1 st Technical Workshop 17 th June 2011, Chennai The 1st Technical Workshop on CEMEx 2011 was held on 17th June 2011 at Dhanvantri Hall, Sri Ramachandra University, Porur, Chennai. The workshop was conducted by the United Nations Disaster Management Team (UNDMT), Sri Ramachandra University (SRU) and SRMC Emergency Physicians Alumni Association (SEPAA). The workshop was attended by 49 members from 16 different stakeholders of CEMEx 2011. Different stakeholders who attended the 1st Technical workshop are UNDMT, UNICEF, NDRF, Sri Ramachandra University, Apollo Group of Hospitals, Sundaram Medical Foundation, Dr. Mehta s Hospital, GVK EMRI 108 Ambulance Services, IIEMS Kottayam, CMC Vellore College of Nursing, AIDMI, OXFAM, RED R India, SEEDS India, VIT Vellore CDMM, and SRMC Emergency Physician Alumni Association. There was a unanimous decision from all stakeholders, that Sri Ramachandra University, Porur; Chennai will host all the academic activities of CEMEx 2011, Opening and Closing Ceremonies, Table top Exercise and Hot wash. YMCA grounds, Nandanam, Chennai would be the venue of the massive simulated disaster drill. After much deliberation by all stakeholders CEMEx 2011 was proposed to be held in the last week of July (27th to 31st July 2011) or 1st week of August (3rd to 7th August 2011). So the dates as 3rd to 7th August 2011 will be looked upon after due deliberation with the Govt of Tamil Nadu. And this will be officially communicated to all stakeholders by mail.
A FOCUS ON HUMANITARIAN AND MEDICAL RESPONSE 3-7 August 2011 Vannakam Chennai!! The nationwide migration of populations to large cities like Chennai has opened up unprecedented socioeconomic opportunities to millions across the country. It has also brought in its wake a significant threat to the urban millions that live in an increasingly volatile environment which is susceptible to both manmade and natural calamities leading to unprecedented humanitarian crises. This effort seeks to examine and strengthen urban disaster response in Chennai. Objectives The Chennai Emergency Management Exercise, scheduled between 3rd and 7th August 2011, is the result of a multi-institutional public-private partnership that is designed to harness the strengths and capabilities of regional emergency responders, educational institutions, hospitals, humanitarian agencies and state agencies as they prepare to confront urban catastrophes. With support of National Disaster Management Authority, Government of India & Government of Tamil Nadu, the initiative is anchored by United Nations Disaster Management Team (UNDMT), India in association with Corporation of Chennai, SRMC Emergency Physicians Alumni Association (SEPAA), Sri Ramachandra University, and a number of leading Private and Humanitarian Institutions. International faculty from the New York Fire Dept., Cornell & Columbia University s New York Presbyterian Hospital, USA, Harvard Affiliated Fellowship in Disaster Medicine, USA, and WHO SEARO, have consented to participate. This is a rigorous 5 days long event with 13 parallel tracks to be held at Sri Ramachandra University, Porur, Chennai for the first 3 ½ days of the event. The 4th day would be a table-top exercise followed by a massive major incident - simulation drill and a hotwash on the last day. Location of the Events Academic tracks, table top exercise, & Hotwash will be conducted at Sri Ramachandra University, Porur, Chennai. The Massive Major incident simulation drill will be conducted at YMCA grounds, Nandanam, Chennai. Hospitals participating in the drill are located in various parts of Chennai. This effort builds on prior successful initiatives undertaken by UNDMT collaborating on this project across India like Disaster Training and Drill for Ahmedabad Civil Hospital (2007), the Emergency Medicine Conference organized by NewYork-Presbyterian Hospital and Hinduja Hospital (Mumbai 2007), Project Mumbai VOICES, under the aegis of the National Knowledge Commission of India, University of Mumbai, All India Disaster Mitigation Institute and the Harvard Humanitarian Initiative (2006-08). As well as Mumbai Emergency Management Exercise- MEMEx-I conducted in the month of November 2008 and MEMEx II in South Mumbai in Month of December 2010. What will delegates at CEMEx learn? CEMEx is focused on thirteen training areas Disaster Response: The Humanitarian Perspective: 3 rd, 4 th, & 5 th August 2011 This track will have two sets of trainings, namely, Humanitarian Response Track focussing on Public Health (3 days) First Responder Training (2 days) It will involve sub-tracks on: Interaction of Hazards and vulnerability (Material, Organizational, Attitudinal) SOPs and approaches for Disaster Response - Public health approach to disaster management Application of Minimum Standards, Sphere & INEE Public Health Promotion and Mass communication Interagency coordination Public Health Assessment 4 th - 5 th August 2011 This course will teach senior level public health practitioners, community medicine PG s and disaster management personnel the skills needed to evaluate disaster response systems. The course will include an overview of various components of disaster response, and practical tools to evaluate these components. Around 60 participants would be present for this track.
