The cost of tender form IS Rs.1000/- and it should be paid by a demand draft in favour of ESI Fund Account No.1 payable at Manesar. Sealed Tenders duly filled in along with all required documents as stated in tender documents should be dropped in Tender Box kept at Administrative Wing (Fourth Floor) of the Hospital up to 01:00 PM sharp 24.03.2014. Technical bids shall be opened on 24/03/2014 at 03:00 PM in the presence of the Tenderers who may wish to be present. In case 24/03/2014 is declared a Public Holiday, Tenders shall be received and opened as per above schedule on next working day..along with tender EMD of amount Rs. 2 Lakhs is to be submitted in the form of DD in favour of ESI Fund A/c No. 1 payable at Manesar. INSTRUCTIONS TO BIDDERS GENERAL 1. The tender is being invited for Man power supply (Nursing & Para Medical). The contractor shall provide man power strictly as per the guidelines laid down in the tender documents. The man power supplied would be engaged at ESIC Hospital, Manesar. The man power supplied would have to perform rotational duties including night shifts 2 ELIGIBLE BIDDERS 2.1. The agency must have experience of minimum five years (as 31.03.2013) for supply of manpower, out of which two years experience should be in the field of supply of Nursing and Para-medical manpower, Certificate of satisfactory performance from such clients is also to be submitted. 2.2 Annual turnover of the agency should not be less than Rs. 1 crore during the last three years i.e. from 01.04.2010 to 31.03.2013. This turnover should be from manpower supply services only. 2.3. The agency should have undertaken any one of the following work during the last three years. One work of manpower supply not less than 60% of the work value to be executed i.e 60 Lakhs Two works of manpower supply not less than 50% of the work value to be executed 50 Lakh Three works of manpower supply not less than 40% of the work value to be executed i.e 40 Lakhs 2.4. The tenderer is to submit Satisfactory Work Completion Certificate in support of their contention of having completed anyone of the aforesaid works failing which the information furnished regarding the completion of work is liable to be treated as invalid. 3. Following documents should be submitted with technical bid (self-attested) along with the tender a) Income tax returns f o r the last 3 years along with Income Tax c l e a r a n c e Certificate i.e 01/04/2010 to 31/03/2013 b) Audited Balance Sheets of last three financial years i.e. 2010-2011, 2011-2012 & 2012-2013. Audited Income and Expenditure statement of last three financial years i.e. 2010-2011, 2011-2012 & 2012-2013 c) Audited Profit and Loss Account of last three financial years i.e. 2010-2011, 2011-2012 & 2012-2013. d) Service Tax registration certificate/vat registration.

e) Registration Certificate of Firm/Agency/Organization. f) ESI and EPF Registration. g) VAT Registration i) Contract Labour (R&A) Act, 1970 registration certificate j) The Agency has to ensure for payment of minimum wages applicable to the respective post in the state of Haryana as per existing rules applicable. The offer of all agencies who qualify in the technical bid will be evaluated on the basis of service charges quoted in financial bid. In addition, antecedents of the manpower to be engaged, such as 3 passport size photographs, details of qualification along with proof, experience certificate etc. need to be submitted on demand at the time of award of the contract. The agency shall be fully responsible for ensuring the correctness of the documents submitted by the persons for engagement against various posts. The agency will also ensure the required representations of SCs/Sts/OBCs/PH candidates as per the list provided by the hospital at the time of awarding contract. The payment for rendering of the services is to be made only to the agency. Further staff to be provided as per recruitment rules of ESIC in terms of qualification & Manpower will be deployed as per reservation roster. An indicative list of qualification for various cadre is given as Annexure C. The Agency will provide undertaking from every person before deployment in the hospital as per Annexure B. The eligibility for the deployment of the manpower would be followed as per the guideline of Recruitment rules decided by ESI Corporation. The maximum of the manpower to be supplied would be 61 in different cadres and final deployment of the various category of staff will be as per the orders of Medical Superintendent, ESIC Hospital, Manesar. The information about the requirement along with the guidelines to be followed would be provided to the agencies at the time of awarding the contract. The Manpower engaged would work on the post of Staff Nurse, Nursing Orderly, Lab Assistant, OT Assistant, Jr. Radiographer, Pharmacist. In case more than one financial bid quoting the same service charges are received, the award of the contract will be decided on the bases of evaluation by committee constituted by the Medical Superintendent, Employees State Insurance Hospital, Manesar. The committee shall decide after verifying the premises, infrastructure, training facility and years of experience etc. The successful bidder will be required to execute an agreement on Rs. 100 Non Judicial stamp paper (purchased by successful bidder) within a period of 2 weeks from the date of issue of letter of award until extended by Medical Superintendent, Employees State Insurance Hospital, Manesar up to a maximum of another two week. Bid Security (EMD) shall be forfeited if the successful bidder refuses or neglects to execute the contract or fails to deposit the security deposit within the time frame specified i.e. within the fortnight of award of contract which can be extended by Medical Superintendent, ESI Hospital, Manesar up to a maximum of further two weeks on the written request of successful bidder.

The successful bidder shall be required to furnish a Performance Security in the form of Security Deposit within 15 days of receipt of Letter of Award for an amount of Rs. 5,00,000/- (Five Lakhs Only) in the form of Bank draft issued from any Nationalised/Commercial Bank, in favour of ESI Fund Account No. 1 payable at Manesar. The successful bidder can also apportion the Earnest Money towards the security deposit subject to payment of the balance amount. The Performance Security/ Security Deposit shall be returned within two months (without any interest) after the expiry of contract and completion of all contractual obligation.. In case the contract period is extended further, the validity of Performance Security shall stand automatically extended. The period of contract shall be of one year or till regular recruitment process is completed, whichever is earlier. Note:- Each page of the tender documents should be duly signed by the tenderer along with the stamp. The Medical Superintendent, ESI Corporation, Hospital Manesar reserves the right to relax/cancel any condition and right to accept or reject any or all the tenders without assigning any reason whatsoever. The Annexure A is financial bid which is to be put in separate sealed envelope superscribing as Financial Bid FOR PROVIDING NURSING & PARAMEDICAL STAFF ON CONTRACT BASIS.
