In TrakCare, data is oriented around the patient with the information you re looking for accessible from a single screen. Sophisticated workflow capabilities enable clinicians to readily view their most important priorities, see outstanding tasks, and collaborate with care teams improving productivity and reducing cognitive burden. InterSystems TrakCare is developed for the Internet era. It lets authorized healthcare professionals access and work with complete and up-to-date patient records on any Internet-connected device tablets, smartphones, or computers. TrakCare automatically detects the type of device in use to optimize specific functionality, such as touch on a tablet or smartphone. Being an Internet-based system, TrakCare provides a lower cost of ownership less expensive to run and easier to support. TrakCare provides analytics that are easy to access because they are embedded in its modules. Analytics are performed on patient records that have been aggregated by TrakCare, so clinicians and administrators are able to make fully informed decisions based on complete and up-to-date information.

Real-time management information is provided through reports, graphs, and dashboards. Users see predefined data models for clinical and administrative data, and can track their key performance indicators (KPI). With TrakCare s preconfigured dashboards and the ability to drill down on dashboard content, clinicians and administrators are able to track using current data what is happening operationally and can take more timely actions. TrakCare is one global product but comes pre-configured with ready-to-run workflows, EPR charts, screen layouts and other key system elements to meet the needs of local markets. This reduces implementation time and risk, and improves user adoption and satisfaction. The table below indicates how TrakCare modules relate to each other. Core Within TrakCare Electronic Patient Record (EPR) captures the continuum of clinical and administrative information about a patient and is provided with every TrakCare implementation. The EPR contains patient demographics, medical history, previous admission, surgery and obstetric information. The medical history includes allergies, disease, family and social history. The EPR is automatically populated by relevant data entered in any available TrakCare module or from integrated legacy healthcare applications. The type of data available will depend on the information sources connected, but may include laboratory and medical imaging orders and results, prescribed medications and discharge summaries. Master Patient Index (MPI) enables healthcare organizations to record an extensive range of socio-demographic and essential medical information about a patient. A single record of all registered patients is created wherein each patient is uniquely identified allowing information to be efficiently organized, patients to be tracked across the organization and information to be easily extracted from records when needed. The MPI underpins the TrakCare patient-centric Electronic Patient Record (EPR) and is an integral part of each TrakCare module. TrakCare Foundations TrakCare PAS (Patient Administration System) Foundation provides core administrative management functionality for all patient populations managed in TrakCare. It includes Inpatients, Outpatients, Medical Records and Coding and an administrative electronic patient record with allergies, alerts, diagnosis entry, episode lists, order history and patient history. PAS automates the administrative workflows associated with inpatient, outpatient and emergency patients to ensure the needs of institutional and community based healthcare organisations are met. Functionality to address specific needs of maternity wards and operating theaters is provided and comprehensive admission, discharge, transfer, resource scheduling and appointment booking and bed management functionality are key features.

TrakCare CIS (Clinical Information System) Foundation delivers the core functionality required for clinical patient management. It includes: Care Provider Work-lists and Workbenches, Care Planning and Documentation, and a clinical electronic patient record with observations and doctor and nurse assessment. CIS allows care providers to organize and manage their daily work beginning with identification of what is being done and remains to be be done for patient under care, determining the patients current status, and taking action and documenting new care activity. TrakCare Departmental Modules TrakCare Community Health gives healthcare professionals the tools needed to provide communitybased patients/clients with the same high quality of care they would receive if admitted to a hospital or when cared for anywhere else. It provides secure access to information from the patient s journey across the entire continuum of care, in one single electronic patient record and manages the unique needs of care and service delivery in a diverse range of community settings such as the patient s home, clinics, schools, commercial spaces, and even call centers. The daily planning for staff and resources, and the recording of administrative and clinical details related to the patient/client are easily accommodated, including medical history, alerts, referral information, appointment bookings, assessments, and creating new care plans. TrakCare Emergency Department facilitates efficient triage and treatment of patients during an emergency attendance. It performs registration, movement and capture of care interventions, and ensures coordination and efficient management of emergency rooms, resources, and clinical staff. TrakCare Laboratory provides comprehensive order processing, specimen and request registration, analysis, results reporting, accounts receivable, referred work functionality and quality control to meet all the operational needs of the modern