Message From the CEO and Board Chair Low-income residents of the Inland Empire have long faced significant barriers to receiving quality healthcare. One of the biggest barriers is an acute shortage of physicians. If residents can t get the care they need, when they need it, their health status declines and the entire community suffers. The overall costs of healthcare increase, particularly those costs associated with treating individuals with chronic or late-stage disease. Bradley P. Gilbert, M.D. Gary Ovitt Since its inception in 1996, IEHP s mission has been to help Inland Empire residents obtain the care they need. During our last three fiscal years, IEHP has continued to invest significant technical and financial resources to improve residents access to care. Those investments include growing our provider network, helping uninsured adults and families with children apply for Medi-Cal, encouraging greater utilization of preventive care services, promoting education and wellness, giving providers resources that enable them to better manage their patients care, and much more. Who We Are Inland Empire Health Plan (IEHP) is a local, not-for-profit, public health plan dedicated to improving the health of the community we serve. Organized as a Joint Powers Agency, we serve lowincome residents in Riverside and San Bernardino counties, through government-sponsored programs such as Medi-Cal, Cal MediConnect, Medicare Special Needs Plan, and Healthy Kids. Our members include adults, families with children, seniors and people with disabilities. In partnership with our network of almost 3,000 providers, we ensure that our community has access to convenient, affordable, quality healthcare. With more than 950,000 members in October 2014, and a projection to grow to one million members by 2015, IEHP is one of the largest not-for-profit public health plans in California. More than 99 percent of IEHP doctors would recommend IEHP to other doctors; more than 98 percent would recommend IEHP to their patients.* We re proud to have earned accreditation for our Medi-Cal program from the National Committee for Quality Assurance (NCQA), the nation s premier quality monitoring organization for health plans. And we re pleased to have been recognized by the Disabilities Rights California advocacy group for uniting and mobilizing organizations to better serve people with disabilities. 20 Years at a Glance When Inland Empire Health Plan (IEHP) began in 1996, healthcare was different than it is today. Back then, IEHP primarily helped families with children get Medi-Cal benefits. Improving Health and Access to Care for Our Members Ensuring Access to Care The Inland Empire has a significant shortage of primary care and specialty physicians. Despite this, IEHP has continuously grown our provider network, significantly improving access for residents who traditionally have had a difficult time finding providers who will accept Medi-Cal and other forms of government-sponsored health coverage. In less than 20 years, IEHP s network has grown from 676 providers to almost 4,000. Most are safety-net providers who understand the healthcare needs and challenges of our community. We also maintain a free 24-hour Nurse Advice Line, to meet the needs of residents after hours and on weekends. Trained nurses offer simple, useful advice that can help avoid long and stressful emergency room visits. Residents can also access numerous local urgent care clinics in our network when they need urgent after-hours care. Thanks to a strong desire to serve the community while ensuring access to care, the IEHP network continues to grow and flourish. We continue to add primary care physician groups and IPAs to meet the growing need for healthcare services. Hospitals 30 Total Providers 3,990 4

Coordinating Care We continually invest in resources to ensure Inland Empire residents get the help they need navigating the often confusing healthcare system. We re available online and by phone to help members find a provider, understand their health plan and plan benefits, access IEHP services, enroll in wellness and health education programs and more. We coordinate all medical, behavioral health, long-term care and home-and-community-based service needs. We also provide patient advocacy and case management, promoting healthy lifestyles, helping prevent critical care situations and reducing the overall cost of care. Health Navigator Program Established in 2010, IEHP s Health Navigator Program serves as a nationwide model for health plans and providers. Bilingual Health Navigators review members health records to identify any health needs, and then meet with members one-on-one in the comfort of their homes or another private location. The Health Navigator listens to the members needs and the barriers they face, and helps the member better understand their healthcare benefits and how to use them. Members also learn the importance of preventive care, how they can avoid going to the emergency room by calling the IEHP 24- Hour Nurse Advice Line or visiting urgent care clinics. After the initial visit, the Health Navigator conducts two more visits to continue education and to see if the members knowledge on how to access care has increased. To measure this, an assessment is done at the final visit. IEHP 2011-2014 Report to the Community 5

