Adult School Scholarship Application Polk Education Foundation 2016 Scholarships offered to students attending Polk Adult Schools These awards are administered by the Polk Education Foundation. General/Personal Essay please type or computer generate Please attach to scholarship package. Use this essay to provide information requested by each individual scholarship; personal financial need information, college and career plans, and any other information you feel would help in the selection process. Things like medical problems, divorce, and special family situations could all be considered when awarding scholarships. Most scholarships will be chosen solely on the information contained in this essay, so be specific and state your need clearly. Honors, awards, clubs, volunteer time, leadership roles and extra-curricular activities may be included in your essay; however, do not send certificates, resumes or other documents as they are discarded and not considered. AFSCME member's name must be noted on application for verification. Minimum Community service hours is 200. Income is not considered, academics are the focus. Adult School students are exempt from Algebra II. AMSCOT Scholarship GPA: 3.0 SAT/ACT: Report Community service hours required: 200 Number awarded: Varies Scholarship amount: Varies Distribution of funds: 1-time payment Scholarship overview: Income is not considered, focus is on academic achievement. Student must have at least 200 community service hours. Adult school students exempt from community service. ELB Scholarship NEW! GPA: 3.0 SAT/ACT: report Community service hours required: n/a Number awarded: Varies Scholarship amount: Varies Distribution of funds: 1-time payment Scholarship overview: Student must have a 3.0-3.499 GPA. Must have taken either the SAT or ACT. One letter of reference required from a teacher or a coach. Additional essay required: Describe a struggle or difficult time in your life and how you worked through it. Preference given to students who have volunteered for the Eagle Lake BBQ Fest. 3

Student must have at least 100 community service hours. Selected students will be interviewed. George Jenkins Scholarship GPA: 3.00 SAT/ACT: report Number awarded: 5 Community service hours required: 100 Scholarship amount: $1000 Distribution of funds: 1-time payment Scholarship overview: Demonstrate financial need and academic achievement. Student must have at least 100 community service hours. Karla Speight Nursing Scholarship (for adult school students from the Fort Meade area) GPA: 2.50 SAT/ACT: NA Number awarded: 1 or 2 Scholarship amount: $300-$500 Distribution of funds: 1-time payment Scholarship overview: Student must plan to enter the field of nursing or related medical specialty. Funds may be used for extraordinary expenses related to classes. $300-$500 for students planning to attend PSC with the intent to become a nurse or related medical specialty. $500 for students who are enrolled in the PSC Nursing Program. Additional essay required: Please attached a personal letter from explaining why they want to become a nurse. Krater Family Scholarship GPA: 2.50 SAT/ACT: report Number awarded: 1 Scholarship amount: $3300 Distribution of funds: $1100/year for 3 years Scholarship overview: For students who are not eligible to receive academic scholarships. Student must be enrolled in college on a full-time basis. Income is not considered. Transcripts will be required to be sent to the Polk Educaton by mid-june of each year, following the initial year, to verify GPA and academic standing to continue receiving award. McDonalds of Polk County Scholarship GPA: 2.50 SAT/ACT: report Number awarded: 7 Scholarship amount: $1000 Distribution of funds: 1-time payment Scholarship overview: Student must demonstrate financial need. Must have been employed by McDonalds for at least 6 months prior to application. Letter required from supervisor confirming employment. Additional essay required: "What working at McDonalds means to me" 4

MIDFLORIDA Credit Union/Rebecca Keith Scholarship Scholarship type: Academic GPA: 3.00 SAT/ACT: report Number awarded: 16 Community service hours required: 100 Scholarship amount: $4000 Distribution of funds: $1000 per year/4 years Scholarship overview: Student or parent/guardian must have an active account with MIDFLORIDA Credit Union for six months. Must include account holder's full name on application. Student must have financial need and ability to do college work. Must have taken the SAT or ACT. Must have at least 100 community service hours. W.S. Badcock Academic Scholarship GPA: 3.50 SAT/ACT: 1210/28 Number awarded: Varying, not to exceed 4 Scholarship amount: $500 Distribution of funds: 1-time payment Scholarship overview: Report community service in general essay. Demonstrate financial need and academic achievement. Adult school students are eligible to apply. W.S. Badcock Career Education Scholarship GPA: 2.50 SAT/ACT: N/A Number awarded: Varies Scholarship amount: $500 Distribution of funds: 1-time payment Scholarship overview: Student must plan to attend an accredited career/technical center. Must demonstrate financial need and academic achievement. Describe community service in essay. Adult school students are eligible to apply. Woman's Club of Winter Haven Scholarship GPA: 3.00 SAT/ACT: NA Number awarded: 1 Community service hours required: 100 Scholarship amount: $ 4000 Distribution of funds: $1000 per yr / 4 yrs. Scholarship overview: Only female applicants. Student must demonstrate financial need. Student must have at least 100 community service hours to qualify. Student must maintain full time status in college/university and able to academically continue their degree path studies in order to continue receiving scholarship monies. Transcripts required to be sent to the Polk Education by mid-june of each year, following the initial award year, to verify full-time status and GPA requirement. 5

SCHOLARSHIP STATEMENT I understand that: (a) if my application is incomplete or does not contain all items from the required list, my application will not be considered, acknowledged or returned, (b) many scholarships will be decided based solely on the essay, (c those other than Polk Education Foundation staff may review my application for the selection process, (d) only those chosen to be interviewed will be notified of interview date/time, (e) if requested, I must attend an interview or be disqualified, (f) only the recipients of the scholarships will be notified by graduation. I further understand and agree that if I am chosen as a recipient: (a) I will attend the Senior Awards program at my high school to receive the award(s), (b) I will send a thank you note to the sponsor(s) of my scholarship(s) and a copy of the same thank you note to the Polk Education Foundation, (c) my scholarship check will not be released until my College Information Sheet AND a copy of my thank you note has been received by the Foundation office, (d) the Foundation may use my name and photo to publicize my award without any further compensation, (e) if I change colleges, it is my responsibility to request that the balance of funds be transferred back to the Polk Education Foundation to be reissued to the college I will be attending. I will notify the Polk Education Foundation of this in writing, (f) all monies received from a one-time award must be used within five years of my graduation. Any money remaining after that time will revert back to the Polk Education Foundation, (g) all scholarship checks will be sent on/about July 1 for Fall enrollment IF the above mentioned paperwork is returned to the Polk Education Foundation.