clinical pathology laboratory and streamlines the diverse activities of the specialized departments within a lab. It supports clinical biochemistry, microbiology, histology/cytology, immunology, serology, hematology, blood transfusion medicine, phlebotomy. TrakCare Radiology streamlines the work processes of a medical imaging department, allowing for easy and efficient order processing, authorization, and scheduling. It also provides pre- and post-examination details, result reporting, transcription, and verification and distribution. TrakCare Operating Theater enables efficient coordination and management of theater rooms, resources and clinical staff, and provides easy to use documentation tools for recording pre-operative, intra-operative, and postoperative surgical and anesthetic case details. An integrated theater resource scheduling, request and booking facility and surgical preferences that include orders and interventions which are pre-planned according to the scheduled procedure and assigned surgical team, ensure workflow is streamlined and data entry minimized. TrakCare Operating Theater supports daily planning and activities in either centralized or decentralized booking environments. TrakCare Anesthesia* delivers a comprehensive anesthetic record of all activity for a procedure during which the patient is anesthetized. Clinical activity including note taking, progressive medication administration and physical measurements. Observation entries are recorded using a flow sheet concept. There is also tight integration with suppliers of bio-medical device connectivity solutions. An inter-operative graphical time line for recording and tracking progress through procedures and capturing key events is also provided. TrakCare ICU* provides functionality dedicated to ICU departments including ICU Flow Sheets for charting, tight integration with suppliers of bio-medical device connectivity solutions, a series of ICU specific assessments e.g. intravascular access, and shift assessment. As this functionality is integrated in TrakCare, all relevant ICU patient data is part of the EPR. TrakCare Maternity dynamically records administrative and clinical data during the ante-natal, intra-partum, and postnatal periods of a pregnancy. A complete obstetric history, including planning and monitoring of care for both the mother and the baby, is facilitated with antenatal appointments as well as post-birth community interactions including discharge support, baby clinic, and vaccination services.

TrakCare Oncology* supports the treatment and management of oncology patients. It includes dedicated oncology functionality in the Electronic Patient Record, enhanced tumor staging, and oncology-specific interactive clinical pathways. Pathways can be coordinated across different care settings and between different care providers and teams. Interactive chemo/radiation/general care plans that are modifiable "on the fly" provide full visibility to ongoing orders and all aspects of progress from an oncology team homepage. TrakCare Pharmacy supports the dispensing and clinical activities of non-retail pharmacy departments within hospitals. It manages prescription/medication review, controlled drugs, drug manufacture and dispensing to meet clinical intent. Workflows for Accepting, Substituting, Rejecting, Packing, Collecting, Returning, Checking, and Disposing of medications and prescriptions are provided. Pharmacy also enables formulary management and pharmacy stock control. Pharmacy links to other TrakCare modules so patient data, both administrative and clinical, can be viewed and assessed by pharmacy staff in the same way that other clinicians see it. A full range of decision support is available subject to the implemented drug database. TrakCare Add-on Modules TrakCare Ordering and Results Communication provides an efficient and safe mechanism for ordering diagnostic tests, treatments, and other clinical interventions or services, together with functionality to document and share results or interpretations issued against an order. Items can be ordered individually or in predefined sets, and frequently requested items can be identified as favorites for quick selection. Configurable alerts and security settings protect patient safety and ensure required data is captured in the most efficient manner. Orders can be automatically distributed to processing or receiving locations using configurable rules. Orders can be linked to diagnosis and predefined against appointments to streamline service delivery. Result Entry tools and views are the basis for recording and communicating information about tests, results and patient care decisions in an efficient and effective manner. TrakCare Medication Prescribing and Administration brings together patient data and decision support capabilities to enable safe and efficient electronic prescribing and administration. Extensive alerts and other checks help to avoid medication errors and order duplication. The use of order favorites and default prescription details makes the process fast. A streamlined process provides the ability to administer any type of medication from the nurse work list or from the EPR medication chart. Delivery of the right medication to the right patient at the right time, and in the right dose and route, is supported by the use of barcode administration at the point of care to enhance safety and reduce the risk of adverse drug events. Full prescription details are accessible to the nurse, including notes entered by the prescriber or pharmacist, and any interactions or alerts. The electronic medication record is accessible to all those involved with the patients care from the EPR. TrakCare Vaccinations provides a comprehensive overview of a patient's current vaccination status as measured against defined vaccinations schedules (e.g., travel, child, adult, employment). The EPR Vaccination chart