The demand for value-based care is encouraging providers to adopt a completely different technique in delivering care to their population. An annual planned physical allows the provider the opportunity to screen, assess and conduct a physical exam on their MA patients in order to collect data on medical, psychosocial, functional capabilities, and limitations of this population.
