Terms: "Initial Admissions" is the number of admissions for patients in the dataset for the calendar year that began a readmission chain. Note that the following types of patients are not counted as Initial Admissions: newborns, delivery cases, transplants, malignancies, HIV, major trauma, left against medical advice and selected other cases not considered by the 3M software. "Only Admissions" is the number of admissions for patients in the dataset for the calendar year where there was no subsequent readmission. The same types of patients removed above in Initial Admissions are also removed in the Only Admissions category. "Actual PPR Rate" is the Potentially Preventable Readmission rate, computed as Total Clinically Related Readmission Chains divided by [Initial Admissions + Only Admissions]. A clinically related readmission chain is a series of admissions for the same patient in a calendar year where the readmission(s) are clinically related. For example, if a patient had an admission then a readmission within 30 days of discharge from the first admission, this counts as one readmission chain. A test is then run to determine if the two cases in the chain are clinically related. Results: The top line in the table below shows how each managed care plan's Potentially Preventable Readmission Rate compares to the overall MCP average rate of 1.0. Below the bolded line, the PPR value for readmissions for each hospital is shown. The first column is the hospital's value weighted across all of the MCPs that it contracts with. Only those hospitals with at least 20 Initial Admissions statewide are shown. In subsequent columns, the value is shown for the hospital just for their business with each MCP. Since a hospital's volume may vary greatly across MCPs, the percentage columns represent the percent of all admissions the hospital had with each MCP. Hospitals that have cells highlighted in gray had less than 20 countable admissions in the time period with the MCP, so their statistics are not valid for public reporting. Hospital Name Ohio Department of Potentially Preventable Readmissions Report Card for Managed Care Plans (MCPs) Results Shown for Readmissions that Occurred During Calendar Year 2014 (January 1, 2014 December 31, 2014) " PPR Rate" is also Clinically Related Readmission Chains divided by Initial Admissions. The PPR Rate differs from the Actual PPR Rate, however, in the number of Clinically Related Readmissions Chains used in the numerator. In the PPR Rate computation, the actual Total Clinically Related Readmission Chains are converted into expected Total Clinically Related Readmission Chains by factoring in for each hospital its mix of adult and pediatric cases by DRG when compared to the statewide mix. It also considers patients with a mental health comorbidity when compared to the statewide mix. The " Ratio", therefore, is the Actual PPR Rate divided by the PPR Rate. If the Ratio is = 1.0, this means the hospital had readmissions within 30 days at a rate that is expected given their patient mix. If the Ratio is less than 1.0, this means the hospital had less readmissions within 30 days than is expected given their patient mix.
