be kept inside a single third envelope and clearly superscribed Annual Repair maintenance for civil work and electrical works both separately as per Annexure X the envelope should be clearly marked as Technical Bid (Envelop-I) and Financial Bid (Envelope-II). This third envelope shall also contain the EMD. 18. Evaluation of Technical Bids : Bids received and found valid will be evaluated by the ESIC to ascertain the best-evaluated bid for the complete work/services under the specifications and documents. The tenderer should take care to submit all the information sought by the ESIC in prescribed formats.

a. Firm s relevant experience and strength Profile of agency, registration details, experience of similar works, annual turnover, total manpower employed. b. Qualification/Related experience. 19. Financial Bid : The financial bid of the tenderers, whose technical bid is found to be suitable, will be opened in the presence of the tenderers, who choose to attend the opening of financial bid. Minimum two day notice will be given to tenderer for this purpose. 20. Award of work : 1. The selection of the agency will be at the sole discretion of the ESIC who reserves its right to accept or reject any or all the proposals without assigning any reason. 2. The contract for S.I.T.C. of Additional 125 kva D.G. set, and Additional 500 kva Transformer s/s shall be awarded to the qualified responsive tenderer who has quoted lowest. 3. Upon evaluation of offers the notification on award of contract will be intimated to the successful tenderer. 21. Duly filled tender documents shall be deposited by hand delivery/dispatch by post in the office of ESIC at the following address: Medical Superintendent ESIC,Model Hospital Nanda Nagar, Indore (M.P.) 22. Delay/ Late tenders for whatever be the reason will summarily be rejected. 23. Any item not mentioned in the schedule of quantity but felt necessary for civil works shall be carried out at CPWD, Delhi Schedule of rate 2007 ( with addition of prevailing cost index ) In case some items are not available in DSR 2007, The same shall be paid on market rate. 24. Tender documents shall be issued only to the eligible contractor after their securitization of their certificate from previous clients, which they should bring along while coming to purchase the tender documents. They should also bring original and attested copy of registration, work experience, Pan Card and income Tax return EPF, ESI Registration and Service Tax etc. The Medical Superintendent reserves the right to refuse issue of tender document to any contractor without assigning any reason. 25. The rates quoted shall be valid upto Completion of Awarded work. 26. The detailed terms and conditions shall also be applicable as per enclosed annexure. 27. Contractors must ensure they are complying all statutory liability and labour law i.e. ESI, EPF and Service tax etc.

28. In case any working day declared or happened to be holiday on that day the receipt issue and opening of tenders will be done on the next working day. 29. The Medical Superintendent reserves the right to add/amend/modify/withdraw any of the tenders and conditions of the tender documents at any time without any notice and the decision of the Medical Superintendent in this regard will be final. 30. Interested Contractors should collect the tender form containing detailed terms and conditions in person from office of the Medical Superintendent Model Hospital ESIC, Nanda Nagar, Indore (M.P) on any working day between 09.00 AM to 3.00 PM Upto 23/6/2012. 31. The enlistment of the contractor should be valid on the last date of sale of tender. In case only the last date of sale of tender is extended, the enlistment of contractor should be valid on the original date of sale of tender. 32.