We invest significant funds each year in a comprehensive wellness program that assesses our members needs and designs programs geared toward building better habits and better health. We offer more than 18 different educational classes and at-home programs, each tailored to meet our member s unique needs. To maximize accessibility and learning preferences, members can choose in-person classes, printed materials or IEHP s audio telephone health line. Because changing long-practiced habits is more difficult for adults, we provide small incentives, such as movie tickets, when our members get immunizations and other preventive health measures when they should. It s all part of our commitment to improving community health. Partnering with Our Providers IEHP works in partnership with our providers to improve health and provide access to healthcare for Inland Empire residents. Preventive Care We commit more than $29 million annually in pay for performance incentives to providers, to ensure IEHP members receive immunizations and other preventive care on a timely basis. Preventive care can dramatically improve health by avoiding or delaying the onset of disease and slowing disease progression, while reducing costs. Autism IEHP is also providing $100,000 to assist in the creation of the Inland Empire Autism Assessment Center of Excellence to serve as a model for providing Autism Spectrum Disorder (ASD) assessments and treatment. Children in San Bernardino and Riverside counties often do not get diagnosed with ASD until 5-7 years of age, even though assessment and diagnosis can be reliably done at age 2. This leads to a significantly reduced quality of life, as well as lifelong medical care costs. From Research to Real Life Together with the Claremont Graduate School of Community and Global Health, the County of Riverside, and the University of California, Riverside School of Medicine, IEHP is working to create the non-profit Community Translational Research Institute (CiTRIs). CiTRIs will translate academic research into community-based programs and prevention strategies to improve health, focusing first on diabetes and obesity. IEHP has committed an initial $50,000 in start-up funding for the institute. IEHP 2011-2014 Report to the Community 7

Partnering With Our Providers Strengthening the Safety Net IEHP provides funding to support numerous safety-net providers who care for residents in medically underserved, low-income areas. For example, we committed $1.6 million to nine Federally Qualified Health Clinics that provide primary care in underserved communities of the Inland Empire. This funding helped to build primary care capacity, expand access to care for uninsured and Medi-Cal patients, and strengthen care delivery for those who need it most. Additionally, IEHP is working collaboratively with Riverside County s community-based family care centers to improve care delivery and coordination at its clinics. Part of this effort includes helping purchase technology that will identify patients with chronic conditions or those needing particular preventive services. Clinics will then be able to reach out to these patients and ensure they receive the care they need. IEHP has also set up an $8 million fund to help expand its Provider network by increasing the number of Primary Care Physicians and Specialists. 8 Working with healthcare providers who contract with or employ Primary Care Physicians and Specialists, the fund will cover up to 50 percent of the initial hiring and setup cost for new physicians. The fund will help support the delivery of care for up to 100,000 more IEHP members.

Championing Electronic Health Records Electronic health records (EHRs) have been shown to reduce patient medical errors, particularly medication errors, and their adoption has been directed by federal legislation. Since 2011, IEHP has funded about $150 million to support providers In 2011, IEHP provided $160,000 in start-up funding to establish the Inland Empire EHR Resource Center (IEEHRC), and supports the IEEHRC s ongoing operations with an additional $325,000 each year. The IEEHRC, developed in partnership with the Riverside County Medical Association and the San Bernardino County Medical Society, helps providers adopt and use EHRs, offers technical assistance for providers who have EHRs, and assists providers demonstrating their meaningful use of EHRs. IEHP also participates in the Inland Empire Health Information Exchange (IEHIE), a collaborative of hospitals, clinics, physician practices, health plans, and other healthcare providers. The IEHIE allows clinicians to securely access the medical records of more than five million Inland Empire residents when they seek care, regardless of where the previous care was provided. Such information sharing results in timely, safe and improved quality of care for patients. IEHP 2011-2014 Report to the Community 9

Tools and Support for Our Providers IEHP works hard to ensure providers have access to tools and services that allow them to provide the best care possible to Inland Empire residents. Electronic Health Care Our secure online portal allows providers to view the status of authorizations, claims and a wide variety of patient information, including health records, eligibility and lab results. They can also search for other IEHP network providers, including IEHP contracted urgent care centers, access formularies, view clinical practice guidelines, and more. Provider Call Center For fast telephone access to information, or to help resolve issues, we also offer a designated provider call center, which functions as a central clearinghouse for all provider calls. This includes online compliance training, web-based continuing medical education and IEHP University, held each year to ensure that our IPA and hospital partners have the information and tools necessary to best serve our members. Communication Information about our newest programs, incentive opportunities, and study results, together with reminders and updates on care guidelines, are disseminated through our provider and provider staff newsletters. 10