Communication Workshop 4 th - 5 th August 2011 The Communication Workshop would focus on: Inter-agency communication: what information each agency would like from the other during disasters, Communicating effectively with the media, Advances in communication technology globally. Around 50 participants would be present for this track. School & Higher Education Preparedness 3 rd - 5 th August 2011 A two-day course on school disaster preparedness, and preparedness audits. This course is targeted at school administrators. Also there will be a separate track on higher education preparedness which will run for 1 day, which will target at the College superintendents /administrators, NSS and NCC Division Heads. This track will be managed by trainers from the All India Disaster Mitigation Institute (AIDMI), India and technically imparted jointly by RedR India. Around60 participants would be present for this track. Industrial and Chemical Disaster Preparedness 3 rd - 5 th August 2011 A two-day course on disaster preparedness, and preparedness audits for Industries. This course is targeted at administrators of various Industries at Suburban Chennai. Around 25 participants would be present for this track. There will be field drill at one of the industrial site too if possible on 5th August 2011. Around 50 participants would be present for this track. IT & Hotel Preparedness 4 th - 5 th August 2011 A two-day course on disaster preparedness, and preparedness audits for Work places like IT companies. This course is targeted at office administrators. Around 50 participants would be present for this track. Emergency Medical Services 3 rd - 6 th August 2011 This course has been carefully designed to give first responders - EMS or Paramedics, the confidence, skills and knowledge they need to provide the highest level of immediate care in a disaster setting. Beginning with basic but critical skills like CPR, log-rolling and extrication, participants will learn the theory and physiological basis of life support, early trauma management, and triage. Around 200 participants would participate. Emergency Nursing Care 3 rd - 6 th August 2011 This course has been carefully designed to give the nursing staff working in the emergency department, the confidence, skills and knowledge they need to provide the highest level of immediate care in a disaster setting in the Emergency Department. Around 100 participants would participate. Hospital Emergency Management 3 rd - 6 th August 2011 While EMS and other first responders provide initial trauma care in the disaster setting, the hospital will be the ultimate destination for majority of survivors. This course is aimed at improving the ability of hospital management to organize intra-hospital care during mass casualty events. With considerations of surge capacity and local physical conditions at the forefront, we will work with participants to develop unique plans to coordinate the delivery of care at all levels and effectively assess and mobilize hospital resources. This course is targeted at hospital administrators, Emergency Physicians and casualty medical officers. Around 50 participants would be trained in this track. ITLS ACCESS Course 3 rd - 5 th August 2011 ITLS ACCESS give EMS crews and first responders the training they need to reach, stabilize and extricate trapped patients. The updated course includes techniques for hybrid vehicles, trucks, buses and small aircraft in addition to its primary focus on traditional vehicles. The eighthour course is built around the concept of using hand tools commonly carried on an ambulance or first responder unit. A pre & post test will be conducted along with ITLS ACCESS provider certification. Around 50 participants would be present for this track. Comprehensive Trauma Life Support 4 th - 5 th August 2011 Comprehensive Trauma Life Support is an initiative of ITACCS India, supported by International Trauma Care (ITACCS). CTLS course is an authentic course on acute trauma management being organised in India. This course will be delivered over a 2 day period, similar to ATLS style but with case presentation / evidence based discussions / skill stations in a very interactive style. This course will teach participating doctors how to assess, resuscitate and deliver initial care in a systematic algorithmic manner to patients with major trauma. Around 50 doctors would participate. A pre & post test will be conducted along with CTLS provider certification.
