HISTORY OF Jimmy Fund Little League Little League Baseball and the Jimmy Fund are two organizations dedicated to providing children with the opportunity to become healthy adults. Participating Little League teams work together to raise funds for Dana-Farber Cancer Institute and the Jimmy Fund by fundraising and participating in local tournaments. Each year, Jimmy Fund Little League presented by Extra Innings and Franklin Sports provides more than 5,000 Little Leaguers and softball players in New England with the opportunity to continue to play baseball or softball after the conclusion of their regular season. On field, the program will provide these players with the opportunity to develop their athletic skills and further build their self-confidence. Off-field, these young heroes reach out into their communities to raise vital funds for the Jimmy Fund and, most importantly, help others by joining in the fight against cancer. For more than 50 years, Little League Baseball has supported Jimmy Fund s fight against cancer. Now in its 29th year, Jimmy Fund Little League is celebrating the hard work and winning spirit of the coaches, parents, and players who have participated in Jimmy Fund Little League, and have helped raise more than $4.3 million for adult and pediatric patient care and research at Dana-Farber. About Dana-Farber and the Jimmy Fund Dana-Farber Cancer Institute provides expert, compassionate care to children and adults and is home to groundbreaking cancer discoveries. The Institute is consistently ranked the number one cancer hospital in New England by U.S.News & World Report. The Jimmy Fund solely supports Dana-Farber Cancer Institute and has raised millions of dollars, since its founding in 1948, through thousands of community efforts to help save lives and give hope to cancer patients around the world. JIMMY FUND LITTLE LEAGUE DESIGNATED HITTER Tommy Tommy is a happy and creative 3rd grader from Leominster, MA. He loves to cook and bake, and wants to be a chef when he grows up. He also creates 3D loom figurines, plays video games, and loves to play with his siblings. Whether it is snuggling and watching movies, or playing pranks on each other, spending time with his family is what makes him happiest. Tommy s favorite baseball team is the Boston Red Sox and he got to meet Clay Bucholz. He is very excited to be this year s Jimmy Fund Little League Designated Hitter! Initially he was diagnosed with Optic Pathway/Chiasm Glioma at the age of two, and shortly after with Chronic Myeloid Leukemia (CML). Due to this, Tommy has spent much of his life in the Jimmy Fund Clinic, but he doesn t let that hold him back from being a kind and fun-loving 9-year-old. His smile rarely leaves his face and his positive attitude is contagious! Tommy has made many friends and is always looking to help others. He often will befriend new patients in the Jimmy Fund Clinic so that they feel more comfortable, and even wants to create videos to show young patients how to swallow pills and take their medicine. In the words of his mother Maria, Tommy is an amazing, giving little person. Thank you for raising money to help each child, adult, and family that comes to Dana-Farber. Your involvement with Jimmy Fund Little League allows us to help patients like Tommy and countless others. TOURNAMENT INFORMATION Date: July-August Registration Fee: $75 per team (100% tax-deductible) How to sign up: - Contact your District Administrator (If you are unsure of who your district administrator is go to JimmyFundLittleLeague.org, Find your team s district ) - Make check payable to The Massachusetts State Little League Fund - Send checks to your district administrator (entry fees help support Little League Baseball) Each district organizes a tournament following official Little League Baseball rules Players must be between 8 and 14 years old Maximum of 14 players per team The coach and/or a designated team parent(s) can oversee the participating team s fundraising efforts for the Jimmy Fund Each team and player fundraises for the Jimmy Fund and earn prizes based on their fundraising efforts 3

You can raise funds for the Jimmy Fund through any of the Grand Slam Fundraising Ideas on page 4, the Fun ways to Fundraise booklet, or you can get creative and think of something on your own! Complete your team roster with players names. Send the team roster and $75 entry fee to your league president by Monday, June 22, 2015. Distribute Jimmy Fund Little League supplies: baseball hats, fundraising posters, collection canisters, handbooks, pledge forms, and more to your team provided by Extra Innings and Franklin Sports If your team s online fundraising page has not already been created at JimmyFundLittleLeague.org, please send the team name, coach and/or fundraising coach s name and email to Casey Pereira at the Jimmy Fund - Encourage players to create and fundraise through their online page Communicate to players that they may raise funds by reaching out to friends, family, neighbors, and area businesses for gifts to the Jimmy Fund Money should be collected at the time of pledge (see page 15 for pledge form) Reminder: Coaches are also eligible to raise funds Collect all funds raised from each player and send to your league president no later than Wednesday August 12, 2015 with a team total The Jimmy Fund will send acknowledgements for all checks made payable to the Jimmy Fund Please do not send cash gifts any cash collected can be converted to a money order or cashier s check made payable to the Jimmy Fund 6

