PURPOSE OF THE POSITION (i) (ii) (iii) (iv) (v) (vi) (vii) (viii) To provide specialist clinical nursing expertise to Head & Neck patients under the care of ORL and/or Plastic Surgery, across the perioperative journey at Counties Manukau District Health Board known as (CM Health) To promote excellence and innovation in clinical practice, ensuring the provision of high quality care that is validated by evidence based best practice. To work collaboratively across transdisciplinary teams and services through the continuum of care for the Head & Neck patient. Take a lead role and be actively involved in the communication process during the patient journey with particular emphasis on the inpatient component. Work closely with all members of the multidisciplinary team to ensure that Head & Neck patients receive seamless, patient/family centred care across the perioperative journey whilst strengthening integration initiatives already underway regionally and in localities of Counties Manukau. Provide nursing leadership and case management within the services in order to ensure patient treatment and care planning is comprehensively co-ordinated and informed by specialist knowledge, utilising care models that best meet clinical priority and service needs, working in in partnership with the patient, family/ whaanau. Provide case management and coordination of patients with complex conditions ensuring discharge planning is commenced early and works to a goal discharge date that is understood by the patient, family/whaanau and ongoing care teams Provide role modelling, education and support for the staff involved in the inpatient component of care for the Head & Neck patients.

NATURE AND SCOPE OF RESPONSIBILITIES Key Accountability Expectations Clinical Practice Demonstrate clinical nurse specialist expertise in the planning and delivery of patient care within the Head & Neck specialty. Utilise advanced clinical assessment, and critical thinking skills to identify potential or actual problems and collaborate across multidisciplinary teams, and services, to proactively address these. Participate in the review, redesign and implementation of Head & Neck referral guidelines and care pathways to better support patient care. Participate in the coordination of complex care that crosses several specialties and teams using a case management model. Liaise with the Faster Cancer Treatment team to ensure the pathway is well managed and process deficiencies addressed. Work collaboratively with all members of the health care team to ensure staff receive high quality education, based on research and best practise guidelines. Work collaboratively with the multi-disciplinary teams to ensure robust discharge planning is undertaken in a timely and appropriate timeframe Develop a robust handover process so the continuum of care is seamless, and timely to promote positive patient outcomes. Initiate and/or contribute to forums that critique current practice and treatment modalities, and debate topical issues Monitor national and international evidenced based research and trends and apply to practice as appropriate. Promote innovative and creative clinical nursing practice and ensure it is validated with best practice and research activities. Promote a patient/family/whanau focused approach by addressing practical aspects of care in response to patients needs thus ensuring great patient experience. Report monthly to Service Manager ORL and Plastic Surgery - keep CND advised on professionals issues Provide and maintain documentation including individual clinical records, referrals, request for advice and discharge. Provide timely and full information or advice back to referrers and/or in response to requests

Demonstrates broad knowledge of health policies, trends and directions with regard to Head & Neck surgery Provides monthly reports directly related to KPIs and including requests for advice and reports Establishes and maintains formal and informal networks at a national, regional and local level Actively involves patients and their experience in co-design of all aspects of the service. Professional Development Provide clinical advice and support for all staff working with Staff Head & Neck patients to promote practise development. Identify deficits in clinical and professional competencies of individuals and services and take appropriate action. Provide formal and informal education to staff within the clinical area and lead the process of reflective practice. Initiate teaching/learning strategies that enhance best practise and learning opportunities for colleagues. Identify potential for development in staff and assist them to progress through career pathway and/or credentialing processes Link in with Community Central and AT Risk Individual programme to provide advice and support across an integrated service. Link in with the Building Capability team to initiate teaching/learning strategies that enhance learning opportunities for staff.

Develop a website to support learning for staff, and for patients, around head and neck cancers and treatment modalities PROBLEM COMPLEXITY The Clinical Nurse Specialist Head & Neck is expected to manage a variety of complex and unpredictable problems that are associated with the assessment, treatment and management of Head & Neck patients. Coordination and case management of care, and appropriate services, need to be patient /family / whaanau focused.there will be a requirement to be able to prioritise issues and negotiate timeframes while providing a quality customer service. The Clinical Nurse Specialist Head & Neck will provide clinical expertise for patients seen in a variety of inpatient, outpatient settings, as well as utilising tele-health. SCOPE FOR ACTION The Clinical Nurse Specialist Head & Neck will be required to develop innovative approaches to problem solving and utilise the skills of other team members to co-ordinate, manage, and resolve problems. They will need to negotiate with and co-ordinate multidisciplinary teams across services within the organisation, and will frequently need to develop individual solutions for each issue and problem. The Clinical Nurse Specialist Head & Neck will be required to respond to referrals from regional and CMH emergency departments, outpatient clinics, wards, theatres, nursing, allied and medical staff. The Clinical Nurse Specialist Head & Neck will be required to provide nursing leadership in the speciality and contribute at an organisational and regional level including making recommendations for improvement in clinical practice within services related to quality and resource issues. This includes effective liaison/consultation with staff, appropriate external parties including patients and their families/ whaanau and contributing to effective training and development.

DIMENSION OF THE POSITION The Clinical Nurse Specialist Head & Neck is responsible to the Service Manager for achievement of objectives and goals for their service and the Clinical Nurse Director for professional nursing advice, guidance and direction. The Clinical Nurse Specialist Head & Neck will be required to provide an expert contribution at the organisational and regional level. They will make recommendations for improvement in clinical practice within the service related to quality, resource issues and performance development. This includes effective liaison with staff and appropriate external parties in localities, Co-ordination of available resources, optimisation of staff skills, as well as effective training and development. In addition to provision of direct clinical expertise and case management, the CNS Head & Neck will be the driver of service co-design and to facilitate integration of care in line with CMH strategies. PERSON SPECIFICATION Education Experience Skills / knowledge/ behaviour Minimum Registered Nurse with current practicing certificate Post registration course or significant clinical experience in specialty Post graduate diploma A minimum of 4-5 years post registration in surgical nursing with relevant Plastic Surgery, ORL Head & Neck nursing experience. Functioning at Proficient or Expert level on the PDRP.