Community Engagement Building Healthier Communities We collaborate with community-based organizations to bring health education and information to our entire community. By joining forces with these organizations, we reach more of the common population we serve, while building stronger bonds with each other. During the past three years we ve participated in more than 300 community events, providing information about healthcare, the benefits of having health coverage, and what IEHP is all about. We often connect with existing IEHP members at these events, and use the opportunity to answer questions, ensure they understand their benefits, and share information about the wellness programs available to them. Our IEHP Superheroes program for kids is one innovative way we reach out, because we know good health habits start early. Super Nutricia rescues kids from the evils of junk food and teaches healthy eating. Eradicator helps kids say no to smoking. Rad Rider promotes bicycle safety. Children can watch and download videos and comic books, and meet the superheroes in person at community events. IEHP Superheroes program with Eradicator, Super Nutricia and Rad Rider for kids is an innovative way we reach out to our community. The IEHP Healthy Spin Game Show Traveling throughout the Inland Empire at community events, Healthy Spin features two popular Spanish-language television actors who serve as game show hosts. The hosts invite audience members up on stage to spin the big wheel to win prizes, while quizzing them on health and pop trivia. The show is a fun and exciting way for the community to learn more about IEHP s health coverage programs and the benefits of living a healthy life. IEHP 2011-2014 Report to the Community 11

Engagement The Member Advisory Committee allows members a place to voice their experience in getting care with IEHP, while learning about new programs and resources. Information Exchange Community-based organizations share our mission of helping people access quality, affordable healthcare services. With Our Members We encourage our members to establish a voice and share their opinions through our two member advisory committees: the Public Policy Participation Committee (for all members) and our Persons with a Disability Workgroup (for seniors and people with disabilities). Members share their experiences and discuss communication needs, educational priorities, healthcare services and coordination of care. Their recommendations guide us as we offer services. In turn, we share updates on state policy and other actions that may impact their healthcare. With Community Partners Community-based organizations share our mission of helping people access quality, affordable healthcare services. Our engagement with our partners includes: Updates on IEHP programs and services. Information on state and federal healthcare policies and their community impacts. Education on healthcare reform programs, including Covered California and the state s Medi-Cal expansion. Assistance applying for outreach grants. Event sponsorships. Providing educational materials for the community. 12

We maintain a reciprocal dialogue with our partners through four advisory committees: Inland Empire Healthcare Enrollers Enrollers play an important part in helping children and families sign up for the no-cost Medi-Cal Children s Percent Program. They also keep families informed about program changes, and help them maintain their health coverage. IEHP hosts a luncheon every quarter to keep enrollers informed. They receive healthcare news and updates on programs and services. IEHP Community Partner Network Meeting Each month more than 65 representatives from community-based organizations, service agencies, clinics and schools connect and collaborate on ways to help adults and families with children in the Inland Empire, with a key focus on enrolling uninsured in health programs. Inland Empire Disabilities Collaborative The IEDC brings together more than 540 members from Inland Empire organizations to promote equal opportunity and universal access for seniors and people with disabilities. Inland Empire Coordinated Care Initiative Advisory Committee This committee maximizes information exchange among providers, county agencies and non-profit community agencies serving people with both Medicare and Medi-Cal who are eligible for the new Cal MediConnect program. Insuring the Uninsured Our most important effort is helping the uninsured apply for health coverage. In each of the past three years we ve helped more than 10,000 children enroll in a public-sponsored health program, at community events and other venues. With the advent of the Medi-Cal expansion in January 2014, our focus is now on ensuring that as many adults as possible receive coverage. Adopt a Child, Family or Senior For some families, economic realities mean there s no money left for gifts at Christmas. In the last three years, IEHP employees have adopted 422 less fortunate children, seniors and adults, to make their holidays bright. Each one writes a wish list; IEHP employees take the list and go shopping, using their own money to buy the items. Children find toys, and often much-needed clothing, under the tree; adults often ask for food and basic household items. 14

IEHP plays an important role in supporting the economic health of the Inland Empire as one of its major employers. IEHP is one of the region s fastest-growing companies, currently employing more than 1,200 people. By providing a variety of employment opportunities, IEHP boosts the economic strength of the entire region. Employees living in the Inland Empire contribute to the stability of their communities through purchases of housing, food, clothing, entertainment, transportation, and other expenditures. IEHP also directly contributes to the economy through its own financial outlays, including payments to providers, office space, local supplier purchasing, and other operating costs. In addition to economic support, IEHP encourages employees to support the community through volunteerism.
