Social Work Clinical Service Leader for professional issues Direct Reports : Nil Functional Relationships with : Internal Clinical Director, Renal Services Allied Health Professional Leader, Social Work NASC Renal Service management, renal MDT and Ward staff Allied Health Professionals and Nursing Social Work Teams Mo Wai Te Ora External Taikura Trust Other DHB staff Community Support Agencies Purpose : To provide a professional social work service for the patients of the Waitemata Renal Service, and the inpatient services as required, in accordance with the policies of Waitemata District Health Board and the ANZASW Standards of Practice. To provide cover across two hospital sites as required To have a collaborative and functional working relationship with the Renal Service staff, other inpatient staff and management KEY TASKS Receive and process referrals for social work intervention. Make accurate and timely assessments of the psychosocial issues, safety and risks for the patient on discharge and other concerns impacting upon patients and their families/whanau. All social work practice is ethical and of a high quality standard. Provide psycho/social assessments, review EXPECTED OUTCOMES Referrals are received, recorded and responded to as early as possible. Assessments are accurate, meaningful and achieved in the context of health services timeframes The social worker is registered and adheres to the ethics and standards of Aotearoa New Zealand Association of Social Workers and the New Zealand Social Work Registration Board Code of Conduct. To work as a key member of the renal multidisciplinary team in the management and support of patients and their families at all stages of the kidney disease continuum; Attend and contribute to family meetings for patients and their families to discuss renal replacement therapy issues. Maintain effective collegial relationships with NASC and other disciplines as a team member involved in the management of the health of renal patients Ensure that patients and/or families/whanau identifying as Maori are offered the services of Mo Wai te Ora Clients and their families from different ethnic and/or cultural backgrounds are offered appropriate cultural services. Maintain an up to date knowledge of networks, services and other resources available and beneficial to patients and/or their families/whanau. Participate in clinical, management and other meetings relevant to the delivery of social work services. Contribute to group work for specific rehabilitation groups as required. Maintain such clinical, administrative and statistical records of social work interventions and outcomes as are required. EXPECTED OUTCOMES accordance with ANZASW Standards of Practice Patients receive services to meet assessed needs in a timely way that ensures continuum of care. Patients with kidney disease enjoy the best quality of life possible for as long as possible Patients and their families are able to make informed choices regarding a renal replacement therapy modality (or palliative care) in a timely fashion which ensures smooth transition to renal replacement therapy, and maximizes patient independence and autonomy. Social work contributes fully with other clinicians in the care of patients. Maori have access to and can receive interventions from Maori cultural services Other cultural and/or ethnic groups have access to and can receive interventions from their specific cultural services Social work knowledge and practice is current and appropriate to the needs of patients and/or their families/whanau. Participation is active, timely and contributes to the overall care of patients. The skills and knowledge of social work contributes to group work. Records are accurate, timely and maintained to the required standard. Participate fully as a member of the social work and multidisciplinary teams, undertake continued professional development and contribute to the ongoing development of social work services. Relevant contributions are made and are consistent with the development social work services in Waitemata District Health Board, in accordance with ANZASW and NZSWRB Best Practice Standards l

PERSON SPECIFICATION POSITION TITLE: Social Worker, Medicine HOP & Surgical Qualification Experience Skills/Knowledge/ Behaviour Minimum Degree or Diploma (B Level) Social Work qualification. NZ SW Registration 2 years social work experience in health Experience of working with Maori clients, families/whanau Experience of work with with cultural groups other than Maori or Tai Iwi. Demonstrated commitment to the Treaty of Waitangi. Ability to form professional relationships with clients, colleagues and clinical staff. Knowledge of and involvement with Community Support Agencies. Demonstrated commitment to selfdevelopment. Self motivated Ability to be flexible and to cope with change, with a positive attitude Competent written and oral communication skills. Computer literate.
