CROWNWeb and the MBI The Medicare Beneficiary Identifier (MBI) is now active on all patient screens in CROWNWeb 8
Dates of Death in CROWNWeb Background: The CROWN Data Discrepancy Support (CDDS) Team has completed entering missing Dates of Death in CROWNWeb from 2012-2017 using data from the REMIS system. Outcome: Facilities will see the death date populated, but will NOT be responsible for completing a CMS-2746 Form. Special Note: All 2746 forms for 2018 should be completed within 14 days of death date as per the CROWNWeb Data Management Guidelines. 9

Today s Presenter: Ahmar Wazir Ahmar Wazir is a member of the EQRS Product Management team. He has over 10 years of business and systems analysis experience, as well as project management experience with helping organizations achieve their full potential. Ahmar studied IT Project Management from University of Maryland University College. He is a certified Scrum Master, Scrum Product Owner, and Lean Six Sigma Green Belt. He has worked on the ESRD program for over 6 years. 11
EQRS 1.2 - Admit Date Question: We know that when a patient is admitted to a facility a Treatment Record is generated on that same date. How is CROWNWeb changing with the EQRS 1.2 release? 12
EQRS 1.2 - Admit Date Question: We know that when a patient is admitted to a facility a Treatment Record is generated on that same date. How is CROWNWeb changing with the EQRS 1.2 release? Answer: With EQRS 1.2, facilities will not be able to change the Admit Date to a date that is different than the Treatment Record date. That will ensure that they both agree. 13

EQRS 1.2 - Treatment Information and the CMS-2728 Form Question: How will CROWNWeb handle a Treatment Record that is associated with an existing 2728? Answer: Up until this release, yes, the facility could inadvertently delete a Treatment Record. However, as of the EQRS 1.2 release, a Treatment Record associated with a 2728 in either a Saved or Submitted status will not be able to be deleted. 15

EQRS 1.2 - Notifications in CROWNWeb Question: Will facilities continue to receive notifications when a patient receives their Medicare Claim Number? Answer: With the upcoming EQRS 1.2 release, a notification alert that a Medicare Claim Number exists for a patient in another external database such as REMIS, but not in CROWNWeb, will be autoaccepted. The patient attributes screen will automatically display the Medicare Claim Number. Users will not need to accept the notification and will not even see it. 17

Answer: When a Medicare Claim Number is auto-accepted for a patient, the Medicare Enrollment will be auto-updated to Currently Enrolled in Medicare Coverage. The Effective Date will be set as well. 20

Answer: Just as we described earlier with the Medicare Claim Number, when an MBI number is auto-accepted, the Medicare Enrollment will also be auto-updated to Currently Enrolled in Medicare Coverage. 23

Answer: All missing 2728s will now be displayed on the Facility Dashboard for patients who were admitted as a New ESRD Patient or had a Re-Entitlement 2728 beginning January 1, 2013. 25

Answer: When a patient is admitted and requires training within the first 90 days of initial dialysis or restarting dialysis, a Supplemental 2728 form will be triggered in CROWNWeb. 27
Update Prevent treatment deletion that is associated to an existing 2728 Admit Date Validation Description CROWNWeb will prohibit deletion of a treatment record if it is associated to either a saved or submitted form 2728 Users are prevented in both the SUI & EDI from editing the Admit Date to be after any associated Treatment Start Dates Auto-Accept Updates to HICNUM The CROWNWeb system auto accepts both New and Updated Health Insurance Claim Numbers (HICNUM). Medicare Certified Status automatically updating when HICNUM is accepted Medicare Certified Status automatically updating when MBI is accepted Allow the missing 2728 forms to display more than a year back Modify Supplemental CMS-2728 Form Triggers for all Cases EQRS 1.2 Updates When a patient is assigned a new HICNUM (via notification), their Medicare Enrollment Status is auto changed from No Coverage to Currently Enrolled in Medicare. The Effective Date will be set as well. When a patient is assigned a new MBI (via notification), their Medicare Enrollment Status is auto changed from No Coverage to Currently Enrolled in Medicare. The Effective Date will be set as well. Update the Facility Dashboard to count all missing forms not submitted to CROWNWeb as of 1-1-2013(Admission date for Initial 2728 and Re- Entitlement 2728 or Treatment Start Date for Supplemental 2728) up to current. CROWNWeb was modified to trigger supplemental 2728 forms regardless of the admit reason (except for acute scenarios). Previously 28 the form was not being triggered for Restart admissions.

Note: If the patient has Kt/V labs drawn again during the month, repeat the process. Enter the additional Kt/V lab date in the adequacy field, and then enter data from other sessions during the week in the Ultrafiltration fields. 34

Missed Treatments Question: A Monday/Wednesday/Friday patient has Kt/V labs drawn on Wednesday BUT missed their Friday session. What other treatment days during that week do I need to enter in the Ultrafiltration section? Answer: Monday You need to enter ultrafiltration data for any treatments at your facility during the week of Kt/V lab draws. If a patient does not attend any scheduled treatment at your facility during that week, do not enter that data enter only the data from treatments at your facility. 37

ESRD QIP Question: The ESRD QIP shows that Home-Hemo patients are excluded from the measure. Are there any plans to adjust the clinical screen to allow for this exclusion as UFR data is incredibly difficult to gather for patients dialyzing in their homes? Answer: CROWNWeb and QIP do not always have the same requirements. You are required to enter ultrafiltration for all hemodialysis patients in CROWNWeb, according to the ESRD QIP final rule. 46