Tournament Management District Administrator Responsibilities Meet with the Little League presidents in your district to promote the program, discuss fundraising initiatives, encourage online fundraising, and encourage the selection of a Jimmy Fund Fundraising Coach to manage fundraising Complete the Supply Request Form sent to you by the Jimmy Fund by Monday, May 25, 2015 and email/mail/fax to: Jimmy Fund Little League Attn: Casey Pereira 10 Brookline Place West, 6th Floor Brookline, MA 02445 Email: casey_pereira@dfci.harvard.edu Fax: 617-582-8403 Send the District Fund Tracker form (page 16) of participating team names, and the coach s names, and email address to Casey Pereira at the Jimmy Fund by Monday, June 29, 2015. The Jimmy Fund can accept this information in any format (This form can also be used to track end of season totals.) Distribute Jimmy Fund Little League supplies: baseball hats, fundraising posters, collection canisters, handbooks, coach s packets, team rosters, and pledge forms to league presidents (see page 14 & 15) Oversee the tournament by keeping in contact with league presidents and coaches (See timeline for suggested outreach) In order to properly recognize the highest fundraisers please send the following to Casey Pereira at the Jimmy Fund by Friday, Aug. 14, 2014: - All fundraising proceeds and pledge forms - Top fundraising team form - Top individual fundraiser form Please ensure all fundraising checks are made payable to the Jimmy Fund Please do not send cash gifts any cash collected can be converted to a money order or cashier s check made payable to the Jimmy Fund 9

TOURNAMENT Prizes The information below is based on timely receipt of team/district paperwork and fundraising dollars. Please note that players and teams fundraise both online and offline. Fundraising for prizes will end Friday, August 14, 2015. Top Fundraising Teams The TOP FUNDRAISING TEAM from each district will receive: Tickets to the Saturday, Sept. 26 - Boston Red Sox vs. Baltimore Orioles game at Fenway Park A Jimmy Fund goody bag Recognition at the game on Fenway Park s center field JumboTron Districts must raise a minimum of $2,500 to send their top fundraising team to Fenway. The SECOND-HIGHEST FUNDRAISING TEAM from each district will attend a Lowell Spinners minor league baseball game and experience the game like a big leaguer as a guest of the Jimmy Fund. Each team will receive vouchers to be used at any 2015 Lowell Spinners game of their choice! Online Fundraising Teams EVERY PARTICIPANT who creates an individual online fundraising page on the Jimmy Fund Little League website will receive a Jimmy Fund Little League iron-on patch. THE FIRST 12 TEAMS to sign up online and raise $1,000 ONLINE will receive one hour in the batting cages as a team, or one private lesson as a team at an Extra Innings facility of your choice. THE TOP THREE TEAMS with the highest online fundraising totals by Friday, July 31, 2015* will receive 20 tickets each to ONE of the following two games. Tickets to be determined by the Jimmy Fund. Saturday, Aug. 8 North Shore Navigators in Lynn, MA Sunday, Aug. 9 Brockton Rox in Brockton, MA * (Players can continue to fundraise until 8/14/15 deadline. July 31st deadline is due to game dates.) 11

TOURNAMENT Prizes Top Individual Fundraisers District Level The top individual fundraiser from each district (only one per district) will receive the following: - Autographed Red Sox memorabilia - Official Jimmy Fund Baseball jersey - Participate in a check presentation on-field at Fenway Park on Saturday, September 26 (Two game tickets also included). The second highest individual fundraiser from each district (only one per district) will receive a $50 gift card to Dick s Sporting Goods. Top Individual Fundraisers Entire Program Level The top individual fundraiser from the entire Jimmy Fund Little League (the highest fundraiser from the entire group of top fundraisers) will win a batting spot at the 2016 Jimmy Fund Fantasy Day at Fenway Park. The second and third highest fundraising individuals from the top fundraisers of each district will receive an Extra Innings Grand Slam Player Pack at the Extra Innings facility of their choice. This prize includes: - One-half batting tunnel rental per week for a whole year - Five private lessons with a professional instructor - One baseball clinic - 10% off all Extra Innings Pro Shop purchases - Unlimited use of the Extra Innings members training area during the team s visit - Additional discounts on all lessons and clinics at an Extra Innings facility of their choice Top Fundraising District The district that raises the most money in 2015 will receive a coach s clinic at one of Extra Innings facilities. The clinic will be open to all coaches in the district who participated in Jimmy Fund Little League. Facility location is to be announced and the clinic must be conducted before the start of the 2016 season. The Jimmy Fund Little League Program fundraising for prizes will end Friday, August 14, 2015. District Tournament Champions The tournament winning team from each district will receive a Jimmy Fund Little League plaque for each player on the team after the championship game. 12
