Background who are we? Gippsland Primary Health Network (PHN) is a not for profit primary health organisation. Our vision is for a measurably healthier Gippsland. Our objectives are for: Better care. Better health. Better value. Improved coordination of care right care, right place, right time. Lower prevalence of prioritized health conditions. Background the grants program The 2017-18 Innovation Small Grants Program aims to support community based health promotion and prevention projects across Gippsland. Projects that are consumer-centred and/or driven by consumers are preferable, and community groups with innovative, sustainable ideas or existing projects are encouraged to apply. 9/7/2017 Insert Footer Here 6

Background the grants program There are two streams of funding available. Community: Up to $25,000 for health services Up to $5,000 for consumer-centred/community projects (auspice on behalf of community group encouraged) After Hours Primary Care: Up to $30,000 for health services or Consortia/partnership approached Projects delivered through these grants must be completed by 30 June 2018 Be able to demonstrate that the seed funding has enabled the project s activities to become part of business as usual activities therefore contributing to health system improvement for Gippsland. 9/7/2017 Insert Footer Here 7

Small Grants Program - will Organisations that are incorporated as not-for-profit entities For-profit entities such as sole traders, proprietary limited companies and public companies, provided the project proposal is not for profit. Organisations based in, or providing services to, people across Gippsland. Applicant organisations may form partnerships or a consortium with other organisations. The assessment process will give priority to applicants who address one or more of Gippsland PHN s identified health priorities, and/or national health priorities. Projects must take place within Gippsland PHN s boundaries. Projects must be completed by 30 June 2018 with final report by 15 July 2018. 9/7/2017 Insert Footer Here 8

Small Grants Program will not Projects which do not meet all eligibility criteria. Projects which are core activities or services of the applicant organisation. Projects or activities that are the primary responsibility of other funding bodies or government agencies. Projects of a commercial nature i.e. for profit, or that raise funds for philanthropic purposes for other organisations. Capital resources of equipment unless demonstrated that it is a vital part of the project and benefits the wider community. Events or conference sponsorship, prizes or awards ceremonies. Retrospective program activities, or expenses already incurred. Activities that do not occur within Gippsland PHN boundary.
