APPLICATION INFORMATION Pre-Licensure Application BEFORE YOU START YOUR APPLICATION This application is only for the Full-Time pre-licensure nursing program that begins in and continues through the Summer 2019 semester. The School of Nursing also admits students for the Full-Time and Part-Time tracks in the Spring and Fall terms. APPLICATION PROCESS Application Steps There are two separate application processes for the School of Nursing: 1. You must first be admitted to NSC as a pre-nursing major before you can apply to the School of Nursing. In order to be admitted to NSC, you must apply online and submit all of your official transcripts. Applications for admission to NSC take approximately 4-6 weeks to process. 2. Once you have been admitted to NSC as a pre-nursing major, you may then submit your application to the School of Nursing. Nursing Application Deadline Applications for the semester must be submitted and received at the School of Nursing by 4:30 PM on Friday, January 19, 2018. The School of Nursing is not responsible for late nor missing applications. Decision Notification Letters will be mailed to the mailing address provided on this application approximately 3 weeks after the application deadline. Letters cannot be emailed nor picked-up in person. The address provided on this application must also match the current mailing address listed in your MyNSC Student Center. Please review your MyNSC Student Center to verify that all of your current contact information is up-to-date. APPLICATION ASSISTANCE To fill out your application you will need to access your "Academic Requirements" page (Degree Audit) in your MyNSC Student Center. Not sure what this is? Go to www.youtube.com/nscacademicadvising and look for the video, "Where's My Degree Audit" Nursing Application Workshops The Academic Advising Center (AAC) and the School of Nursing are holding two information/open lab sessions on the application to help you complete your application properly. This is an opportunity to ask any questions, submit any degree audit discrepancies, and submit any exceptions to policies. Attendance is highly recommended. The sessions are scheduled for: Workshop 1 Date: Thursday, January 4, 2018 Time: 1:00-2:00 PM Location: NSE 255 Workshop 2 Date: Tuesday, January 16, 2018 Time: 11:00 AM - Noon Location: NSE 255 Space is limited to 60 people and is firstcome, first-served. DEGREE AUDIT REPORT: Submit a copy of your current NSC pre-nursing degree audit report. The report submitted with your application must include the grades received in all courses taken at NSC through the Winter 2018 session. TEAS RESULTS: Submit the results of all TEAS exam taken within the past 2 years. You may locate your results by logging into your account at www.atitesting.com, click on My Results, click on ATI TEAS to open your results, and click on Download Report (if taken more than once, you will need to do this for each separate exam). Section IV: Licensing Information These questions are asked prior to admission because they may also appear on the Application for Licensure with the State Board of Nevada. Applicants with affirmative responses should contact the Nevada State Board of Nursing directly. Although applicants will not necessarily be precluded from being admitted on the basis of prior felony convictions, they are advised that the professional licensing boards in the State of Nevada and elsewhere may refuse to issue a license if an individual has a felony or other conviction on his or her record. You are advised that, if you have been convicted of a criminal offense, other than a minor traffic offense, you will be required to disclose the nature of the offense, the court in which the conviction occurred, and what disposition occurred as a result of that offense. This disclosure must be made irrespective of whether you served a sentence and had your civil rights restored, or whether you have had the conviction(s) expunged from your record. A background check will be required for all students prior to clinicals. YES NO 1. Section V: Nursing Prerequisite Courses The School of Nursing requires that the following prerequisite courses be completed before starting the nursing program. You may have up to 12 nursing prerequisites credits still in progress during the Spring 2018 semester if applying for admission with the following conditions: 1) courses must only be taken at NSC; 2) BIOL 223, BIOL 224, and BIOL 251 must be completed at the time of application*. In the below table, indicate the status of your prerequisite courses: If you have completed the course, enter the grade received. Include the grades for any courses that may have not yet transferred to NSC and/or are pending course substitution requests. If you have not completed the course, indicate when and where you are taking the course.