provides a single place from where vaccination history can be viewed and new vaccinations can be quickly ordered (when linked to TrakCare Ordering), administered and fully documented. Alerts notify care providers when vaccinations are due, thereby improving both vaccination rates and patient safety, and adverse event records can be entered should a patient react negatively. Vaccinations that have been given using TrakCare, as well as those provided by external care providers can be recorded, ensuring a complete record is available. TrakCare Genogram enables care providers to access and create a structured, visual representation of the relationships between patients and their family members, household relations or other close personal connections. Recording both the nature of relationships and conditions that may affect individuals, the genogram enables care providers to explore the cause of certain problems between individuals. This is particularly useful in primary and community models of care delivery where a holistic approach is required. TrakCare Active Clinical Notes provides a central work space from where all patient clinical activity can be initiated. It facilitates contextual clinical documentation through a powerful yet simple user interface that is organized around the interaction of a clinician with a patient during an encounter. Active Clinical Notes presents a problem-oriented and timeline-based view that leverages advanced terminology and natural language capabilities. TrakCare Family Health supports social and community care health programs to promote services such as prevention, recovery, rehabilitation, and maintenance of good health in the context of the family group and/or all occupants of a particular dwelling. With Family Health, teams comprised of doctors, nurses, dentists, and other health workers can efficiently manage the delivery of healthcare services to all residents of dwellings within a defined geographic region, including the scheduling of events such as group health promotion and individual or group appointments. A 'residence' record captures important information about accommodation standards and environment that can impact health and includes a record of all household or resident members. Residents can be linked to multiple residences and all residents of a single dwelling are also linked making it easy to identify and locate individuals who may be impacted by health issues associated with a specific residence or location.

TrakCare Mental Health Administration provides clinical and administrative functionality to record, track, manage, and keep a history of a patient s legal status, including recording detainment for assessment and/or treatment, consent status, forms and reviews specific to government regulations. TrakCare Referral and Waiting List Management is used to create, receive, review/triage and process electronic and paper-based referrals from external and internal sources. Referrals can be rejected or accepted, and prioritized onto waiting lists or by booking outpatient appointments or inpatient admissions. A patient s waiting list, recall entry, and any appointments or services generated from a referral can be linked using a unique referral pathway identifier. TrakCare Discharge Summary simplifies the process of providing relevant patient and clinical information for the continuation of care as patients transition from one provider or care setting to another. It provides rich flexibility for configuring content and format, and supports a wide array of options for automated or manual distribution. TrakCare Billing enables staff to coordinate and manage bills for different patient types including inpatient, same-day patient, outpatient, and emergency, as well as for services such as allied health, community, radiology, and dental. TrakCare Active Decision Support provides a fully integrated decision engine for creating user-defined targeted rules that drive decision support actions within TrakCare. Using advanced 'Visual Rules' technology, Active Decision Support assists care providers to gather clinical information, aid them in decision making and actively elicit actions to be taken. TrakCare Mobile extends TrakCare to mobile devices with touch-optimized user interfaces for phone and tablet users. Functionality availability includes the ability to enter or view clinical notes, diagnoses, vital signs, orders, images, results, and other patient data. TrakCare Extended Analytics provides near real-time access to management and performance information through reports, graphs and dashboards. Based on InterSystems DeepSee technology that enables active analytics to be embedded in transactional applications, TrakCare Extended Analytics includes predefined data models and key performance indicators for clinical and administrative data throughout the TrakCare system. TrakCare Government Standards modules assist customers in producing government reports, connecting to key government applications and systems, and fulfilling other government mandates. Each module includes a countryspecific list of reports, interfaces and other capabilities. TrakCare EPR Connectivity is used to connect third-party information systems, and devices to TrakCare. EPR Connectivity includes the full range of TrakCare HL7 message types, and the TrakCare Healthcare Messaging Framework for generating and consuming CDA in support of standards-based interoperability. TrakCare Specialty Interfaces connect TrakCare with specialized data sources such as third-party clinical content. TrakCare Lab External Ordering/Results enables external users to order laboratory tests and view results via a simple Web interface. TrakCare Additional Reports/Dashboards enables the purchase of client-specific reports or dashboards, developed by InterSystems, to supplement those provided as part of TrakCare. *Available in TrakCare release T2015