Advanced Disaster Life Support (ADLS) Course 3 rd - 5 th August 2011 The Advanced Disaster Life Support (ADLS) course offers a more advanced practicum for those individuals who have completed the BDLS course. This intensive course trains participants in mass casualty decontamination, use of personal protective equipment, essential skills and mass casualty incident information systems and technology applications. Using simulated all-hazards scenarios, interactive sessions and drills with high-fidelity mannequins and volunteer patients, the course creates a true-to-life, practical experience in treatment and response. It is presented over two days and includes both classroom instruction and hands-on training distributed over four ADLS course training stations. A pre & post test will be conducted along with AMA ADLS provider certification. Around 50 participants would be present for this track. Table-top Simulation 6 th August 2011 On the day before the field drill, all participants, especially team leaders from various collaborating local agencies will meet along with the Disaster Management Cell of the Chennai Municipal Corporation to practice simulated scenarios to identify roadblocks in communication and execution, with a focus on the drill scheduled for the following day. What is the table-top exercise about? The table top is a unique exercise which has been developed over years of practice in various locations across the world. It incorporates lessons learnt from other cities in high, middle, and low income settings with and without many resources. It will bring together leaders of Chennai s response agencies: Fire, Police, NDRF and EMS, along with heads of hospitals, Doctors, Nurses, non-governmental organizations, and walk through a simulated disaster step by step. As the scenario unfolds, the facilitator will stop, and ask each group to propose in on their response strategy. Such exercises have proven to reveal the strengths and gaps in inter-agency communication and discrepancies and overlaps in response planning among the various stake holders. MEMEx II 10 th to 16 th Dec 2010 Mumbai A Mass Casualty Simulated Field Drill What will happen in the field drill? 7 th August 2011 On August 7th, a mass casualty accident will be simulated at YMCA Grounds Nandanam, Chennai, with over a 360 injured victims, and the city s response agencies will be alerted and police, fire, NDRF and EMS will respond. A triage area will be set up at the site of the disaster and all injured will be triaged, applying the skills learnt during the training session. Every single victim triaged will be monitored by local evaluators trained at CEMEx. The colour coded patients (red, yellow, green, black) will be transported by GVK EMRI 108 Ambulance Service to Govt. General Hospital, Park Town, Chennai, Govt. Kilpauk Medical College & Hospital - Kilpauk, Sri Ramachandra Medical College & Research Institute Porur, Sundaram Medical Foundation Anna Nagar, Apollo Hospital Greams Road, Apollo Hospital - Teynampet, Apollo First Med Kilpauk, Apollo Hospital - Tondiarpet, and Dr. Mehta s Hospitals - Chetpet, with realistic time delays built into the system. At each hospital, the hospital emergency plan will be set into motion, and doctors, nurses, stretchers, pharmacies, and equipments will be mobilized to accommodate the additional flow. Doctors trained at CEMEx will run the casualty wards applying their trauma care skills to patient care. The entire drill will be monitored by local evaluators, in addition to the faculty, and will be recorded on video. A final report will be prepared under the supervision of Chennai Municipal Corporation s Disaster Management leadership to examine areas of strength and improvement. Following the drill, will be a Hot Wash where all participants will self-scrutinize what worked and what didn t, and ways to make improvements. Needs Assessment Workshop Following the drill, a one day workshop will be conducted in Chennai that will bring together various local, national and regional stakeholders to identify current training initiatives across the country, and areas of greatest need in training and capacity building. This debriefing and needs assessment workshop will be organized by the project s collaborating partners, whose role would be to facilitate a lively interagency dialogue that would generate a priority list of training areas.
