Submission Select the dates for the period covered by the report Select grant agreement period start and end date Use the pop up calendar when selecting dates. Do NOT input dates manually as you may use the incorrect format and this will not allow you to validate the report
Identification of Beneficiary Organisation & Partners These sections will be pre-filled It is important that we have accurate contact details for the beneficiary organisation and all partners e.g. address, contact details etc. If any details are incorrect you must inform us immediately upon receiving the form so that details can be corrected
Consortium Confirm and explain changes to the consortium give reasons for any changes, if applicable Confirm any changes to allocation of tasks and budget show any redistribution of finances Provide a summary of consortium meetings Include minutes of all consortium meetings within your annexes
Project Achievements and Results Comment on how the project met the projects initial objectives Describe the results achieved including: Changes to any results from the application stage Result type Language and mediums used Target groups and target sector Type of evaluation and findings Numbering of results must be consistent with the application form
Type of Transfer Describe the transfer strategy you used including the positive and negative aspects of the chosen transfer strategy State how the transfer of innovation has been implemented from a sectoral, geographical or other perspective Comment on the suitability and the success of your strategy Acknowledge and explain any differences from the application
Dissemination & Exploitation of the Results (1) Describe dissemination and exploitation activities taken place and indicate any changes from the application Indicate the target sectors/groups/organisations and number of participants targeted through each dissemination activity Identify reasons for choosing target organisations listed and relevance to project objectives Group similar dissemination activities within the report so that the form does not become too long
Dissemination & Exploitation of the Results (2) Describe the results and feedback from target groups or sectors Describe the aims of activities undertaken and comment on suitability and success of activities Include sample dissemination materials from all partners within your annexes Clearly reference annexes to the final report!!
Impact (1) Target groups and sectors Describe the impact of the project on target groups and/or sectors Include both quantitative and qualitative indicators you used Geographical Target Areas Describe impact on geographical areas including those outside the partnership if applicable
Impact (2) National VET Systems Impact on VET systems (UK and partner countries) Impact statistics Indicate on which target group the project had impact and quantify Indicate the educational field, level and economic sector the target group belonged
Sustainability Describe which institutions/organisations have expressed an interest in using the project results Number of people in target group who will benefit from the project results in a year from now in each partner country Describe the project partnership and the activities that are planned to be maintained after the end of the project Explain whether results will be exploited on a commercial basis
Contribution to EU Policies Leonardo European Priority Select the European Priority that you selected in the application form and comment on how your project contributed to this priority National Priorities List the National Priorities you targeted and describe how your project contributed to these LLP Horizontal Issues Tick the relevant horizontal issues that your project directly addressed Lisbon Key Competences Select the appropriate Lisbon key competences your project contributed to
Evaluation & Lessons Learnt Provide an evaluation on the following aspects: Usefulness of information Application and reporting forms Quality of Service Future participation and recommendation Lessons Learnt: Difficulties encountered and solutions found Comments and suggestions to the NA or the Commission on management and implementation of TOI projects
Annexes Minutes of all consortium meetings List of workshop participants, if applicable Copies of results (including login details if necessary) Dissemination materials (publications, leaflets, posters etc.) Any other relevant materials (e.g. project management, evaluation) Financial annexes (bank transfers, subcontracting, Summary Sheet) Remember to clearly reference annexes to the final report!! Submit annexes electronically where possible.
Declaration of Conformity Verify conformity of the report Declare you entered and updated information on ADAM database Legal representative to sign the declaration Remember the Final Report covers project activities and results for the WHOLE two year project duration
Submission Validate the e-form and if necessary fill in incomplete compulsory fields; Submit the form online; Print the form; Arrange for the Declaration of Conformity to be signed; Post the signed e-form (with financial tables (1 copy) and any hard copy annexes (2 copies)) to your Project Officer Email the submitted e-form (with financial tables and electronic annexes) to your Project Officer
Important Points to Remember Ensure consistency with the Grant Agreement (including the Application and Contracting Forms) and any approved Agreement Amendments Ensure results correspond with the descriptions and the numbering within the Agreement Remember to ensure consistency between annexes listed in the report and annexes submitted to the NA clear labelling is essential
Hints and Tips DOs Work with your partners to complete the Final Report Write as if the reader has no knowledge of your project Acknowledge and explain the differences from the application Clearly reference all additional documentation submitted Ensure financial information in the Final Report is consistent with the information in the Financial Tables Ask us if you are unsure about anything!
