Dear Family Member/Friend: Thank you for your interest in Mercy Center Nursing Unit. Mercy Center Nursing Unit is a 59 bed skilled nursing facility operated by the Sisters of Mercy on the campus of Misericordia University. The facility staff at Mercy Center is committed to providing the highest quality of care for an individual s physical, psycho-social, and spiritual needs. Professional staff assists residents and their families through a wide range of services. The current daily room rates are as follows: semi-private room $270.00/day; and a private room $275.00/day. Mercy Center Nursing Unit also accepts Medicare and Medicaid insurances when applicable. Please complete the Admission Application and kindly return it to the facility along with copies of insurance cards. As part of the admission process, Mercy Center Nursing Unit also requires that medical information about the applicant be released in order to determine if the facility can best meet the applicant s needs. \ Mercy Center Skilled Nursing Unit Information Sheet Rooms There are private and semi- private rooms in the Skilled Nursing Unit. The cost differs slightly between semi- private and private. A private room may not always be available upon admission but the staff will do their best to accommodate a resident s wishes as soon as possible. Residents are encouraged to bring to the facility those personal effects that will add to their comfort. We encourage residents to personalize their living area with pictures, knick- knacks, etc. Since rooms are fully furnished, furniture items must be pre- approved. Throw- rugs or carpeting are not permitted for safety reasons. Televisions are provided. Basic cable service is also provided as well as free wi- fi. Each room has a phone with internal access and the ability to receive calls. There is a small monthly fee for making local calls from the room and an additional per minute cost for long distance calls. There are also phones in the halls from which local calls can be made at no charge. As mentioned, hanging pictures is acceptable but we ask that maintenance provide assistance with this. Open flames and space heaters are prohibited by state law. Services In order to assure that a full complement of medical services are available to you, we have agreements for the following services: Physician s Service X- Ray Service Pharmacy Service Hospital Service Podiatry Service Laboratory Service Consulting Dentist Therapy Services Consulting Optometrist The fee for any other outside service provided is determined by the provider of the service and the insurance carrier: the fee is not the responsibility of Mercy Center Nursing Unit. The beauty salon is available five days a week.

Information Sheet Mercy Center Skilled Nursing Unit Information Sheet (Cont d) If you choose to avail yourself of our laundry services, soiled clothing (machine washable articles) is picked up and returned on a routine schedule. There is a nominal monthly charge for personal laundry service. Therapeutically planned diets, designed to meet your nutritional needs, are served daily. You are encouraged to enjoy the companionship of social dining; however, room service is also available, if needed. There is a nominal charge for a visitor s meal. Staff Many people work diligently to make your stay with us a pleasant and comfortable one. The staff are highly trained professionals dedicated to your good health and well- being. Each department is directed by an expert who will be glad to assist you in any way possible.

(570) 675-2131 PO Box 370, Lake Street Dallas, PA 18612 SCHEDULE OF CHARGES (continued) If a MEDICARE or MEDICAID resident prefers (rather than needs) and requests a special brand or product other than that supplied by Mercy Center Nursing Unit, the facility may only charge the resident the difference between the cost of the brand generally provided by the facility and the cost of the brand the resident requests, if the facility chooses to provide the requested brand. A resident cannot be charged for any service or item he/she did not request. THESE STANDARD CHARGES ARE APPLICABLE FOR ALL RESIDENTS OF MERCY CENTER NURSING UNIT: Beautician: $12.00 Wash & Set $17.00 Wash/Cut/Set $15.00 Wash/Cut/Blow Dry $35.00 Permanent $35.00 Color $12.00 Dry Cut $12.00 Wash/Blow Dry Transportation: $10.00 per trip (for round trips of 20 miles or less) plus $ 0.50 per additional mile The following items will be billed based on the type of service provided, duration of service, and/or number of units supplied: Laundry Physical Therapy Occupational Therapy Speech Therapy Durable Medical Equipment Pharmacy Diagnostic X-Ray Services Clinical Laboratory Testing Ambulance Medical Supplies (considered non-routine as dictated by Medicare - urology, wound care, etc.) Surgical Dressings, Splints, Casts, and Other Devices Prosthetic Devices such as colostomy (and other ostomy) bags and necessary accoutrements required for attachments Pneumococcal Pneumonia, Influenza Viruses, and Hepatitis B Vaccines These charges may be covered by your insurance.