Deliverables This multi-disciplinary training and exercise will foster the development of skill sets focused on emergency management during mass casualty events, while providing an opportunity for local agencies to work together and practice a multidisciplinary, coordinated response to a local disaster. The train-the-trainer approach of the course and drill will enable local partners to eventually replicate the training and drill elsewhere in the region. Finally, the course and drill will help identify the strengths and weaknesses of the city s emergency management plans and help to better prepare the city for future events. These outcomes are summarized below: Trainings Hands-on training in hospital and pre-hospital emergency management, including mass casualty pre-hospital and hospital triage, medical stabilization and resuscitation, monitoring and evaluation. Hands on training on school emergency preparedness, as well as a effective and coordinated humanitarian response. Table-top Provides an opportunity for all of the stake holders to work together in a coordinated response to a city disaster. Field Drill Real-time simulation of a mass casualty event involving multi-agency coordination and response. This will drill into existing disaster plans in the public and private sector. Debriefing Post-drill hot-wash identifying the strengths and weaknesses in current plans. This will help to identify areas needing further training and capacity building. Evaluation Domestic and international experts in disaster management will be invited to observe and evaluate the entire exercise and to make recommendations for future improvement. Needs Assessment This workshop will prompt the subsequent implementation of a national survey of all current disaster related trainings in the country, followed by a compilation of proposed courses in deficient areas. This collaborative exercise and its evaluation component will be of tremendous value to both the local partners, as well as to the global emergency response community as we continue to learn from the successes and challenges of such trainings and drills. The hope is that this project will serve to create a locally driven, multi-disciplinary, sustainable commitment to emergency management activities and will, over time, become a validated model that can be easily reproduced in other cities and countries. Further in the near future it is hoped that the training will be able to facilitate and strengthen the following in Tamil Nadu: The model will be used to train and chisel the skills of medical and health members of the Search and Rescue Teams formed at several levels (state, institutional, municipal, taluka, and district and village level) The lessons learnt and methodology could get adapted for the regular mock drills which get conducted twice in the state at different levels. The current urban Risk reduction project of GOI UNDP has a component on hospital safety Several elements and resources used here can be replicated and appropriately utilized here. Some of the elements of this methodology, organizations involved, and curriculum can be replicated and used to strengthen the functioning of the Regional Disaster Management Centres existing in the state. The modules on emergency medical preparedness will further get replicated for various cadres of public health departments in the state. How does this benefit the city? The training and the drill bring together the major responders, testing real-time inter-agency communication and coordination at all levels of these organizations, especially the Incident Command Structure within Chennai Municipal Corporation and its various agencies, and the private sector and citizen groups. The drill pushes the medical response system to handling extreme scenarios, and compels the physicians and medical staff to prioritize care to maximize the savings of life and limbs, under duress. We also train in monitoring and evaluation using stateof-the-art international tools developed for evaluating disaster drills. This cadre of professionals can lend significant support to local planners in conducting and strengthening similar drills in the future.

