WHO WE ARE Your Local Health Integration Network Toronto Central LHIN is responsible for planning, funding and integrating local health services that meet the needs of 1.3 million residents and tens of thousands of others who come to Toronto for care. Our Strategic Plan 2015-18 focuses on working with our residents, patients, caregivers, 170 health service providers, and other partners to build a local health system that is coordinated, easy to navigate and provides timely and equitable access to care. Supporting Primary Care Local Leads Primary Care Clinical Leads have been recruited into leadership positions in five sub-regions within the Toronto Central LHIN. These Leads will partner with primary care providers, other local health service providers and organizations to improve access to care, co-ordination and service integration. Our goal: To build a population-based, person-centred, and integrated health care system, with a focus on primary health care and community care What Happens Next? Primary care physician input is needed to help build the system of primary care that we need. Local primary and community care committees are being formed in each sub-region to guide, develop and implement improvements locally that will improve access and care coordination across the health care system overall. The five sub-regions within Toronto Central LHIN are useful for planning purposes and understanding population health, but are not boundaries. OUR SHARED PRIORITIES Through engagement with primary care providers, physician leaders have confirmed five priorities for system improvement: Attachment, access and 1 continuity of care Ensure that all residents of the Toronto Central LHIN have access to a primary care provider and are able to receive care in a timely manner. 2 Access to Inter- Professional Teams Improve access to inter-professional teams for all patients of primary care providers. Access to Specialist 3 Consultations Improve and streamline access to specialists. 5
HOSPITAL DISCHARGE 4 PLANNING Improve the quality and timeliness of discharge summaries and to expand enotification around patients who access emergency or inpatient care in hospital. 5 Secure Communication Expand to secure communication between providers to expedite follow-up, improve care coordination, and reduces duplication of tests. Secure provider/patient communication will also be explored in this work.