Disaster Awareness Week In the Third week of JUNE, when CEMEx 2011 is being organised, we propose to organise Disaster awareness week from 25th to 31st July 2011 in Chennai City. During the Disaster Awareness week, plan to carry out a full page print on Disaster Awareness Literature in all major English and Regional Newspapers of Chennai. At the same time, plans are in place to collaborate with radio and television channels to have talk shows on disaster preparedness and a debate on some of the prime time news networks. In association with UNDMT, Sri Ramachandra University, Apollo group of Hospitals, Peace Rangers of Chennai, Red Cross India, Various local Universities, and SRMC Emergency Physicians Alumni Association is planning to organise rallies, road shows and surveys on Disaster Awareness in all the Major School and Universities of Chennai during the last week of July. A report on Disaster Awareness survey will be presented at the Opening Ceremony of CEMEx 2011. stakeholders Government of Tamil Nadu Govt. of Tamil Nadu has provided the whole hearted support to the cause. The state government in its pledge to invite organizations from the Non-Government and Government sectors to actively participate in the relief and rehabilitation exercise has left no stone unturned to make this workshop effective and successful. Govt. of Tamil Nadu s many initiative in disaster management are taking shape in very fruitful manner and CEMEx 2011 is just another chapter of such initiatives. Corporation of Chennai The Corporation of Chennai is the civic body that governs the city of Chennai. The Corporation of Chennai (previously Madras) is the Oldest Municipal Institution in India established on the 29th September 1688. The Madras Municipal Corporation Act, 1919 (as amended) provides the basic statutory authority for the administration now. The city has been growing in size and population. The numbers of territorial Divisions have grown from 30 in 1919 to 155. Now, population of the city has gone up from 5 lakhs in 1921 to approximately 42 lakhs now. Likewise the area has increased from 27.6 sq.kms in 1921 to 174 sq.kms. The Corporation has been taking earnest efforts to solve the multi-faced problems which have accompanied the rapid growth and expansion of the city. For the sake of administrative convenience the Corporation area is divided into 10 Zones. National Institute of Disaster Management A body under the Union Ministry of Home Affairs, the National Institute of Disaster Management (NIDM) is a Premier organization working for human resource development at national level in the area of disaster mitigation and management. It came into existence in October 2003 by a Government of India order upgrading the National Centre for Disaster Management (NCDM). It endeavours to work as a think tank for the government by providing policy advice and facilitating capacity building services including strategic learning, research, training, system development and expertise promotion for effective disaster preparedness and mitigation. United Nations Disaster Management Team, India The UNDMT plays a crucial coordination role in emergencies. The UNDMT is currently very active in the country in disaster preparedness, response and risk reduction with respect to natural disasters and CEMEx is another extension of the work it has committed itself to. The UNDMT in India was established in the 1990 s. Currently, ten UN agencies comprise the UN Disaster Management Team in the country: FAO (Food & Agriculture Organization), UNDP (United Nations Development Program), UNICEF (United Nations Children s Fund), UNESCO (United Nations Educational, Scientific and Cultural Organization), UNHCR (United Nations High Commission For Refugees), WFP (World Food Program), UNFPA (United Nations Population Fund), WHO (World Health Organisation), and UNAIDS (Joint UN Program on HIV / AIDS). UNICEF India UNICEF has been working in India since 1949. The largest UN organization in the country, UNICEF is fully committed to working with the Government of India. The magnitude of CEMEx and its seriousness is further stamped when UNICEF decided to lend its best possible support. The organisation s credibility in using quality research and data to understand issues, its resolutions are put to the best use. SRMC Emergency Physicians Alumni Association SRMC Emergency Physicians Alumni Association (SEPAA) is a non profit organization, with the main goal of Advancing Emergency Medicine in India. SEPAA was formed under the leadership of Prof. Dr. T. V. Ramakrishnan, on 1st July 2010. It was registered under the Tamil Nadu Societies Registration Act, 1975 on 14th October 2010 in South Chennai, Serial No. 505 of 2010. SEPAA represent about 80 Emergency Physicians who passed out of Sri Ramachandra Medical College, working together to make Standard Emergency Care available in all parts of the Country. Now the alumnus has a total of 50 members registered from all over India, Australia, Qatar, United Kingdom and USA. Sri Ramachandra University Sri Ramachandra Medical College and Research Institute was established by Sri Ramachandra Educational and Health Trust in the year 1985 as a private not for profit self-financing institution and dedicated to serve the society as a centre of excellence with emphasis on medical education, research and health care.
